        미세-전단 결합 강도 시험을 이용한 상아질 접착제와 수복용 복합 레진의 호환성에 관한 연구

        The purpose of this study was to compare and to evaluate the combination use of 5 kinds of dentin adhesiye systems and 5 kinds of composite resins using micro-shear bond test. Five adhesive systems (Prime & Bond NT (PBN), Onecoat bond (OC), Excite (EX), Syntac (SY), Clearfil SE bond (CS)) and five composite resins (Spectrum (SP) Synergy Compact (SC), Tetric Ceram (TC), Clearfil AP-X (CA). Z100 (Z1)) were used for thing study (5×5 = 25group, n = 14/group). The slices of horizontally sectioned human tooth were bonded with each bonding system and each composite resin, and tested by a micro-shear bond strength test. These results were analyzed statistically. The mean micro-shear bond strength of dentin adhesive systems were in order of CS (22.642 Mpa), SY (18.368 MPa), EX (14.599 MPa), OC (13.702 MPa), PBN(12.753 MPa). The mean bond strength of self-etching primer system group (CS, SY) in dentin was higher than that of self-priming adhesive system groups (PBN, EX, OC) significantly (P<0.05). The mean bond strength of composite resins was in ordel of SP (19.008 MPa), EA (17.532 MPAa), SC (15.787 MPa), TC (15.058 MPa), Z1 (14.578 MPa) Micro-shear bond strength of SP was stronger than those of other composite resins significantly (P < 0.05). And those of TC and Z1 were weaker than other composite resins significantly (P < 0.05). No difference was found in micro-shear bond strength of composite resin in self-etching primer adhesive system groups (CS, SY) statistically. However, there was significant difference of micro-shear bond strength of composite resin groups in self-priming adhesive systems group (PBN, EX, OC). The combination of composite resin and dentin adhesive system recommended by manufacturer did not represent positive correlation. It didn't seem to be a significant factor.

        수종 이원중합 레진 씨멘트의 중합률 및 세포 독성에 관한 실험적 연구

        AbstractThe degree of conversion of composite resin was known to have influence on the mechani- cal properties of composite materials such as hardness, strength, wear resisitance, dimensio- nal and color stability. Also unreacted monomer was reported to be harmful to the pulp. So the degree of conversion was a very important factor in the success of composite resin restorations.In recent, the dual cure resin cement was developed with the advocations that it could increase the curing rates in the sites where the curing ligt could not reach. Moreover many manufactors added some adhesive components in the resin cement.This study was undertaken to observe the effects of curing depth and light curing times on the degree of conversion of dual cure resin cements. CR INLAY CEMENT, DUAL CEMENT and OPTEC BOND, by the Fourier transform Infrared analysis, changing the curing depth 1mm, 2mm and 3mm, and varying the light curing time 20 seconds, 40 seconds and 80 seconds at each depth. The cytotoxicity of dual cure resin cements was tested by the in vitro MTT method using L929 cell. The results was evaluated and compared statistically.The results were obtained as follows: 1. The dual cure resin cements reavealed various degree of conversion, CR INLAY CE- MENT and DUAL CEMENT had a tendency to be more reactive to the light cure and OPTEC BOND was a more chemical one. 2. CR INLAY CEMENT and DUAL CEMENT showed the lowest degree of conversion in 2 mm depth, and in 3mm depth the degree of conversion increased, which were due to the chemical cure of dual cures, but OPTEC BOND showed decreasing degree of conversion with increasing curing depth and all experimental groups showed lower degree of conversion than CHEMICAL group which cured in dark room with no light, so the weak light?uring of dual cure resin cement prevented the chemical cure. (P<0. 05) 3. CR INLAY CEMENT and DUAL CEMENT showed increasing degree of conversion in 1 mm and 3 mm, according to the increasing cure times, but in 2 mm depth the degree of conversion decreased with increasing light?uring times and OPTEC BOND showed contrary tendency, but there was no ststistical importance in the differences among the experimental group.(P>0.05) 4. The optical density by MTT assay of extractions of CR INLAY CEMENT, DUAL CE- MENT and OPTEC BOND revealed no statitically important differences comparing with optical density of negative control.(P>0.05) 5. CR INLAY CEMENT showed a tendency of increaing cytotoxicity with days and DUAL CEMENT and OPTEC BOND showed higher cytotoxicity in 2 days than in 4 days, but there was no statistical importance in the differences.         교정용 고무 링의 삽입과 임시 전장관의 수정을 통한 보철 수복 전 인접면 공간 획득: 증례보고

        Proximal caries or coronal defect in posterior teeth may result in the loss of proximal space and drifting of neighboring teeth, which makes restoration difficult. Inability to restore proper contours and to align tooth axis properly are commonly encountered problems when planning tooth restoration. Moreover, tilted teeth aggravate periodontal tissue breakdown, such as pseudo-pocket, and angular osseous defect. The purpose of this case presentation is to describe a simple technique for inducing minor tooth movement with orthodontic separating ring and provisional restoration modification. This method was used to create crown placement space on mesially tilted molar. This method is easy, simple and efficient technique which could be used in interproximal space gaining in selected situation.

