Purpose: Safely postponing the use of chemotherapy is important for quality of life maintenance in patients with hormone receptor-positive advanced breast cancer. In previous studies, a combination of cyclin-dependent kinase 4 and 6 inhibitors (CDK4/6i) and fulvestrant prolonged the time to chemotherapy (TTC). In this study, we used real-world data to evaluate TTC in the context of CDK4/6i therapy. Methods: We performed a retrospective chart review of women with estrogen receptor-positive, human epidermal growth factor receptor 2-negative advanced breast cancer treated at the Aichi Cancer Center Hospital. The patients were categorized into having received CDK4/6i therapy first (n = 41), second (n = 33), and none at all (n = 67). The change in TTC among the groups was examined. Results: The median follow-up time was 13.8, 27.5, and 30.3 months in the CDK4/6i (first), CDK4/6i (second), and non-CDK4/6i groups, respectively. The median progression-free survival (PFS) with first-line therapy for metastasis was 30.0, 11.9, and 13.0 months, respectively (CDK4/6i [first] vs. non-CDK4/6i; p = 0.018, CDK4/6i [second] vs. non-CDK4/6i; p = 0.383). The median TTC was not reached in the CDK4/6i (first) group, was 39.1 months in the CDK4/6i (second) group, and was 44.2 months in the non-CDK4/6i group (CDK4/6i [first] vs. non-CDK4/6i; p = 0.880; CDK4/6i [second] vs. non-CDK4/6i; p = 0.407). The non-CDK4/6i group with TTC ≥ 60 months included more cases of secondary endocrine therapy resistance (p = 0.017), no perioperative chemotherapy (p = 0.021), and a longer disease-free interval (p = 0.093). Conclusion: Although PFS was significantly longer in the CDK4/6i (first) group than in the non-CDK4/6i group, TTC did not significantly differ among the three groups in real-world data. The non-CDK4/6i group showed a long TTC in patients with late recurrence and low risk at the primary lesion site, who benefited greatly from hormone monotherapy.
Influence of a SiO2 Mask on the Growth of Semi-Polar (11-22) GaN on Patterned Si (311) Substrates

The selective growth of semi-polar (11-22) GaN layers on patterned Si (311) substrates was performed by using metal organic vapor phase epitaxy (MOVPE) without using a SiO2 mask to decrease the density of crystal defects that may be caused by the existence of the mask material. We found that even without using a SiO2 mask (11-22) GaN layers could be selectively grown on Si (111) facets when the mesa width of the patterned silicon substrates was smaller than 1 micrometer. Low temperature (7 K) cathodoluminescence (CL) measurement confirmed that the intensity of the emission peak arising from the crystal defect region was remarkably decreased by the use of a maskless selective growth method. Low-temperature CL measurements were also performed on InGaN multiple-quantum-well (MQW) structures grown on (11-22) GaN templates. From the CL spectra, we could observe that the InGaN MQW structure grown on a (11-22) GaN template, which was selectively grown without a SiO$_{2}$ mask, had fewer crystal defects than that grown on a (11-22) GaN template formed by using the conventional selective growth method.

Infrared reflectance spectroscopy was applied to the GaN/AlGaN triangular stripes grown on a Si(111) substrate by selective area metalorganic vapor phase epitaxy. Because of the triangular shape of the GaN, which worked itself as a prism, we could detect the local vibration modes of Si-N, N-H$_2$, Si-H, and Si-O bonds near the GaN/Si interface with high sensitivity.

The intersubband relaxation processes of electrons in GaAs/AlGaAs mesoscopic disks were investigated by femtosecond reflectance spectroscopy. It was found that the relaxation time in a mesoscopic disk is on the order of sub-picosecond and depends on the shape of the disk. Under the lowest excitation intensity where the relaxation time is independent of the excitation intensity, the LO phonon emission rate is lower than that expected in a bulk GaAs. Moreover the time constant depended on the shape of the disk. A possible origin of the suppression of LO phonon emission rate is discussed from the point of the modification of the scattering cross section in a disk of mesoscopic size.

