만성 정신분열증 환자에서 Paroxetine과 Haloperidol 병합투여시 정신병리증상과 Haloperidol, Reduced Haloperidol 혈중농도의 변화

Selective serotonin reuptake inhibitors(SSRIs), as haloperidol, ore metabolized in the cytochrome P450IID6. They can cause inhibition of metabolism of antipsychotics to elevate the serum level of antipsychotics and exacerbate the extrapyramidal symptoms when co-administered with antipsychotics. Among these SSRIs, there ore a few studies about paroxetine compared to fluoxetine or sertraline. In this study, we have intended to know the drug interaction of paroxetine and haloperidol when co-administered two drugs for the chronic schizophrenics by assessing the changes of positive, negative symptoms and extrapyramidal symptoms. for this purpose, we selected 29 subjects, the chronic schizophrenics with no physical problems. They were under maintenance therapy of haloperidol. They ore randomly assigned to placebo group(n=12) and drug group(n=17) by using double blind method. And then, placebo or paroxetine 20mg were administered to the subjects of each groups during 8 week period. We have assessed their psychopathology and extrapyramidal symptoms using Positive and Negative Syndrome Scale(PANSS), Hamilton Rating Scale lor Depression(HRSD), Simpson-Angus Scale at 0, 2, 4, 6, 8 weeks and serum haloperidol, reduced haloperidol levels at 0, 4, 8 weeks during the period. The results ore analysed by using repeated measure MANOVA. 27 subjects have completed the study during 8 weeks. among the subjects, 1) PANSS, HRSD ; no significant difference between groups. 2) Simpson-Angus Scale ; no significant change according to the time and no significant difference between the groups(no group and time effect). 3) Haloperidol and reduced haloperidol level ; no significant change. When co-administered paroxetine and haloperidol, there ore no significant changes of the psychopothology and no significant changes of the extrapyramidal symptoms. In this result, paroxetine seems to be not to affect the metabolism of haloperidol.

Fluoxetine과 Haloperidol의 병합투여시 정신병리 및 추체외로 증상의 변화

본 연구는 정신분열증 환자 38명을 대상으로 하여 haloperidol과 fluoxetine을 8주간 병합투여 하였고, PANSS, CGI, Simpson-Angus 척도를 투여전과 투여후 2, 4, 6, 8주에 시행하여 임상증상 및 추체외로 부작용을 평가하였다. 결과는 다음과 같다. 1) 8주간의 연구기간동안 양성, 음성증상의 유의한 변화가 없었다. 2) 8주의 연구기간동안 추체외로 부작용의 증가가 나타나지 않았다. 이는 기존 연구에서 fluoxetine에 의해 haloperidol의 혈중농도가 증가하여 효과 및 부작용이 증가한다는 연구보고와는 다르며, 이 결과는 haloperidol과 fluoxetine의 병합사용을 안전하게 할 수 있다는 것을 의미한다고 하겠다. Objectives : The authors have intended to know the drug interaction of fluoxetine and haloperidol when coadministering two drugs to the chronic schizophrenics by assessing the changes of positive, negative symptoms and extrapyramidal symptoms. Conclusion : When co-administering fluoxetine and haloperidol, there were no significant changes of psychopathology and extrapyramidal symptoms. These results suggest that it is safe to coadminister fluoxetine to schizophrenic patients with haloperidol treatmemt.

Manic Patient with Meningioma Treated with Low dose Risperidone and Valproic Acid

We describe the case of a 73 year-old female patient, YSG, who initially presented with a manic episode without any previous psychiatric history and was later diagnosed as having a meningioma in the left frontal lobe. YSG's symptoms were characterized by hyperactivity, insomnia, aggressive behavior with an auditory hallucination. She showed no abnormal signs on a complete neurologic examination. A gadolinium-enhanced MRI study showed a huge, extra-axial mass with homogenous enhancement in the left high convexity of the frontal lobe. Her manic symptoms subsided after administration of risperidone 1mg and valproic acid 500mg daily, for three weeks without surgical resection of the tumor. These findings suggest that YSG's mania might have resulted from the left-sided frontal tumor, and that her symptoms were treated rapidly by small doses of risperidone combined with valproic acid. Medical staff who care for manic patients should be aware of this possibility of a organic lesion without evidence of neurologic disease.

저에너지 레이저 조사가 창상치유에 미치는 영향

This study was performed to determine the optimal dose of laser energy for wound healing. The skin wound with 8mm diameter was induced over the lumbar vertebrae of the rats, and wound squares, scab hardness, hematologic findings and histopathlogic findings according to irradiation of laser energy were studied. 1. Wound square was significantly reduced at Day 1 (p<0.01), 2 (p<0.01), 3 (p<0.05), 7 (p<0.01) and 8 (p<0.05), respectively in experimental groups, especially group II, compared with control group. 2. Scab hardness was significantly increased at Day 1 (p<0.01), 2 (p<0.01), 3 (p<0.05), 5 (p<0.01) and 7 (p<0.01), respectively in experimental groups, especially group II, compared with control group. 3. In hematological findings, red blood cells and white blood cells in experimental group were increased according to the lapse of days, but they were not significant. 4. In histopathologic findings, experimental groups, especially group II, revealed early scab formation, early appearance of phagocytes and fibroblast, rapid growth of granulation tissue and collagen, and promotion of wound healing in the result.

