        Dipyridamole 부하 심근 SPECT에서 Dipyridamole의 작용과 부작용

        N/A Dipyridamole is an agent that may be used to noninvasively evaluate coronary artery disease. The effect of dipyridamole infusion its generally related to its induced peripheral vasodilatory effect. In normal person, heart rate is generally increased slightly while blood pressure decrease, but the achieved double product and related myocardial oxygen consumption have no significant change. The purpose of this study is to examine the effect and side effect of dipyridamole, and to compare different response to dipyridamole among the patients. We evaluated 847 patients who underwent dipyridamole stress myocardial SPECT. 93.6% of them had induced hypotension 0.9% showed no change of blood pressure, 5.5% had increased blood pressure 8.3% had no change of pulse rate more than 10% of basal pulse rate. Among diabetes, 16.9% was not change of pulse rate, 6.7% in non-diabetes. There was no significant correlation between age and rate pressure product rest(RPPr), in patients without perfusion defects on SPECT(y=7.1x+48.4r=0.13 p>0.01). As increasing age, RPPs/RPPr was declined(y=-11.6x+68.9 r=0.17 p〈0.01), similar results were obtained in patients with perfusion defect. The size of perfusion defect on myocardial SPECT have no correlation between RPPr and RPPs/RPPr. The side effects of dipyridamole included chest pain and chest tightness, headache, abdominal pain, dizzness, nausea, and dyspnea. As increasing age, dipyridamole-induced cardiac work at rest was increased, cardiac response to dipyridamole was decreased.

        경제위기와 한국의 무역경쟁력:1997년 외환위기와 2008년 금융위기의 비교

        7)ㅃ This paper looked into how Korean trade has been changing in the process of suffering two economic crises. The number of items to be analyzed was 159, ranging from SITC 5 to 8. They were measured in terms of the sum of imports and exports and balance of trade and then were compared to see how they differed in the process of two economic crises. Also, the 159 items to be analyzed were sorted again into 16 industries according to the properties of goods. Trade Specification Index(TSI) was measured for each industry, showing how they have changed in two economic crises. According to the result of the analysis, the way TSI of industrial products has changed since the mid-1990s was very similar to how won-dollar exchange rate has changed. In general, price competitiveness rises when the won-dollar exchange rate goes up. It then brings about trade surplus, which leads to the rise in TSI. The result of the comparison and analysis in this paper showed that the TSI rose more sharply in 1997 foreign exchange crisis than in 2008 global financial crisis. It is attributed to the effect of the rise in the foreign exchange rate. Another reason is that, amid the global financial crisis in 2008 when the world-wide depression was going on, Korean export suffered comparatively more than in 1997. The overall Korean trade competitiveness rose, though temporarily, while undergoing two economic crises. However, the monthly TSI which each industry shows in the two economic crises is inconsistent and shows such a wide range of fluctuation, which makes it hard to make a simple comparison. As such, only some unusual industries were mentioned in this paper. The classification of relevant industrial products according to the level of technology also showed that the 1997 crisis had a positive impact on the export competitiveness by and large, whereas there was not much influence in the case of the year 2008 except low-and-middle technology industry.

      • FC 2-8 : Factors associated with inflammatory change of epidermal cyst

        Background: Epidermal cysts (EC) are the most common tumors found on the skin and accompanying inflammation is also frequent. However, the predictors for inflammatory change have not been investigated yet. Objectives: This study was performed to investigate the difference between inflamed and uninflamed epidermal cysts, and to assess associated factors for inflammatory change. Methods: We analyzed the biopsy-proven 1723 epidermal cysts (515 inflamed and 1208 uninflamed) at two tertiary hospitals from 2011 to 2021. Results: The proportion of inflamed epidermal cysts (IEC) and uninflamed epidermal cyst (UIEC) were 29.9 % and 70.1 %, respectively. Inflammatory change was less frequently observed on the face (25.5 %) than the other sites (32.1 %) (p=0.0046). The anatomic location with the highest rate of inflammatory change was back (40.8 %, p=0.0004). Mean duration of inflammatory change from UIEC to IEC on the face was shorter than the other sites (12.5 months vs 17.9 months, respectively, p= 0.0473). On ultrasonography, long axis of IEC (20.7mm) was generally longer than that of UIEC (15.7mm) (p=0.0218). Moreover, Inflammatory change was more frequent on the EC with punctum compared to those without punctum (35.3 % vs 30.3 %, P=0.0471). Conclusion: Based on the location, size and presence of punctum, decision on the optimal treatment strategies can be established when encountering patients with EC.

