In this paper, an approach on the dynamic rollover characteristics has been introduced and explained using some of the latest information from NHTSA. A new NCAP test has been announced by NHTSA including Static Stability Factor and Fish-Hook test which is a dynamic test for the prediction of dynamic rollover propensity. From 2004, a new star rating on rollover propensity results of new cars has been issued by NHTSA to US customer. Until now the rating is an only reference data for the purchasing process of customer. Basically, it is not a regulation item of car makers, but information for the selecting their own cars. This rating is an index for the rollover risk for each vehicle. In the near future, this rating may be a critical data for the sales of cars in US, especially, for SUV. Therefore, we have to understand the status of rollover related research in the world and have to prepare with the change of market requirements and information of customer. This dynamic rollover related topic is a research area of vehicle dynamics parts before crash and rollover. The star rating on the rollover is based on the huge rollover crash information and vehicle basic dimension like as tread and height of center of gravity data. Also, a dynamic test result has been adopted as a scaling factor for the compensation of static criteria for the decision making on the rollover propensity. The start point on this approach for rollover related topic is the exact measuring basic dimensions and the clear understanding of rollover mechanics based on vehicle dynamics. We have to make a development strategy for the consideration of rollover at the early design stage before the major freezing. Finally, basic rollover mechanics, parameters of dynamic rollover, strategy on rollover mechanics, and the state-of-the-art of dynamic rollover related topics are introduced in this paper.

School sport provides a structured setting for teaching life skills. Although the research has focused mainly on coaches, sport psychology consultants (SPCs) can also foster positive development in student-athletes. The objective of this study was to examine the experiences of SPCs in teaching life skills development and transfer to student-athletes in sport-study programs specific to high school settings. A basic qualitative study consistent with a constructivist paradigm was used. Semi-structured interviews were held with seven SPCs employed in a sport-study program. First, results revealed that all SPCs held a holistic philosophy and aimed for student-athletes’ positive development. Second, SPCs incorporated some life skills development and transfer strategies into their interventions (e.g., workshops, group discussions). Third, SPCs encountered certain challenges in teaching life skills development and transfer, and particularly in following up on life skills transfer and working with other stakeholders (e.g., psychoeducators, social workers). Despite the SPCs’ efforts to teach life skills development and transfer, the results highlight the need for better training in effective interventions.

<P><B>Abstract</B></P> <P><B>Background</B></P> <P>Exercise stress echocardiography (ESE) can be used to identify left ventricular (LV) dysfunction in asymptomatic chronic MR. NT-proBNP is the best marker for monitoring LV dysfunction.</P> <P><B>Objective</B></P> <P>The aim of this study was to estimate the complementary prognostic value of ESE and NT-proBNP in asymptomatic degenerative MR with preserved LV ejection fraction (EF).</P> <P><B>Methods</B></P> <P>Symptom-limited treadmill ESE was performed in 114 asymptomatic with significant degenerative MR (ERO >20mm, RV >30ml), LV end-systolic diameter <40mm and preserved LV function (EF >60%). Post-exercise EF increase of >4% was defined as contractile reserve (CR)+.</P> <P><B>Results</B></P> <P>MV operation was performed in 19 (16.7%) and new-onset LV systolic dysfunction was developed in 23 (20.2%) patients over 3.5±1.5years. Based on ROC curve analysis, a NT-pro BNP of 100 was deemed the most relevant cutoff value to predict primary outcome with Youden's index=131.84. In sequential Cox models, a model based on clinical data and resting echocardiography variables (χ<SUP>2</SUP> =6.87) was improved by NT-proBNP (χ<SUP>2</SUP> =13.9) and presence of CR in ESE (χ<SUP>2</SUP> =20.8; <I>p</I> =0.0002).</P> <P><B>Conclusions</B></P> <P>In asymptomatic moderate to severe or severe degenerative MR and preserved LVEF, the presence of CR in ESE and NT-proBNP provide important incremental clinical determinants. In particular, the prognosis is markedly poor for those with high NT-proBNP but with absence of CR than low NT-proBNP with presence of CR.</P>

Objective: We aimed to compare the aortic valve area (AVA) calculated using fast high-resolution three-dimensional (3D) magnetic resonance (MR) image acquisition with that of the conventional two-dimensional (2D) cine MR technique. Materials and Methods: We included 139 consecutive patients (mean age ± standard deviation [SD], 68.5 ± 9.4 years) with aortic valvular stenosis (AS) and 21 asymptomatic controls (52.3 ± 14.2 years). High-resolution T2-prepared 3D steady-state free precession (SSFP) images (2.0 mm slice thickness, 10 contiguous slices) for 3D planimetry (3DP) were acquired with a single breath hold during mid-systole. 2D SSFP cine MR images (6.0 mm slice thickness) for 2D planimetry (2DP) were also obtained at three aortic valve levels. The calculations for the effective AVA based on the MR images were compared with the transthoracic echocardiographic (TTE) measurements using the continuity equation. Results: The mean AVA ± SD derived by 3DP, 2DP, and TTE in the AS group were 0.81 ± 0.26 cm2, 0.82 ± 0.34 cm2, and 0.80 ± 0.26 cm2, respectively (p = 0.366). The intra-observer agreement was higher for 3DP than 2DP in one observer: intraclass correlation coefficient (ICC) of 0.95 (95% confidence interval [CI], 0.94–0.97) and 0.87 (95% CI, 0.82–0.91), respectively, for observer 1 and 0.97 (95% CI, 0.96–0.98) and 0.98 (95% CI, 0.97–0.99), respectively, for observer 2. Inter-observer agreement was similar between 3DP and 2DP, with the ICC of 0.92 (95% CI, 0.89–0.94) and 0.91 (95% CI, 0.88–0.93), respectively. 3DP-derived AVA showed a slightly higher agreement with AVA measured by TTE than the 2DP-derived AVA, with the ICC of 0.87 (95% CI, 0.82–0.91) vs. 0.85 (95% CI, 0.79–0.89). Conclusion: High-resolution 3D MR image acquisition, with single-breath-hold SSFP sequences, gave AVA measurement with low observer variability that correlated highly with those obtained by TTE.

