      • Clinico-Pathological Patterns and Survival Outcome of Colorectal Cancer in Young Patients: Western Saudi Arabia Experience

        Background: The prognosis of young colorectal cancer (CRC) patients has been addressed by several studies but with contradictory results. The aim of the present study was to evaluate the clinico-pathological features of young Saudi patients with CRC in addition to displaying their survival outcome. Materials and Methods: In this retrospective study, young CRC patients (${\leq}40$ years) diagnosed between 2007 and 2011 from 4 centres in western Saudi Arabia, were included. Clinico-pathological features, tumor markers, dates of disease relapse and death were collected. Survival parameters were compared with those of older Saudi patients, reported in previous studies. Results: One hundred and sixteen young patients with CRC were identified (32.2% rectal, 67.8% colon). Some 44% were metastatic while 32.7% had stage III at diagnosis. Patients with grade 3 tumors made up 29.4% of the total while 49.5% had positive lymphovascular invasion (LVI), 56% had a lymph node (LN) ratio ${\geq}0.2$ and 40.2% were K-ras mutant. Median disease-free survival (DFS) and overall survival (OS) in non-metastatic cases were 22.8 and 49.6 months respectively with better median DFS in K-ras wild compared to mutant patients (28.5 vs 20.9 months, p=0.005). In metastatic cases, median OS was 19.5 months. These survival outcomes are inferior compared to those of older Saudi patients reported in prior studies. Conclusions: Young CRC patients present more commonly with advanced stage and a high incidence of adverse prognostic factors such as LVI and high LN ratio. Young CRC patients seem to have worse survival compared to older Saudi patients.

        Study on (n,p) reactions of 58Ni, 99Tc, 99Ru, 131Xe, 133Cs and 186Os radioisotopes used in medicine

        In the last decade, nuclear medicine appears to be a good choice of medicine. 58Co, 99Mo, 99Tc, 99Re, 133Xe and 186Re are very important radionuclides for nuclear medicine. In this study, the excitation functions of 58Ni (n, p) 58Co, 99Tc (n, p) 99Mo, 99Ru (n, p) 99Tc, 131Xe (n, p) 131I, 133Cs (n, p) 133Xe and 186Os (n, p) 186Re nuclear reactions were calculated at neutron energies between 1 and 20 MeV using TALYS 1.95 and EMPIRE 3.2 nuclear codes. Furthermore, the cross sections were calculated with the empirical formula derived in our past study at 14e15 MeV. The obtained results were compared with the measured values in EXFOR library, and with the evaluated data of (JENDL-4.0/HE, JEFF-3.3, TENDL-2019, ENDF/B-VIII.0, IRDFF-II, JENDL/ImPACT-18). The results are in good agreement with those of the evaluated data libraries and experimental results and indicates that these radioisotopes can be produced by smaller cyclotrons.

        Study on (n, α) reactions for the production of 51Cr, 89Sr, 99Tc, 131I, 133Xe, 137Cs and 153Sm radioisotopes used in nuclear medicine

        Nuclear medicine seems to be a decent choice of medicine in the recent decade. The radioactive isotopes 51Cr, 89Sr, 99Tc, 131I, 133Xe, 137Cs and 153Sm are extremely essential in nuclear medicine. The excitation functions of the 54Fe (n, a) 51Cr, 92Zr (n, a) 89Sr, 102Rh (n, a) 99Tc, 134Cs (n, a) 131I, 136Ba (n, a) 133Xe, 140La (n, a) 137Cs and 156Gd (n, a) 153Sm reactions were calculated in this study using the EMPIRE 3.2.3 and TALYS 1.95 nuclear codes. Additionally, the cross sections at 14e15 MeV were calculated using empirical formulae and the experimental data. The computer codes were compared to the experimental data and Empirical formulas as well as the evaluated data (TENDL 2021, JENDL 3.3, JENDL 5, JEFF 3.3, EAF 2010, CENDL 3.1, CENDL 3.2, ROSFOND 2010, FENDL 3.2 b, and BROND 3.1).

