        An unusual case of microsatellite instability–high/deficient mismatch repair (MSI-H/dMMR) diffuse large B-cell lymphoma revealed by targeted gene sequencing

        Microsatellite instability-high/deficient mismatch repair (MSI-H/dMMR) status has been approved as a tissue-agnostic biomarker for immune checkpoint inhibitor therapy in patients with solid tumors. We report the case of an MSI-H/dMMR diffuse large B-cell lymphoma (DLBCL) identified by targeted gene sequencing (TGS). A 90-year-old female who presented with vaginal bleeding and a large mass in the upper vagina was diagnosed with germinal center-B-cell-like DLBCL, which recurred at the uterine cervix at 9 months after chemotherapy. Based on TGS of 121 lymphoma-related genes and the LymphGen algorithm, the tumor was classified genetically as DLBCL of EZB subtype. Mutations in multiple genes, including frequent frameshift mutations, were detected by TGS and further suggested MSI. The MSI-H/dMMR and loss of MLH1 and PMS2 expression were determined in MSI-fragment analysis, MSI real-time polymerase chain reaction, and immunohistochemical tests. This case demonstrates the potential diagnostic and therapeutic utility of lymphoma panel sequencing for DLBCL with MSI-H/dMMR.

        제2형 당뇨병 환자에서 녹색 전곡류 잡곡밥 섭취가 체중과 허리둘레에미치는 효과

        연구배경: 제2형 당뇨병 환자에서 일반적인 전곡류의 영양학적 장점을 유지하면서 거친 식감을 개선한 녹색 전곡류를혼합한 식사가 주식으로써의 만족감과 대사 지표에 미치는 효과를 알아보고자 하였다. 방법: 제2형 당뇨병 환자 52명을 대상으로 기존의 당뇨병치료는 유지하면서 녹색 전곡류가 30% 섞인 잡곡밥을 주식으로 12주간 섭취 시켰다. 모든 대상 환자를 대상으로 섭취 전․후로 몸무게, 체질량지수, 허리둘레, 혈압을 측정하였으며 혈당, 인슐린, C-펩티드 및 지질 대사 관련 혈액 검사를 시행하였다. 또한 대상 환자에게 설문조사를 시행하여 식습관과 녹색전곡류 잡곡밥의 만족감을 확인하였다. 결과: 녹색 전곡류를 섭취한 후, 모든 대상군에서 평균 체중과 체질량지수는 유의한 변화가 없었으나 허리둘레는 86.8 ±9.5 cm에서 84.9 ± 9.5 cm로 유의하게 감소하였다(P = 0.01). 반면 비만군(체질량 지수 25 kg/m² 이상)은 허리둘레가 92.0± 9.8 cm에서 90.0 ± 8.7 cm로 유의하게 감소되었을 뿐만 아니라(P < 0.01), 평균 체중이 69.2 ± 11.2 kg에서 68.4 ± 10.8kg (P = 0.012), 체지량지수가 28.0 ± 2.5 kg/m2에서 27.7 ±2.5 kg/m2 (P < 0.01)로 모두 유의하게 감소하였다. 모든 대상군 및 비만군에서 공복혈당 및 HOMA-IR은 통계적으로 유의한 변화는 없었으나 감소하는 경향을 보였다. 녹색 전곡류 잡곡밥은 식감이 부드럽고 먹기 좋았다고 하였으며, 향후 지속적인 섭취를 원한 경우가 85.7%로 식사의 만족도가 높았다. 결론: 12주간의 녹색 전곡류 잡곡밥은 제2형 당뇨병 환자의혈당 조절 및 체중 감소에는 효과는 없었지만 허리둘레 감소효과를 보였고, 특히 비만한 제2형 당뇨병 환자는 허리둘레 감소와 더불어 체중 감소도 나타나서 복부 비만 개선 효과를 보였다.

        Effect of compositional variation on the soft magnetic properties of Fe(87xy)CoxTi7 Zr6By amorphous ribbons

        The effect of the replacement of Fe by Co or B on the thermal stability and soft magnetic properties of the Fe-based amorphous metallic ribbons with Fe(87xy)CoxTi7Zr6By (x ¼ 10, 20% and y ¼ 8, 10, 12%) produced by melt-spinning technique was investigated. For the melt-spun amorphous ribbons, the values of saturation magnetization and coercivity were observed to range from 107.00 to 152.38 emu/g and from 0.012 to 0.446 Oe, respectively. The thermal properties such as Tg, Tx, and DTx were in the range of 796.7 e809.6 K, 840.2e853.5 K, and 35.8e54.5 K, respectively. In the FeeCoeTieZreB alloys, the Co substitution for Fe improved the soft magnetic properties but decreased the thermal stability. For magnetic properties, the coercivity (Hc) decreased and saturation magnetization (Ms) increased by the addition of Co. However, the supercooled liquid region (DTx) decreased by the addition of Co. Meanwhile, the B substitution for Fe had no meaningful change on the thermal stability and soft magnetic properties. The amorphous ribbon of Fe59Co20Ti7Zr6B8 exhibited the best soft magnetic properties such as the low coercivity of 0.025 Oe and the high saturation magnetization of 152.38 emu/g.

