그룹에서 Nebulized Budesonide 와 Dexamethasone 근주의 치료효과

목 적 : 크룹환아의 급성기에 투여한 budesonide 분무홉입과 dexamethasone 근주의 상기도 폐색 완화 효과를 관찰하고자 전향적 연구를 시행하였다. 방법 및 대상 : 1994년 3월부터 1997년 2월까지 이화의대 부속 목동병원에 중등도의 크룹으로 입원한 환아 40명을 대상으로 무작위로 budesonide 흡입군 [budesonide 2ml(1 ㎎) 분무 흡입, 15명), dexamethasone 근주군 (dexamethasone 0.5 ㎎/㎏ 근주, 10명), 대조군(saline 2 ml 분무 홉입, 15명)으로 나누어 치료하였다. 치료효과로 크룹점수, 심박수, 호흡수 및 동맥혈 산소 포화도를 치료전과 치료후 12, 24, 36, 48시간의 변화를 관찰하였고, 치료후 12, 24, 36, 48시간에서 크룹점수 1점이하로 호전된 환아의 비율 및 입원기간을 비교하였다. 결 과: 1) 치료후 12, 24, 36 시간에서의 크룹점수는 budesonide 흥입군에서는 각각 1.7±0.8점, 1.1±0.7점, 0.8±0.7점, dexamethasone 근주군에서는 각각 2.2±0.7점, 1.2±0.8점, 1.0±0.5점으로 대조군의 3.4±0.9점, 2.8±0.9점, 1.7±0.8점보다 유의하게 낮았고(P<0.05), budesonide 흡입군과 dexamethasone 근주군 간에는 유의 한 차이가 없었다. 2) 심박수 호흡수 동맥혈 산소포화도는 치료후 유의한 변화가 없었다. 3) 치료후 크룹점수 1점 이하로 호전된 환아의 비음은 치료후 12, 24, 36시간에 budesonide 흡입군에서 53%(8/15), 73%(8/15), 87%(13/15)로 대조군의 0%(0/15), 13%(2/15), 47%(7/15)보다 유의하게 높았고(P<0.05), dexamethasone 근주군에서는 치료후 24시간에만 60%(6/10)로 대조군의 13%(2/15)보다 유의하게 높았다(P<0.05). 4) 입원기간과 48시간이전에 퇴원한 환아의 비율은 세 군간에 유의한 차이가 없었다. 결 론 : 중등도 크룹의 급성기에 budesonide 흡입과 dexamethasone 근주의 치료는 상기도 폐색을 신속하게 완화시켰으며 특히 budesonide 흡입치료의 보다 신속한 효과는 임상치료에 유용할 것으로 생각된다. Purpose : To evaluate the effect of nebulized budesonide and intramuscular dexamethasone in relieving laryngeal obstruction of croup. Methods : Forty patients with moderate croup (croup scores of 3 to 7 out of 17) were randomly assigned to budesonide group [1 ㎎(2 ml) of nebulized budesonide, N=15], dexamethasone group [intramuscular dexamethasone(0.5 ㎎/㎏) and nebulized saline, N=10] and control group(2 ml of nebulized saline, N=15). The changes of croup scores, heart rate, respiratory rate, and arterial oxygen saturation(SaO_2) were observed at post-treatment of 12, 24, 36 and 48 hours. The duration of admission and the rates of improvement to the croup score of ≤1 at the post-treatments were compared. Results :Croup scores at 12, 24 and 36 hours post treatments were 1.7±0.8, 1.1±0.7 and 0.8±0.7 in budesonide group and 2.2±0.7, 1.2±0.8 and 1.0±0.5 in dexamethasone group, respectively, which were significantly lower than 3.4±0.9, 2.8±0.9 and 1.7±0.8, the scores for group (P<0.05). Heart rate, respiratory rate, and SaO_2 were not significantly different among the three groups. Improved rates to croup score of ≤1 in budesonide group at post-treatments of 12, 24, and 36 hours were 53%, 73% and 87%, which were significantly higher than 0%, 13% and 47% in control group(P<0.05). Improved rate to croup score of ≤1 in dexamethasone group was 60% which was significantly higher than 13% in control group only at posttreatment of 24 hours(P<0.05). Conclusion : Nebulized budesonide and intramuscular dexamethasone are both effective in reducing obstructive symptoms in patients with moderate croup. The effect of nebulized budesonide was more rapid than that of intramuscular dexamethasone.
