Bentall씨 수술후 발생한 상행대동맥 가성동맥류 치험 1례

A 31 year-old male patient underwent surgical treatment of the pseudoaneurysm of the ascending aorta complicating after the Bentall operation, He had undergone the replacement of the ascending aorta using the composite valved graft with direct coronary reimplantation under the diagnosis of the annuloaortic ectasia of ascending aorta associated with Marfan syndrome. Eleven months after the operation, he started to feel dyspnea and anterior chest pain, and was diagnosed as pseudoaneurysm around the ascending aortic graft. The second operation consisted of the dacron patch closure of the defect of the aortic graft which was the hole for previous coronary reimplantation, and the anastomosis between the coronary orifice and the aortic graft with the intermediate graft of a 10mm woven dacron tube, and suture closure of the fistula opening from the aneurysm. His postoperative course was uneventful and discharged without complication. He is doing well 10 months postoperatively.

관동맥 우회술의 수술성적-수술전 처치 및 수술수기의 영향에 관한 연구

A total of 40 patients having a diagnosis of atherosclerotic coronary arterial disease were analysed on the operative outcomes according to variables as follows: 1) preoperative risk factors such as age, sex, CCS (Canadian Cardiovascular Society) functional class, type of angina, number of diseased vessel, presence of left main coronary artery stenosis, previous history of habitual smoking and presence of other medical diseases (diabetes mellitus, essential hypertension), 2) preoperative management such as intravenous infusion of nitroglycerine, preoperative IABP (intra-aortic balloon pump) support and whether the operation was scheduled as emergency or not, 3) intraoperative variables such as infusion method and composition of cardioplegic solutions, number of distal anastomosis, use of internal mammary artery, total cardiopulmonary bypass time and total cross clamp time. Complications included operative death in 12.5%, perioperative myocardial infarction in 25.0% and perioperative arrhythmia in 17.5%. Nineteen perioperative variables were analyzed to identify risk factors for these end points. For operative death, presence of left main coronary artery stenosis (p = 0.056) and cardiopulmonary bypass time (p = 0.029) were significant in the univariate analysis, but presence of left main coronary artery lesion (p = 0.011, $\chi$$^2$= 6.45) and abscence of preoperative of IABP support (p = 0.069, $\chi$$^2$ = 3.30) were independent predictor in multivariate analysis (stepwise linear logistic regression).

This study deals with the dwelling quality and ubiquitous system status of the apartment housing by the site types(site type and tower type). The research findings are as follows: ①The dwelling quality factors of the apartment housing are amenity, economic, safety/security, convenience, health and extension(space/time). ②The amenity, economic, safety/security factors of the tower type are superior to the site type. ③The relationship between architectural and ubiquitous system should be programmed together in the zoning works(design and field works). ④The linkage(site↔bldg↔unit house) of ubiquitous systems should be connected by the daily life territoriality. ⑤ The parking, entry, CCTVmonitoring, homenetwork and safety/security systems are linked with space territoriality(public↔semipublic↔semiprivate↔private).

Budd-Chiari syndrome(BCS) can result from a variety of causes. In the fareast asia, BCS is frequently secondary to membranous obstruction of the inferior vena cava(MOVC). It is not confirmed that such membranes are congeni tal or acquired. The importance of its early recognition lies in the potential curability of the disease after surgical treatment. In order to reach a final diagnosis, caval venography with two catheters, one placed above and the other placed below the obstruction, has been the essential procedure. We decided operative procedure according to the findings of the venography-transatrial membranotomy or transatrial membranectomy in membranous obstruction and dorsal cavoatrial bypass grafting in segmental obstruction. From 1969 to 1990, we underwent transatrial membranotomy in 7 cases, transatrial membranectomy in 3 cases, and dorsal cavoatrial bypass in 2 cases. There was no operative mortality. Percutaneous transluminal angioplasty(PTA) c/s stent was done in 2 restenosed patients with good results. Hepatocellular carcinoma was detected in one patients. In long term follow up(mean duration 60.5 mo.), 6 patients(50%) are in good state. We think that transatrial membranectomy using deep hypothermia and circulatory arrest is safe and effective procedure.