        The role of internal architecture in producing high-strength 3D printed cobaltchromium objects

        Purpose. The objectives of the current study were to estimate the influence of self-reinforced hollow structures with a graded density on the dimensional accuracy, weight, and mechanical properties of Co-Cr objects printed with the direct metal laser sintering (DMLS) technique. Materials and methods. Sixty-five dog-bone samples were manufactured to evaluate the dimensional accuracy of printing, weight, and tensile properties of DMLS printed Co-Cr. They were divided into Group 1 (control) (n = 5), Group 2, 3, and 4 with incorporated hollow structures based on (spherical, elliptical, and diamond) shapes; they were subdivided into subgroups (n = 5) according to the volumetric reduction (10%, 15%, 20% and 25%). Radiographic imaging and microscopic analysis of the fractographs were conducted to validate the created geometries; the dimensional accuracy, weight, yield tensile strength, and modulus of elasticity were calculated. The data were estimated by one-way ANOVA and Duncan’s tests at P < .05. Results. The accuracy test showed an insignificant difference in the x, y, z directions in all printed groups. The weight was significantly reduced proportionally to the reduced volume fraction. The yield strength and elastic modulus of the control group and Group 2 at 10% volume reduction were comparable and significantly higher than the other subgroups. Conclusion. The printing accuracy was not affected by the presence or type of the hollow geometry. The weight of Group 2 at 10% reduction was significantly lower than that of the control group. The yield strength and elastic modulus of the Group 2 at a 10% reduction showed means equivalent to the compact objects and were significantly higher than other subgroups.

        Association of Air Pollutant Index (API) on SARS-CoV-2 of Coronavirus Disease 2019 (COVID-19) in Malaysia

        Malaysia reported its first COVID-19 case on January 25, 2020, and the cases have continued to grow, necessitating the implementation of additional measures. Hence, determining the factors responsible for the significant increase in COVID-19 cases is the top priority issue for the government to take necessary action and ultimately restrain this virus before the vaccine availability. Researchers had predicted that air pollution had an indirect relationship with COVID-19 in terms of virus infections. As a result, this study focuses on the link between the Air Pollutant Index (API) and COVID-19 infections. The initial data set consists of daily confirmed COVID-19 cases in Malaysia and API readings obtained from the Ministry of Health (MOH) and the Department of the Environment (DOE). The results show that Klang (S22) recorded the highest mean of API which at 62.70 while the lowest is at Limbang (S37) (25.37). Next, due to the implementation of Movement Control Order (MCO) in Malaysia and reducing social movement, 27 stations recorded a good level of API compare to the stations that recorded moderate and unhealthy levels. There is positive relationship between API and COVID-19 at each of the region which are North 0.4% (R2=0.004), Central 2.1% (R2=0.021), South 0.04% (R2=0.0004), East 1.6% (R2=0.016), Sarawak 0.2% (R2=0.002), meanwhile Sabah recorded negative correlation at 4.3% (R2=0.043). To conclude, the API value did not have a strong relationship with the rising number of COVID-19 daily cases.

        Optimal 3D UAV Base Station Placement by Considering Autonomous Coverage Hole Detection, Wireless Backhaul and User Demand

        The rising number of technological advanced devicesmaking network coverage planning very challenging tasks for network operators. The transmission quality between the transmitterand the end users has to be optimum for the best performance outof any device. Besides, the presence of coverage hole is also an ongoing issue for operators which cannot be ignored throughout thewhole operational stage. Any coverage hole in network operators’coverage region will hamper the communication applications anddegrade the reputation of the operator’s services. Presently, thereare techniques to detect coverage holes such as drive test or minimization of drive test. However, these approaches have many limitations. The extreme costs, outdated information about the radioenvironment and high time consumption do not allow to meet therequirement competently. To overcome these problems, we take advantage of Unmanned aerial vehicle (UAV) and Q-learning to autonomously detect coverage hole in a given area and then deployUAV based base station (UAV-BS) by considering wireless backhaul with the core network and users demand. This machine learning mechanism will help the UAV to eliminate human-in-the-loop(HiTL) model. Later, we formulate an optimisation problem for3D UAV-BS placement at various angular positions to maximisethe number of users associated with the UAV-BS. In summary, wehave illustrated a cost-effective as well as time saving approach ofdetecting coverage hole and providing on-demand coverage in thisarticle.