Background/Aims: Several studies have suggested that surgical resection (SR) can provide a survival benefit over transarterial chemoembolization (TACE) for hepatocellular carcinoma (HCC) at the intermediate stage according to the Barcelona Clinic Liver Cancer (BCLC) staging system. However, the criteria for SR remain to be determined. This study compared the long-term outcome of intermediate-stage HCC patients treated by either TACE or SR as a primary treatment modality, with the aim of identifying the patient subgroup that gained a survival benefit by either modality. Methods: In total, 277 BCLC intermediate-stage HCC patients treated by either TACE (N=225) or SR (N=52) were analyzed. Results: The overall median survival time was significantly better for SR than TACE (61 vs. 30 months, P=0.002). Decision-tree analysis divided patients into seven nodes based on tumor size and number, serum alpha-fetoprotein (AFP) level, and Child-Pugh score, and these were then simplified into four subgroups (B1.B4) based on similarities in the overall hazard rate. SR provided a significant survival benefit in subgroup B2, characterized by ‘oligo’ (2.4) nodules of intermediate size (5.10 cm) when the AFP levels was <400 ng/ml, or ‘oligo’ (2.4) nodules of small to intermediate size (<10 cm) plus a Child-Pugh score of 5 when the AFP level was ≥400 ng/mL (median survival 73 vs. 28 months for SR vs. TACE respectively; P=0.014). The survival rate did not differ significantly between SR and TACE in the other subgroups (B1 and B3). Conclusions: SR provided a survival benefit over TACE in intermediate-stage HCC, especially for patients meeting certain criteria. Re-establishing the criteria for optimal treatment modalities in this stage of HCC is needed to improve survival rates. (Clin Mol Hepatol 2016;22:250-258)

<P><B>Background and Aims</B></P> <P>Endoscopic submucosal dissection (ESD) has been widely accepted for treating superficial esophageal squamous cell carcinoma (SESCC). However, long-term outcomes of ESD and esophagectomy for SESCC have not been compared. We compared the clinical outcomes of ESD and esophagectomy in a matched cohort.</P> <P><B>Methods</B></P> <P>Patients who underwent ESD and esophagectomy for SESCC were included. We selected SESCCs without obvious submucosal invasion from the surgical database by reviewing endoscopic images. To minimize the effect of selection bias, propensity score matching was performed. Overall survival (OS), disease-specific survival (DSS), recurrence-free survival (RFS), and metachronous RFS were compared between the 2 groups. Adverse event rates were also compared.</P> <P><B>Results</B></P> <P>In a matched cohort of 120 pairs, OS, DSS, and RFS were comparable between the 2 groups. The 5-year OS, DSS, and RFS rates were 93.9% versus 91.2%, 100% versus 97.4%, and 92.8% versus 95.3% for the ESD and esophagectomy groups, respectively. The metachronous RFS was worse in the ESD group than in the esophagectomy group (<I>P</I> = .004). The 5-year metachronous RFS rates were 90.3% versus 100% for the ESD and esophagectomy groups, respectively. The esophagectomy group showed a higher overall adverse event rate than the ESD group (55.5% vs 18.5%, <I>P</I> < .0001). In each subgroup of mucosal and submucosal cancer, OS, DSS, and RFS were also comparable between the 2 groups.</P> <P><B>Conclusions</B></P> <P>ESD provides long-term outcomes comparable with esophagectomy in patients with SESCC without endoscopic evidence of obvious submucosal invasion. ESD should be considered as the first-line treatment for these patients.</P>

<P>Conclusions: Complete revascularisation of lesions with >70% diameter stenosis in vessels 22.5 mm is better than ICR, and is a reasonable goal for patients with multivessel disease.</P>

This study explored the perceptions of Korean university students studying English as a foreign language (EFL) after using the AI chatbot tool ChatGPT in an advanced writing class for one semester. A questionnaire was used to collect data from students about three learner variables (being comfortable in completing a task, being motivated to complete a task, and being confident that they can complete a task). The questionnaire also asked students if they felt their learning experience was improved by using AI tools (as opposed to classes taught only using traditional methods) and if they thought a blended method using both AI tools and traditional methods was preferable to one using strictly one or the other. The results showed that students were much more comfortable writing in English with the assistance of AI writing tools, that they felt less anxiety while completing tasks with the assistance of AI writing tools, and that they felt more confident when writing in English with the assistance of AI writing tools. They also indicated that using AI writing tools in the classroom was preferable to classes taught only in the traditional method, and that classes using a blended method were preferable to those taught using strictly AI tools or traditional methods alone.