        조기 천장골염의 자기공명영상 소견

        Objective: The early detection of sacroiliitis on X-ray is important to diagnose spondyloarthropathy(SpA). The conventional radiography, bone scan, and computer tomogram have the problems of sensitivity, specificity, and side effects. We examined sacroiliitis using MR imaging in patients who had inflammatory back pain, but no definite changes in conventional radiogrphy. Other radiologic techniques were compared in detection of early sacroillitis. Methods: Twenty six patients with SpA were examined. The MRI findings of both sacroiliac joints were evaluated in every patients, such as pannus, periarticular osteitis and fat accumulation, subchondral sclerosis, and joint space changes(irregularity, ankylosis). Also we analyzed the changes of sacroiliac joints in sacroiliitis. Results: The predominant sites of involvement were upper aspect and iliac surface of sacroiliac joint in subchondral sclerosis, lower and iliac in pannus, periarticular fat accumulation, and iliac in periarticular osteitis. The MPGR images were superior to spin echo images in evaluation of joint space changes(irregularity, and ankylosis). Conclusions: Sacroiliitis is initiated in iliac side and lower portion of sacroiliac joints. MRI can detect early changes of sacroiliitis in patients with no conventional radiographic changes. MRI seems to be needed in patients with inflammatory back pain.

        무증상 갑상선기능저하증 환자에서 Thyroxine 치료에 따른 혈청 지질 및 아포지단백의 변화

        Background: Subclinical hypothyroidism(SCH) is a common biochemical abnormality which can be found in routine screening tests of thyroid function. We are increasingly faced with the question of whether its an indication for thyroxine replacement therapy. The effect of thyroxine replacement on lipid profile in SCH has aroused a great interest because of an association of overt hypothyroidism(OVH) with hyperlipidemia and increased risk of coronary artery disease. Method: We prospectively evaluated the changes in lipids and apoproteins before and after thyroxine replacement therapy in 23 patients with SCH and in 37 patients with OVH. We measured serum total cholesterol and triglyceride using autoanalyzer, high density lipoprotein(HDL) chole-sterol by dextran sulfate method, Apo A1 and Apo B by immunonephelometric assay. Results: Thyroxine replacement therapy significantly decreased total cholesterol, low density lipoprotein(LDL) cholesterol and apo B levels, but did not affect the level of triglyceride, HDL cholesterol or apo AI in patients with OVH. In SCH, thyroxine replacement therapy with the doses to normalize serum TSH concentrations also decreased significantly the level of cholesterol and LDL cholesterol albeit apo B levels did not change. Moreover, in most of patients with OVH (11 of 12) and in all of patients with SCH(5 of 5) who had had hyperchlesterolemia before treatment, thyroxine replament normalized their cholesterol and LDL cholesterol levels. Conclusion: In regard to the beneficial changes in blood lipid levels, patients with SCH should be treated, especially in cases who have other risk factors for the development of atherosclerosis. If thyroxine replacement only will reduce the incidence of coronary artery disease in SCH remains to be elucidated by long-term prospective studies(J Kor Soc Endocrinol 11:41-51, 1996).

        Derivation and verification of an aerosol dynamics expression for the below-cloud scavenging process using the moment method

        <P><B>Abstract</B></P><P>An aerosol dynamics equation for the below-cloud scavenging process considering phoretic and electric charging effects in addition to the conventional mechanisms (the Brownian diffusion, interception, and impaction) is developed by using the moment method. Then, the dynamics of particle size distribution by the below-cloud scavenging process is calculated by using the developed equation and verified with the measurement data. The calculated particle size distribution changes are quite small compared to the measured changes. The calculated removal rate is smaller by 10<SUP>−2</SUP>–10<SUP>−3</SUP> than the measured data when only the conventional mechanisms are considered. With the extended mechanisms, the scavenging coefficient increases upto 20 times, mainly for the particle size range of 0.1μm<<I>d<SUB>p</SUB></I><3.0μm. However, the difference between the calculated and measured scavenging coefficient is still large, especially, for <I>d<SUB>p</SUB></I><0.1μm. Other possible scavenging mechanisms that might affect the below-cloud scavenging process such as coagulation and condensational growth of hygroscopic particles, turbulence, and updraft into cloud are discussed. It is recommended that further studies on wet scavenging process are needed.</P>