        산 완충용액의 pH 및 유산의 온도가 인공치근우식의 형성에 미치는 영향

        치근 우식증은 복합적인 요인에 의해 발생되는 질환으로 고령인구의 증가로 최근 증가추세 에 있으나 아직까지는 병소깊이, 상아질 탈회의 정도 및 양상, collagen의 파괴 정도 및 수산화인회석 결정 변화에 대한 탈회 완충액의 조직학적 연구는 미비한 실정이다. 본 연구는 우식 형성에 영향을 주는 산 완충용액 내의 pH와 유산의 농도변화에 따른 치근 상아질 우식 병소의 진행에 미치는 변화를 편광현미경을 이용하여 관찰하고 관찰된 우식 병소층의 수산화인회석의 결정 형태 변화를 주사전자현미경으로 관찰하여 탈회 과정을 살펴보고자 세 가지 pH (4.3, 5.0, 5.5)군과 각각의 pH군에 세 가지 유산의 농도 (25 mM, 50 mM, 100 mM)를 이용하여 인공치근 우식을 형성한 후 비교 분석하여 다음과 같은 결과를 얻었다. 1. 편광현미경 소견에서 우식 병소의 깊이는 pH 보다는 유산의 농도에 의해 더 영향을 많이 받았다. 2. 주사전자현미경 소견에서 유산의 농도가 높아질수록 그리고 pH가 낮아질수록 수산화인회석 결정의 소실이 더 많이 진행되었다. 3. 탈회는 수산화인회석 결정의 변연부가 소실되며 결정 cluster내 결정의 숫자 및 크기가 감소하였고 결정 cluster 사이의 간격이 넓어지는 양상으로 관찰되었다. 4. 표면층에서의 주사전자현미경 관찰 시 유산의 농도가 높아질수록 수산화인회석 결정 cluster의 형태는 소실되고 콜라겐 섬유 표면에 수산화인회석 결정의 용해, 재결합된 양상으로 관찰되었다. 5. 탈회 과정에 대한 주사전자현미경 관찰 시 상아질의 탈회는 단순히 탈회만 독립적으로 일어나는 과정이 아닌 탈회와 재광화가 동시에 일어나는 양상으로 관찰되었다. 이상의 결과 산 완충용액 내의 유산의 농도가 높아지고 pH가 낮아질수록 탈회의 속도가 증가하고 탈회의 과정은 수산 화인회석 결정 cluster의 표면으로부터 진행되며 시간이 경과함에 따라 수산화인회석 결정의 형태는 원형 또는 타원형에서 불규칙한 형태로 변화되었다. The purpose of this study is to compare and to evaluate the effect of pH and lactic acid concentration on the progression of artificial root caries lesion using polarizing microscope, and to evaluate the morphological changes of hydroxyapatite crystals of the demineralized area and to investigate the process of demineralization using scanning electron microscope. Artificial root caries lesion was created by dividing specimens into 3 pH groups (pH 4.3, 5.0, 5.5), and each pH group was divided into 3 lactic acid concentration groups (25 mM, 50 mM, 100 mM). Each group was immersed in acid buffer solution for 5 days and examined. The results were as follows: 1. Under polarized microscope, the depth of lesion was more effected by the lactic acid concentration rather than the pH. 2. Under scanning electron microscope, dissolution of hydroxyapatite crystals were increased as the lactic acid concentration increased and the pH decreased. 3. Demineralized hydroxyapatite crystals showed peripheral dissolution and decreased size and number within cluster of hydroxyapatite crystals and widening of intercluster and intercrystal spaces as the pH decreased and the lactic acid concentration increased. 4. Under scanning electron microscope evaluation of the surface zone, clusters of hydroxyapatite crystals were dissolved, and dissolution and reattachment of crystals on the surface of collagen fibrils were observed as the lactic acid concentration increased. 5. Under scanning electron microscope, demineralization of dentin occurred not only independently but also with remineralization simultaneously. In conclusion, the study showed that pH and lactic acid concentration influenced the rate of progression of the lesion in artificial root caries. Demineralization process was progressed from the surface of the cluster of hydroxyapatite crystals and the morphology of hydroxyapatite crystals changed from round or elliptical shape into irregular shape as time elapsed.

