        임상연구 : 여성 호르몬과 생리 주기가 수술 후 통증에 미치는 영향

        Background: This study was designed to examine the effects of female hormones and the menstrual cycle on postoperative pain. Methods: Ninety women who underwent gynecologic surgery involving a lower abdominal incision were asked for information regarding their menstrual cycles, and blood samples were obtained to determine the progesterone and estrogen levels of the patients at the time of surgery. Patient controlled analgesia was applied to control postoperative pain and an estimate of the consumption of analgesic drugs by the patients was made. Analgesic consumption and pain scores were recorded at 2, 24, and 48 hours after operation. Results: There was no relationship observed between the concentration of progesterone and estrogen and the consumption of analgesic drugs. However, patients that were in the luteal phase at the time of surgery consumed a significantly lower amount of analgesic drugs during the 2-24 hours following surgery than patients that were in the follicular phase (19.4 ± 6.5 ml vs 24.6 ± 11.0 ml, P < 0.05), Theree was, no significant difference in pain scores between two menstrual phases. Conclusions: This study demonstrates that there is less postoperative pain experienced by patients that are in the luteal phase of their menstrual cycle at the time of surgery than in patients that were in the follicular phase of their menstrual cycle, however, the results of this study did not reveal a relationship between the blood concentration of female hormones and postoperative pain.         Performance of a Prototype Sector Chamber of the PLS Storage Ring

        포항가속기 저장링 진공시스템의 성능을 예견하기 위하여, 초도품 진공 챔버를 제작, 조립 및 테스트를 하였다. 비휘발성 흡착펌프와 이온펌프로 구성되어 있는 다섯대의 혼합펌프와 한대의 터보펌프를 사용하였다. 그 결과 낮은 10^(-10)Torr를 얻음으로서 설계된 진공도를 성취하였다. A prototype sector vacuum chamber has been fabricated, assembled and tested for the purpose of predicting the performance of the vacuum system of the PLS storage ring. The chamber has been pumped by five combination pumps consisting of a 60ι/s sputter ion pump and non-evaporable getter module(s), and a 200ι/s turbomolecular pump for roughing. The pressure obtained is low 10^(-10)Torr which is the designed vacuum. After the chamber assembly and the vacuum system are briefly described, the performance of the vacuum system is presented.

        Buerger 병 치료를 위한 국소 정맥내 Prostaglandin E1 주입

        Buerger's disease is a chronic occlusive arterial disease in which a non-arteriosclerotic lesion involves medium-sized arteries, veins, and nerves of the distal leg or arm. Sympathetic inter- ruption is indicated to improve blood flow to the involved extremity, although sympathetic blockade can provide temporally relief of vasospasm and pain. Chemical or surgical sympa- thectomy has been performed for this purpose and intravenous regional sympathetic block (IRSB) is an alternative. Guanethidine or reserpine has been administered for IRSB. Intra- arterial or intravenous systemic administration of prostaglandin El(PGE1) has been recom- mended for the treatment of Buerger's disease. We used PGE1 for intravenous regional administration as an IRSB with results as good as that of intra-arterial injection. The advantages of the method include that it is less expensive than systemic administration, less invasive than intra-arterial injection, and simplc in technical application.

        재왕절개술시의 마취도입에 관한 비교연구

        Several methods for the management of intractable pain for various conditions including malignant disorders, herpes zoster, and low back pain and/or sciatica in 80 patients were described and analyzed. Neurolytic blodks of celiac plexus were given to the patienta suffering from intra-abdominal pain from malignant origin or chronic pancreatitis. Herpes zoster was treated mainly by chemical sympathectomy via paravertebral route and occassionaly corticosteroid was administered epidurally with the local anesthetic, bupivacaine. All the patients with low back pain and/or sciatica were given epidural corticosteroid one to three times. Other peripheral verve blocks and regional corticosteroid therapy were given to the rest of the patients. Fair to excellent result was obtained in 71% of the patients but the response was poor in the remaining 28% of the patients. It seems that the earlier the patients were referred to the pain clinic, the better the results were.