      • Fenugreek Induced Apoptosis in Breast Cancer MCF-7 Cells Mediated Independently by Fas Receptor Change

        Trigonella foenum in graecum (Fenugreek) is a traditional herbal plant used to treat disorders like diabetes, high cholesterol, wounds, inflammation, gastrointestinal ailments, and it is believed to have anti-tumor properties, although the mechanisms for the activity remain to be elucidated. In this study, we prepared a methanol extract from Fenugreek whole plants and investigated the mechanism involved in its growth-inhibitory effect on MCF-7 human breast cancer cells. Apoptosis of MCF-7 cells was evidenced by investigating trypan blue exclusion, TUNEL and Caspase 3, 8, 9, p53, FADD, Bax and Bak by real-time PCR assays inducing activities, in the presence of FME at $65{\mu}g/mL$ for 24 and 48 hours. FME induced apoptosis was mediated by the death receptor pathway as demonstrated by the increased level of Fas receptor expression after FME treatment. However, such change was found to be absent in Caspase 3, 8, 9, p53, FADD, Bax and Bak, which was confirmed by a time-dependent and dose-dependent manner. In summary, these data demonstrate that at least 90% of FME induced apoptosis in breast cell is mediated by Fas receptor-independently of either FADD, Caspase 8 or 3, as well as p53 interdependently.

      • Stromal Modulation and its Role in the Diagnosis of Papillary Patterned Thyroid Lesions

        The papillary patterned lesion of thyroid may be challenging with many diagnostic pitfalls. Tumor stroma plays an important part in the determination of the tumor phenotype. CD34 is thought to be involved in the modulation of cell adhesion and signal transduction as CD34(+) fibrocytes are potent antigen-presenting cells. Smooth muscle actin (SMA) positivity could be diagnostic for fibroblast activation during tumorigenesis. We aimed to examine the expression of CD34 and alphaSMA in the stroma of papillary thyroid hyperplasia, papillary thyroid carcinoma and papillary tumors of uncertain malignant potential in order to elucidate their possible differential distribution and roles. A total number of 54 cases with papillary thyroid lesions were studied by routine H&E staining, CD34 and ASMA immunostaining. ASMA was not expressed in benign papillary hyperplastic lesions while it was expressed in papillary carcinoma, indicating that tumors have modulated stroma. Although the stroma was not well developed in papillary lesions with equivocal features of uncertain potentiality, CD34 was notable in such cases with higher incidence in malignant cases. So ASMA as well as CD34 could predict neoplastic behavior, pointing to the importance of the stromal role. Differences between groups suggest that the presence of CD34 + stromal cells is an early event in carcinogensis and is associated with neoplasia, however ASMA+ cells are more likely to be associated with malignant behavior and metastatic potential adding additional tools to the light microscopic picture helping in diagnosis of problematic cases with H&E.

        Nizwaside: a new anticancer pregnane glycoside from the sap of Desmidorchis flava

        The sap from the succulent Desmidorchis flava (N.E.Br) Meve and Liede yielded a new pregnane glycoside, named nizwaside whose structure was established using 1D and 2D NMR techniques as well as mass spectrometry (ESIMS). Nizwaside was tested for anticancer, DPPH antioxidant, urease enzyme inhibition, a-glucosidase enzyme inhibition and acetylcholinesterase inhibition activities. Interestingly, nizwaside showed significant anti-proliferative effects on MDA MB231 breast cancer cells with an IC50 of 23.5 lg/ml. Moreover, nizwaside was more effective than Doxorubicin, a wellknown clinical anticancer drug, in suppressing MDA MB231 cell proliferation even at concentrations lower than that of Doxorubicin (75 lg/ml nizwaside vs. 100 lg/ml Doxorubicin). On the other hand, nizwaside showed relatively weak antioxidant activity with 15 % inhibition.

        Integrated Algal Engineering for Bioenergy Generation, Effluent Remediation, and Production of High-value Bioactive Compounds

        Increased demand for energy worldwide has resulted in increasing interest in alternative renewable sources of biofuels. Demand for improved systems of bioenergy generation, environmental remediation, and coproduction of high value bioactive compounds has led to the potential use of algae in biomass utilization. In Malaysia, palm oil industries generate high amount of solid wastes. Palm Oil Mill Effluent (POME) is estimated to be three times of the amount of crude palm oil produced. POME is a heavily polluting wastewater due to its high chemical oxygen demand (COD), high biochemical oxygen demand (BOD), and high contents of minerals such as nitrogen and phosphorus that can cause severe pollution to the environment and water resources. A combination of wastewater treatment and renewable bioenergy co-generation with recovery of high-value biochemicals would benefit the palm oil industry.