        Effect of cooling rate in [(Fe0.5Co0.5)0.75B0.2Si0.05]100-xNbx alloy system on magnetic properties

        The effect of cooling rate on the thermal stability and soft magnetic properties of[(Fe0.5Co0.5)0.75B0.2Si0.05]100-xNbx (x = 5, 6, 7, 8 at. %) system was investigated. The alloys were producedinto the form of ribbon and cylindrical rod by melt-spinning and injection casting, respectively. Theirstructure, thermal, mechanical and soft magnetic properties were investigated by x-ray diffraction,differential scanning calorimetry, universal testing machine and vibrating sample magnetometer,respectively. All of the alloys were identified as fully amorphous by X-ray diffraction. It turned out thatthe rod samples had exceptionally high saturation fields reaching 3.0 kOe, which is key properties forsensor application. Also, among these Fe,Co-based samples, the Fe35.25 Co35.25 B18.8 Si4.70 Nb6 ribbonexhibits the highest saturation magnetization with 142.1 emu/g.         Clinicopathological implications of immunohistochemical expression of TBX21, CXCR3, GATA3, CCR4, and TCF1 in nodal follicular helper T-cell lymphoma and peripheral T-cell lymphoma, not otherwise specified

        Background: The classification of nodal peripheral T-cell lymphoma (PTCL) has evolved according to histology, cell-of-origin, and genetic alterations. However, the comprehensive expression pattern of follicular helper T-cell (Tfh) markers, T-cell factor-1 (TCF1), and Th1-and Th2-like molecules in nodal PTCL is unclear. Methods: Eighty-two cases of nodal PTCL were classified into 53 angioimmunoblastic T-cell lymphomas (AITLs)/nodal T-follicular helper cell lymphoma (nTFHL)-AI, 18 PTCLs-Tfh/nTFHL–not otherwise specified (NOS), and 11 PTCLs-NOS according to the revised 4th/5th World Health Organization classifications. Immunohistochemistry for TCF1, TBX21, CXCR3, GATA3, and CCR4 was performed. Results: TCF1 was highly expressed in up to 68% of patients with nTFHL but also in 44% of patients with PTCL-NOS (p > .05). CXCR3 expression was higher in AITLs than in non-AITLs (p = .035), whereas GATA3 expression was higher in non-AITL than in AITL (p = .007) and in PTCL-Tfh compared to AITL (p = .010). Of the cases, 70% of AITL, 44% of PTCLTfh/nTFHL-NOS, and 36% of PTCL-NOS were subclassified as the TBX21 subtype; and 15% of AITL, 38% of PTCL-Tfh/nTFHL-NOS, and 36% of PTCL-NOS were subclassified as the GATA3 subtype. The others were an unclassified subtype. CCR4 expression was associated with poor progression-free survival (PFS) in patients with PTCL-Tfh (p < .001) and nTFHL (p = .023). The GATA3 subtype showed poor overall survival in PTCL-NOS compared to TBX21 (p = .046) and tended to be associated with poor PFS in patients with non-AITL (p = .054). Conclusions: The TBX21 subtype was more prevalent than the GATA3 subtype in AITL. The GATA3 subtype was associated with poor prognosis in patients with non-AITL and PTCL-NOS.

        Soft Magnetic Properties of Fe(87−x)Ti7Zr6[BxSix]0.5 Amorphous Metallic Ribbons Prepared by Melt-spinning

        The soft magnetic Fe-based amorphous metallic ribbons Fe(87−x)Ti7Zr6[BxSix]0.5 (x = 8, 10, 12%)were prepared using the melt-spinning technique to have thickness of 15-22 μm and widths of 1-2mm. Increasing the amounts of B and Si improved the soft magnetic properties: the coercivity (Hc)decreased, and the saturation magnetization (Ms) increased, the thermal and structural propertiesremained the same. Due to the high sensitivity of the alloys to the process conditions, variations inthe widths of the melt-spun ribbons induced structural modifications, resulting in significant changein the soft magnetic properties. While the ribbons made with 1 mm widths had fully amorphousstructures and soft magnetic properties, the ribbons with 2 mm widths showed semi-hard magneticproperties, which was attributed to the formation of nanocrystals inside the amorphous structures.