WSN은 센서 노드에 의해 구성된 네트워크로, 센서 노드는 한번 배치되면 재충전하거나 위치적으로 재배치가 불가능하다. 또한 센서노드들은 제한된 에너지를 가지고 통신에 참여하게 된다. 그러나 기존 제안되었던 클러스터링 기법들은 불균일한 분포로 배치된 WSN환경에 적용 시 지역적 특징으로 통신 단절이 발생되는 문제점으로 네트워크의 신뢰성에 문제점을 갖는다. 따라서 제안 알고리즘에서는 WSN환경에서 센서노드의 불균형 배치를 고려해 센서필드를 분할하고 분할영역의 센서노드 밀집도에 따라 고정, 정적, 동적 클러스터링 알고리즘을 선별적으로 적용함으로 센서노드의 통신 참여율을 25% 향상시켰다. 그리고 전체 네트워크 생명주기는 14%연장하여 네트워크의 신뢰성을 보장하였다. WSN based on wireless sensor nodes, Sensor nodes can not be reassigned and recharged if they once placed. Each sensor node comes into being involved to a communication network with its limited energy. But the existing proposed clustering techniques, being applied to WSN environment with irregular dispersion of sensor nodes, have the network reliability issues which bring about a communication interruption with the local node feature of unbalanced distribution in WSN. Therefore, the communications participation of the sensor nodes in the suggested algorithm is extended by 25% as the sensor field divided in the light of the non-uniformed distribution of sensor nodes and a static or a dynamic clustering algorithm adopted according to its partition of sensor node density in WSN. And the entire network life cycle was extended by 14% to ensure the reliability of the network.

장애인스포츠 실업팀 활성화를 위한 법,제도 개선 방안

이 연구는 장애인스포츠 선수들의 직업안정성 도모를 위한 장애인스포츠 실업팀 활성화의 기초자료를 제공하는데 목적이 있다. 이를 위해 우리나라 장애인스포츠 실업팀의 운영현황과 실태를 알아보고 실업팀 활성화를 위한 법·제도 개선 방안을 제시하고자 하였다. 연구방법으로는 문헌조사와 면접조사 그리고 전문가 회의를 실시하였다. 문헌조사와 담당자 면접조사를 통해서 현재 국내 장애인스포츠 실업팀 운영 현황과 실태를 조사하였으며, 운영의 근거가 되는 관련조례, 규정 등을 파악하였다. 또한 창단과 운영의 근거가 되는 상위 법령과의 비교검토를 통해 문제점을 파악하였다. 특히, 장애인체육을 전공한 대학교수 3명과 장애인체육행정가 2명으로 구성된 전문가 회의에서는 장애인스포츠 실업팀 활성화에 대한 법·제도의 한계와 개선방안을 도출하였다. 또한, 연구의 방향성을 논의하고 현행 법·제도의 검토를 바탕으로 한 문제점을 진단하였으며, 개선방안에 대한 논의 및 검토과정을 거쳐 최종 개선방안을 도출하였다. 첫째, 장애인실업팀에 대한 별도규정이 있는 경우, 선수개인의 사회보장 혜택을 강화하는 조항을 추가해야 한다. 둘째, 장애인실업팀에 대한 별도조항이 있는 경우, 장애인스포츠 실업팀의 운영·관리·지원문제를 명확히 하여 장애를 가진 선수들의 접근성과 이동권을 보장하는 조항이 구체적으로 마련되어야 할 것이다. 셋째, 장애인실업팀에 대한 공통규정이 있는 경우, 장애인선수 특성에 맞는 시설과 용·기구지원 등 훈련환경 제공에 대한 구체적인 내용을 규정할 필요가 있다. 넷째, 장애인실업팀에 대한 규정이 마련되지 않은 미규정의 경우 장애인스포츠 실업팀 설치근거를 마련하되, 실업팀 육성·지원 조항, 시설의 설치·운영 조항, 복무 관련 조항 등 세밀한 관련조례와 조항이 마련되어져야 할 것이다. This study has for its object to offer the basic materials of invigorating the business-sponsored sport teams for individuals with disabilities to promote the job stability of the disabled athletes. With this in mind, we investigate the operation status and actual conditions in the business-sponsored sport teams for individuals with disabilities and present improved ways in legislation and system in order to grow the business-sponsored sport teams for individuals with disabilities. Study methods of documentary research, interview and experts meeting were conducted. Through documentary research and interview with the person in charge, we researched the operation status and actual condition on the current domestic business-sponsored sport teams for individuals with disabilities and learned the ordinance and regulation at the base of operation. Also, we understood the problems by comparing and analyzing with high regulations at the base of foundation and operation. In the experts meeting with two university professors specialized in physical education for the disabled and two disabled sports administrators, we drew betterment for the limit of the legislation and system regarding invigoration of the business-sponsored sport teams for individuals with disabilities. Especially, we discussed the directions in research and diagnosed the problems of the current legislation and system. Through the process of discussion and examination regarding the improvement, the following reform strategies are suggested. (a) If there is a separated regulation, an article to strengthen the benefits of social security for the athletes with disabilities should be included. (b) if there is a separated article, clarify the system of operation, management, and support of the business-sponsored sport teams for individuals with disabilities and make a detailed article to guarantee the accessibility and mobility of the athlete with disability. (c) If there is a common regulation, we need to define the detailed contents to provide customized facility, equipment and training environment for the athlete with disability. (d) If there is no regulation, prepare the establishment basis of the business sports team for the disabled and it requires very detailed ordinances and articles on promoting and supporting the business team, facility installation, operation and military service.
