        Drug-Resistant Hyponatremia after Escitalopram Intake: A Series of Two Case Reports

        The consumption of new selective serotonin reuptake inhibitors (SSRIs) is raising dramatically especially in European countries. It contributes to occurrence of clinically important drug side effects. One of which can be hyponatremia. We present two case reports of 85-year-old and 84-year-old women who developed hyponatremia after escitalopram administration. We hypothesize that in both cases hyponatremia was connected with antidepressants administration. However, due to multiple comorbidities and polypharmacy it is often impossible to establish the exact mechanism of hyponatremia. Moreover, it is crucial to distinguish subtypes of drug-induced syndrome of inappropriate antidiuretic hormone hypersecretion (SIADH), such as SSRI-induced SIADH, reset osmostat SIADH, thiazide-associated hyponatremia, thiazide-induced hyponatremia, mineralocorticoid responsive hyponatremia of older adults, in order to properly diagnose and treat geriatric patients. Administration of antidepressants or thiazides should be followed by a regular monitoring of serum sodium level.

      • Effect of Media Components and Phytohormones on in vitro Frond Proliferation of Lemna gibba G3 and 24 Additional Lemna gibba Strains

        The effects of basal media, sucrose and phytohormone concentrations, and gelling agent combinations on in vitro frond proliferation of Lemna gibba G3 and 24 additional Lemna gibba strains were examined. Frond proliferation was equivalent on Schenk and Hidebrandt, Murashige and Skoog, Nitsch and Nitsch, and Gamborg s B5 media and poor on Murashige and Skoog medium in the absence of benzyladenine. With the addition of benzyladenine, Schenk and Hildebrandt and Gamborg s B5 were superior and equivalent. The addition of benzyladenine increased equally frond proliferation at either 1 or 10uM, however at 10uM fronds were severely curled or fused. Benzyladenine and thidiazuron suppressed root growth but kinetin was found to greatly enhance root growth. Gibberellic acid inhibited frond proliferation. Frond proliferation was significantly different on the four sucrose concentrations of 0, 1, 3, and 8% Among them, 3% sucrose was found to be superior. The reduced frond size observed in cultures grown on 8% sucrose could be explained by showing medium osmotic potential in excess of frond water potential. Gelling agents also varied significantly in their ability to promote frond proliferation with 0.25% Gelrite or a mixture of 0.15% Gelrite and 0.4% agar. Proliferation of 25 Lemna gibba strains on medium near optimal for Lemna gibba G3 showed a six-fold variation across strains with Lemna gibba G3 placing in the top 5 fastest proliferating strains.

        The effect of office based flexible and rigid cystoscopy on pain experience in female patients

        Purpose: Rigid and flexible cystoscopies are both routinely used in female patients. Literature is conflicting whether flexible cystoscopy is less painful compared to rigid cystoscopy. The aim of this study was therefore to investigate whether using flexible cystoscopy leads to less discomfort and pain compared to rigid cystoscopy in female patients who underwent first time cystoscopy. Materials and Methods: One hundred eighty-nine female patients, who never had undergone cystoscopy, were randomized into 2 groups: 92 patients underwent rigid cystoscopy and 97 patients flexible cystoscopy. Directly after the cystoscopy procedure all patients were asked to fill out their pain experience on a 100-mm visual analogue pain scale (VAS). Results: Median VAS score was significantly lower for women undergoing flexible cystoscopy (0 [0–20]) compared to rigid cystoscopy (15 [0–38], p<0.001). In addition, age was inversely associated with VAS score, indicating that younger females experienced more pain (R=−0.30, p=0.001). The use of flexible cystoscopy was associated with a decrease in VAS score and remained significant after adjustment for age, sex of urologist, performing urologist and indication (standardized β=−0.17, p=0.048). Conclusions: The use of flexible cystoscopy resulted in a significantly lower pain experience compared to rigid cystoscopy. Based on patient's pain experience during cystoscopy, this study implicates to use flexible cystoscopy in female patients who undergo first time cystoscopy.
