Introduction : Surgical removal of extracranial trigeminal schwannoma has been challenging due to anatomic complexity and rarity of the disease. Transzygomatic approach can be a good option for extracranial trigeminal schwannoma, especially in cases extended to infratemporal fossa, pterygopalatine fossa and parapharyngeal space. Authors report several cases of extracranial trigeminal nerve schwannomas which were treated using modified or combined transzygomatic approaches. Materials and Methods : 3 patients were diagnosed as extracranial trigeminal schwannoma and underwent surgical removal via modified or combined transzygomatic approach from January 2013 to June 2016. Results : We analyzed the tumor control, complication rate, and origin of tumor. Among 3 patients, combined transzygomatic approach was applied to one patient. It was combined with transcervical approach for the patient. The huge extracranial trigeminal schwannomas were completely excised in all 3 cases. There were no significant complications. Conclusion : The modified transzygomatic approach is safe and useful for removal of huge extracranial trigeminal schwannoma without facial scar and is easily combined with transcervical approach.
자율주행 환경에서 가상현실(VR)과 증강현실(AR) 경험을 통한 차량 내 인포테인먼트 시스템과 사용자 간의 인터랙션 사례 분석

완전 자율주행 자동차는 이동수단의 의미를 넘어 새로운 편의공간으로 정의되면서 실내 공간 내 사용자 경험이 중요시되고 있다. 차량의 자율주행 레벨이 높아질수록 운전자의 전방주시 역할이 감소하며 차량 안에서 소비하고자 하는 콘텐츠와 서비스 수요가 증가할 것으로 예상된다. 이에 따라 자율주행 자동차 환경에서 VR(Virtual Reality, 가상현실), AR(Augmented Reality, 증강현실)과 같은 기술이 접목되어 주행과 관련된 정보를 효과적으로 시각화하고 메타버스(Metaverse)와 같은 새로운 콘텐츠 경험을 제공할 수 있는 차량 내 인포테인먼트(Infotainment) 시스템이 적용되면서 사용자와 시스템 간 자연스러운 상호 연결을 위한 인터랙션 디자인 관점의 고찰이 필요하다. 본 연구는 자율주행 차량 내 사용자의 활동 범위가 넓어짐에 따라 차량 내 VR, AR 기술 활용이 확대될 수 있음을 확인하였고, 이를 적용하여 사용자의 콘텐츠 몰입감 또는 활동 능률을 높일 수 있는 인터페이스 요소들을 도출하였다. 추후 연구로는 자율주행 차량 내 새로운 콘텐츠 경험이 가능한 인포테인먼트 시스템이 개발되고 있음에 따라 감성 기능에 대한 만족감과 사용자의 기술 수용도를 높일 수 있는 인터랙션 방식 및 활용성에 대한 연구가 필요하다.
microRNA-183 is Essential for Hair Cell Regeneration after Neomycin Injury in Zebrafish

Purpose: microRNAs (miRNAs) are non-coding RNAs composed of 20 to 22 nucleotides that regulate development and differentiationin various organs by silencing specific RNAs and regulating gene expression. In the present study, we show that the microRNA(miR)-183 cluster is upregulated during hair cell regeneration and that its inhibition reduces hair cell regeneration followingneomycin-induced ototoxicity in zebrafish. Materials and Methods: miRNA expression patterns after neomycin exposure were analyzed using microarray chips. Quantitativepolymerase chain reaction was performed to validate miR-183 cluster expression patterns following neomycin exposure (500 μM for 2 h). After injection of an antisense morpholino (MO) to miR-183 (MO-183) immediately after fertilization, hair cell regenerationafter neomycin exposure in neuromast cells was evaluated by fluorescent staining (YO-PRO1). The MO-183 effect also was assessed in transgenic zebrafish larvae expressing green fluorescent protein (GFP) in inner ear hair cells. Results: Microarray analysis clearly showed that the miR-183 cluster (miR-96, miR-182, and miR-183) was upregulated after neomycintreatment. We also confirmed upregulated expression of the miR-183 cluster during hair cell regeneration after neomycin-induced ototoxicity. miR-183 inhibition using MO-183 reduced hair cell regeneration in both wild-type and GFP transgenic zebrafishlarvae. Conclusion: Our work demonstrates that the miR-183 cluster is essential for the regeneration of hair cells following ototoxic injuryin zebrafish larvae. Therefore, regulation of the miR-183 cluster can be a novel target for stimulation of hair cell regeneration.

Refinement of surgical techniques has allowed hearing preservation after tumor resection to be prioritized. Moreover, restoration of hearing after tumor removal can be attempted in patients with bilateral vestibular schwannomas or those with a schwannoma in the only-hearing ear. Cochlear implantation (CI) has emerged as a proper method of acoustic rehabilitation, provided that the cochlear nerve remains intact. Studies of electrical promontory stimulation in patients after vestibular schwannoma resection have demonstrated favorable results. We describe herein two cases of hearing rehabilitation via CI implemented at the time of vestibularschwannoma resection. Tumors were totally removed, and cochlear implant electrodes were successfully inserted in both cases. Also, post operative CI-aided hearing showed improved results.
Relationship between Hearing Loss and Dementia Differs According to the Underlying Mechanism

Background and Purpose The associations between hearing loss (HL) and the mechanisms underlying cognitive impairment (CI) remain unclear. We evaluated the effects of clinical factors, vascular magnetic resonance imaging (MRI) markers, and CI mechanisms on HL. Methods In total, 112 patients with CI (59% demented) and subjective HL prospectively underwent MRI, amyloid positron-emission tomography (PET), hearing evaluations, and neuropsychological tests including a language comprehension test. Patients were categorized into pure-Alzheimer’s disease-related CI (ADCI), pure-Lewy-body disease-related CI (LBCI), mixed-ADCI/LBCI, and non-ADCI/LBCI groups based on clinical features and PET biomarkers. Results The risk of peripheral HL [defined as a pure-tone average (PTA) threshold >40 dB] was higher in the pure-LBCI group than in the pure-ADCI and mixed-ADCI/LBCI groups, and lower in the presence of ADCI. The non-ADCI/LBCI group had the most-severe vascular MRI markers and showed a higher risk of peripheral HL than did the pure-ADCI and mixed-ADCI/LBCI groups. While the pure-LBCI group had a higher risk of comprehension dysfunction than the pure-ADCI group regardless of the PTA and the score on the Korean version of the Mini Mental State Examination (K-MMSE), those in the pure-LBCI group even with a better K-MMSE score had a risk of comprehension dysfunction comparable to that in the mixed-ADCI/LBCI group due to a worse PTA. Conclusions Peripheral HL could be associated with the absence of significant β-amyloid deposition in patients with CI and characteristic of the pure-LBCI and non-ADCI/LBCI groups.