        강원도 남부지역에서 소나무림 벌채 후 초기 종조성 변화

        강원도 남부 지역의 발달단계가 다른 소나무림 벌채지에서 16년 동안의 식생 변화를 연구하였다. 교란 후, 천이 궤적 및 환경 변화를 온도, 상대습도, 서열분석(Detrended Correspondence Analysis, DCA), Multiple Responses Permutation Procedure(MRPP) 및 지표종 분석(Indicator Species Analysis, ISPAN) 등을 통하여 분석하였다. 벌채 직후, 수관열림도는 대조구와 비교하여 세 배(1yr 68.3% 및 R1 23.0%)로 급격히 증가한 후 감소하였으나, 상대습도는 비교적 작은 변화(약 6% 감소)를 나타내었다. DCA 결과, 벌채 후 초기에는 종조성이 이질적으로 변화하였으나, 점차 벌채 이전(대조구)의 것과 유사하게 발달하였다. MRPP 결과, 각 발달 단계(1yr, 3yr, 10yr 및 16yr)의 종 조성은 대조구(R1, R3, R10 및 R16)의 것과 유의한 차이를 보였다. 1yr 및 3yr에서는 목본성 종이, 10yr 및 16yr에서는 초본 종들이 주요 종으로 분석되어, 교란 직후에는 목본성 종들이 활발한 재생을 나타내며, 이후 초본 식생이 발달하는 것을 보였다. 연구 대상지역의 천이는 초기 식물상 조성 모델로 설명할 수 있을 것으로 판단된다. Vegetation changes were studied for 16 yr in clearcut logged Pinus densiflora forests in the southern Gangwon-do province in Korea by applying chronosequence approach. Ambient temperature and relative humidity, Detrended Correspondence Analysis (DCA), Multiple Responses Permutation Procedure (MRPP), Indicator Species Analysis (ISPAN) were used to examine successional trajectory and compositional changes. After clearcutting, canopy openness was increased abruptly at three folds (1yr 68.3% and R1 23.0%) and then decreased, but relative moisture was slightly decreased (6%) compare to control site. In the result of DCA, right after clear cutting, vegetation composition was developed heterogeneously compared to control sites, and then approached to control sites within 16 years. Based on MRPP, species composition of each developmental stages (1yr, 3yr, 10yr and 16yr) revealed signigicant differences to that of control vegetation (R1, R3, R10 and R16). Indicator species in 1yr and 3yr samples included various woody species rather than herbaceous species, but in 10yr and 16yr, herbaceous were more abundant. Earlier succession of pine forests likely can explain to Initial Floristic Composition (IFC) Model.

        악안면 성장예측 모형개발에 관한 연구 : 6세 어린이의 1년간 악안면성장에 관한 연구

        The purpose of this study was to obtain the data on the maxillofacial growth changes of the Korean six-year-old children in a year to establish the maxillofacial growth prediction model. Dental arch stone models of the 682 students in 1995 and of the same students in 1996 were analysed. The maxillofacial growth changes of the students during one year were found by the analysing the dental arch stone models. The dental arch growth changes in a year were analysed by student t-test and the qunantity of growth per a year of dental arch lengths and widths by paired t-test. The obtained results were as follows: 1. It was found that all dental arch lengths and widths had grown during one year. 2. There is no difference between male and female students in maxillofacial growth changes. 3. Maxilla had grown a little more than Mandible during one year. 4. Dental arch lengths had grown a little more than dental arch widths during one year.

      • Gene profile of fibroblasts identify relation of CCL8 with idiopathic pulmonary fibrosis

        <P><B>Background</B></P><P>Idiopathic pulmonary fibrosis (IPF) is characterized by the complex interaction of cells involved in chronic inflammation and fibrosis. Global gene expression of a homogenous cell population will identify novel candidate genes.</P><P><B>Methods</B></P><P>Gene expression of fibroblasts derived from lung tissues (8 IPF and 4 controls) was profiled, and ontology and functional pathway were analyzed in the genes exhibiting >2 absolute fold changes with <I>p</I>-values < 0.05. <I>CCL8</I> mRNA and protein levels were quantified using real-time PCR and ELISA. <I>CCL8</I> localization was evaluated by immunofluorescence staining.</P><P><B>Results</B></P><P>One hundred seventy eight genes differentially expressed and 15 genes exhibited >10-fold change. Among them, 13 were novel in relation with IPF. <I>CCL8</I> expression was 22.8-fold higher in IPF fibroblasts. The levels of <I>CCL8</I> mRNA and protein were 3 and 9-fold higher in 14 IPF fibroblasts than those in 10 control fibroblasts by real-time PCR and ELISA (<I>p</I> = 0.022 and <I>p</I> = 0.026, respectively). The <I>CCL8</I> concentrations in BAL fluid was significantly higher in 86 patients with IPF than those in 41 controls, and other interstitial lung diseases including non-specific interstitial pneumonia (<I>n</I> = 22), hypersensitivity pneumonitis (<I>n</I> = 20) and sarcoidosis (<I>n</I> = 19) (<I>p</I> < 0.005, respectively). Cut-off values of 2.29 pg/mL and 0.43 pg/mL possessed 80.2 and 70.7% accuracy for the discrimination of IPF from NC and the other lung diseases, respectively. IPF subjects with <I>CCL8</I> levels >28.61 pg/mL showed shorter survival compared to those with lower levels (<I>p</I> = 0.012). <I>CCL8</I> was expressed by α-SMA-positive cells in the interstitium of IPF.</P><P><B>Conclusions</B></P><P>Transcriptome analysis identified several novel IPF-related genes.