Purpose: Fine-needle aspiration cytology (FNAC) of axillary lymph nodes (AxLNs) is performed to diagnose nodal metastasis in patients with breast cancer. Although the sensitivity of ultrasound-guided FNAC for identifying AxLN metastasis is in the range of 36%–99%, whether sentinel lymph node biopsy (SLNB) should be performed for neoadjuvant chemotherapy (NAC) patients with negative FNAC results is uncertain. This study aimed to determine the role of FNAC before NAC in the evaluation and management of AxLN in early breast cancer patients. Methods: We retrospectively analyzed 3,810 clinically node-negative (a lymph node with no clinical metastasis without FNAC or radiological suspicion of metastasis with negative FNAC results) patients with breast cancer who underwent SLNB between 2008 and 2019. We compared the positivity rate of sentinel lymph nodes (SLNs) between patients who received and those who did not receive NAC with negative FNAC results or without FNAC and axillary recurrence rate in the neoadjuvant group with negative SLNB results. Results: In the non-neoadjuvant (primary surgery) group, the positivity rate of SLNs in patients with negative FNAC results was higher than that in patients without FNAC (33.2% vs. 12.9%; p < 0.001). However, the SLN positivity rate of patients with negative FNAC results (false-negative rate for FNAC) in the neoadjuvant group was lower than that in the primary surgery group (3.0% vs. 33.2%; p < 0.001). After a median follow-up of 3 years, one axillary nodal recurrence was observed, which was a case from the neoadjuvant non-FNAC group. None of the patients in the neoadjuvant group with negative FNAC results had axillary recurrence. Conclusion: The false-negative rate for FNAC in the primary surgery group was high; however, SLNB was the proper axillary staging procedure for NAC patients who have clinically suspicious AxLN metastases on radiologic examination but negative FNAC results.

Endoscopic ultrasonography (EUS) combines endoscopy and intraluminal ultrasonography, and allows imaging with a high-frequency transducer over a short distance to generate high-resolution ultrasonographic images. EUS is now a widely accepted modality for diagnosing pancreatobiliary diseases. EUS-guided fineneedle aspiration (EUS-FNA) using a curved linear- array echoendoscope was initially described more than 20 years ago, and since then many researchers have expanded its indications to sample diverse lesions and have also used it for various therapeutic purposes. EUS-guided biliary drainage (EUS-BD) is one of the therapeutic procedures that has been developed using a curved linear-array echoendoscope. Technically, EUS-BD includes rendezvous techniques via transesophageal, transgastric, and transduodenal routes, EUS-guided choledochoduodenostomy (EUS-CDS), and EUS-guided hepaticogastrostomy (EUS-HGS). Published data have demonstrated a high success rate, albeit with a comparatively high rate of nonfatal complications for EUS-CDS and EUS-HGS, and a comparatively low success rate with a low complication rate for the rendezvous technique. At present, these procedures represent an alternative to surgery or percutaneous transhepatic biliary drainage (PTBD) for patients with obstructive jaundice when endoscopic biliary drainage (EBD) has failed. However, these procedures should be performed in centers with extensive experience in linear EUS and therapeutic biliary ERCP. Large prospective studies are needed in the near future to establish standardized EUS-BD procedures as well as to perform controlled comparative trials between EUS-BD and PTBD, between rendezvous techniques and direct-access techniques (EUS-CDS and EUS-HGS), and between EBD and EUS-BD. Asian Consensus Guidelines for the Diagnosis and Management of Gastrointestinal Stromal Tumor

Gastrointestinal stromal tumors (GISTs) are the most common mesenchymal tumors originating in the gastrointestinal tract. With the introduction of molecular-targeted therapy for GISTs which has yielded remarkable outcomes, these tumors have become a model of multidisciplinary oncological treatment. Although Western clinical guidelines are available for GISTs, such as those published by the National Comprehensive Cancer Network (NCCN) and the European Society of Medical Oncology (ESMO), the clinical situations in Asian countries are different from those in Western countries in terms of diagnostic methods, surgical approach, and availability of new targeted agents. Accordingly, we have reviewed current versions of several GIST guidelines published by Asian countries (Japan, Korea, China, and Taiwan) and the NCCN and ESMO and discussed the areas of dissensus. We here present the first version of the Asian GIST consensus guidelines that were prepared through a series of meetings involving multidisciplinary experts in the four countries. These guidelines provide an optimal approach to the diagnosis and management of GIST patients in Asian countries.

In case of K_lmax < K_lscc, the corrosion fatigue of high strength steel in O.1N H₂SO₄ solution and 3.5% salt water is as follows.<br/> 1. The fatigue life shortens in order of 3.5% salt water and O.1N H₂SO₄ solution.<br/> 2. The fatigue crack growth rate in air is obtained as the following equation.<br/> (dc/dN)_?=7.23×10^-?(△K)^?<br/> 3. The corrosion fatigue crack growth rate in environment is divided into three regions, that is, First Region, Second Region and Third Region from the small cyclic stress intensity.<br/> 4. The formation rate of the active surface on metal is slower than the mechano-chemical reaction rate in First Region. The crack growth rate depends on time and the cyclic stress intensity and is expressed as the following equation.<br/> (dc/dN)=C(△K)^δ<br/> 5. The formation rate of the active surface is faster than the mechano-chemical reaction rate in Second Region and the synergistic effect by stress and corrosion becomes slow.