        2종 전자근관장측정기의 일관성에 관한 in vitro 연구

        정확한 근관장 측정은 성공적인 근관치료를 위해 필수적이다. Kuttler는 근관협착부는 주근단공에서 0.5 mm 상방에 위치한다고 하였고 Lee는 상아백악질경계점부위보다 주근단공이 재현성이 높고 임상적으로 관찰하기 용이하다고 보고하였다. 본 연구는 alginate model상에서 2개의 전자근관장측정기를 사용하여 얻은 측정치의 정확성(accuracy)을 평가하고 각각 0.5 mark와 Apex mark중에서 어느 지점에서 더 일관성(consistency)을 보이는지를 비교하고 또한 근관장을 측정할 때 파일을 전진시키면서 Apex mark를 측정한 값과 치근단부위를 지나친 후 다시 후퇴하면서 측정한 값의 차이를 비교하고자 하였다. 52개의 발거된 건전한 제 1, 2소구치를 대상으로 하였으며 Root ZX와 E-Magic Finder Deluxe를 이용하여 각각 26개중 13개는 파일을 전진하면서 0.5 mark에서 근관장을 측정하고 Apex mark에서 근관장을 측정한 후 0.5 mark에서 고정하였다. 나머지 13개는 0.5 mark에서 근관장을 측정한 후 Apex mark에서 근관장을 측정하고 다시 치근단 부위를 넘어선 후 후퇴하면서 Apex mark에서 근관장을 측정하고 고정한 후 치근단부 4 mm를 삭제하여 현미경상에서 파일 끝부터 주근단공까지의 거리를 측정하였다. 그 결과 Root ZX와 E-Magic Finder 모두, 실험군 100%에서 주근단공과 file tip간의 거리가 임상적 허용범위인 ± 0.5 mm 내에 있었으며 0.5 mark보다 Apex mark에서 더 높은 일관성을 보였다. 주근단공에서 file tip 사이의 거리는 Root ZX의 0.5 mark에서 -0.18 mm, Apex mark에서 -0.07 mm 이고 E-Magic Finder의 0.5 mark에서 -0.25 mm, Apex mark에서 -0.02 mm를 나타내었다. Apex band는 0.04 mm였다. 따라서 Alginate model을 사용한 본 실험조건에서는 Apex mark에서 근관장을 측정한 결과 거의 주근단공과 일치하고 다른 여러 연구에서 주근단공에서 0.5 mm 상방에 근관협착부가 위치한다고 보고하였으므로 임상적인 측면에서 Apex mark에서 측정한 근관장에서 0.5 mm 를 빼서 근관장으로 사용하는 것이 임상적으로 더 좋으리라고 사료된다. The purpose of this study was to evaluate the consistency of two electronic apex locators in vitro model. Materials consisted of fifty two extracted premolars and two electronic apex locators: Root ZX (J.Morita. Osaka, Japan) and E-Magic Finder Deluxe (S-Denti, Cheonan, Korea). After access preparation, the teeth were embedded in a saline-mixed alginate model. Canal lengths of each tooth were measured at "0.5" and "Apex" mark of the apex locators, respectively so that each tooth had two measurements from 0.5 and Apex points. The file was fixed at final measurement using a glass ionomer cement. The apical 4 mm from the apex was exposed to measure the distance from the file tip to the major apical foramen of each tooth. Average distances and standard deviations were used to evaluate the consistency. Results showed that all measurements of both Root ZX and E-Magic Finder located the major foramen the range of ± 0.5 mm level. Both apex locators showed better consistency at Apex mark than at 0.5 mark. The average distance of file tip-major foramen was - 0.18 mm at 0.5 mark and - 0.07 mm at Apex mark in Root ZX, - 0.25 mm at 0.5 mark and - 0.02 mm at Apex mark in E-Magic Finder. Standard deviation was 0.21 at 0.5 mark and 0.12 at Apex mark in Root ZX, 0.12 at 0.5 mark and 0.09 at Apex mark in E-Magic Finder.

        Fluoride, Bisphosphonate 및 Gallium이 상아질 흡수 억제에 미치는 영향

        Replacement resorption is followed by the delayed replantation of an avulsed tooth. Currently no effective treatment is substantiated for replacement resorption. The purpose of this study was to investigate the effect of stannous fluoride, bisphosphonate(etidronate disodium) and gallium nitrate, which have been shown to reduce dentin resorption, on human dentin. Osteoclasts were collected from tibeas of chich embryo. The cells were well agitated to prevent adhesion and seeded onto the sliced human dentin wafers which had been soaked in either culture media(control), or several different concentrations of stannous fluoride, etidronate disodium(1-hydroxyethylidene-1, 1-bisphosphonate disodium), and gallium nitrate. Resorption was measured by counting the number of resorptive pit produced by the cells. Results are ad follows. Stannous fluoride and etidronate disodium showed statistically significant reduction of dentin resorption(p<0.05) but the effect of stannous fluoride seemed to be its high cytotoxicity. Etidronate disodium did not show cytotoxicities in all experimented concentrations. Gallium nitrate did not show differences in resorption either between different concentrations of from the control group.
