Background: Acute posthypoxic myoclonic seizure is not an uncommon presentation in clinical practices. However, there have been only a few reports which include detailed description on the treatment of acute posthypoxic myoclonic status. Methods: We retrospectively analyzed the etiology, clinical course, nature and duration of myoclonus after hypoxic brain damage during a ten year period from 1992 to 2002. Patients who had possible explanations for myoclonic seizure other than hypoxic-ischemic events were excluded. Results: Eighty-two patients aged 24 to 81 years old were included in this study. All but 3 patients with pulmonary edema had a history of cardiorespiratory arrest caused by probable cardiogenic origin, acute asthma attack and upper airway obstruction. Ictal EEGs demonstrated intermittent generalized spike and polyspike activities or biPLEDs. Most patients died within a week and 6 patients remained in persistent vegetative states. The clinical outcome was not affected by the drug response of myoclonic status epilepticus or etiology of hypoxia. Conclusions: Posthypoxic myoclonic status reflects severe and diffuse cortical damage, and implies a very poor prognosis. It may be a transient manifestation of severe cortical damage before massive and irreversible neocortical neuronal death, and aggressive anticonvulsant drug treatment would not be necessary in most patients.
해면정맥동 경막동정맥루의 경정맥색전술 이후에 발생한 가역적 외전신경마비

Transvenous embolization has become the treatment of choice for cavernous sinus dural arteriovenous fistula (cDAVF). However, there are potential complications associated with this procedure such as cranial nerve palsies and venous perforations. A 66-year-old woman presented with a 2-week left periorbital swelling and conjunctival injection. Brain MRI showed engorgement of the left superior ophthalmic vein. Cerebral angiography revealed a dural arteriovenous shunt of the cavernous sinus with retrograde venous drainage into the superior ophthalmic vein. Her proptosis and conjunctival injection resolved completely after transvenous embolization of cDAVF. However, an abducens nerve palsy developed the day after the procedure, which, fortunately, resolved spontaneously. She was symptom-free at the followup evaluation 2 months later. The abducens nerve palsy related to the transvenous embolization of cDAVF was presumed due to either dense packing of the sinus, venous thrombosis, or direct nerve injury. We report a case of transient abducens nerve palsy associated with transvenous embolization of cDAVF, suggesting the benign course of this complication. 66세 여자 환자가 2주 전부터 시작된 좌측 눈의 결막충혈과 안구돌출을 주소로 입원하였다. 입원 당일 시행한 뇌자기공명영상촬영에서 좌측위비스듬근 위에 팽창된 상안정맥이 관찰되었고, 다음날 시행한 뇌혈관 조영술에서 내경동맥의 아래뇌하수체동맥과 외경동맥의 내상악동맥의 분지가 해면정맥동으로 많은 수의 루를 형성하고 있었다. 경정맥 색전술 후 결막충혈과 안구돌출 증상은 즉시 호전되었으나, 다음날 좌측외전신경마비가 발생되었다. 특별한 치료없이 경과 관찰하였으며, 2달 후 증상은 저절로 회복되었다. 저자들은 경막동정맥루의 치료를 위한 경정맥색전술 이후에 발생한 가역적 외전신경마비를 경험하여 이를 문헌고찰과 함께 보고하는 바이다.

2025년, 우리나라는 노인 20.6%로 초고령사회로 진입할 것으로 전망되고 있다. 이에 본 연구는 지속적인 건강 관리를 위하여 디지털 기술과 높은 교육 수준을 지닌 베이비 붐 세대를 대상으로 ‘인지·정서 향상 챗봇 융합제품’의 사용에 따른 전후 건강상태 변화를 측정하였다. 연구대상은 건강공동체 23개소 중에서 서울지역의 중·고등학교 이상의 학력으로, 스마트기기 사용에 능숙하며, 자발적 사용 경험에 따른 데이터 제공 의지가 있는 대상자 50명을 선별하였다. 자료 수집 기간은 2023년 7월 24일부터 10월 5일까지이다. 50명의 신청자 중 탈락자와 소수의 남자를 제외한 43명의 베이비 부머 세대 여성을 대상으로 분석하였다. 연구결과 챗봇 사용 후 신체활동은 43.5.21(±1310.39) MET, 우울 정도는 6.84(±3.53) 통계적으로 유의하게 차이가 있었다. 그리고, 챗봇 사용 전후로 두 변수간에 통계적으로 유의한 상관성은 없었다. 신체활동과 우울척도에서만 유의한 효과가 나타난 것은 챗봇이 특정 건강 측면에 미치는 효과가 있을 수 있음과 각각에 다른 영향을 미쳤음을 시사한다. 향후 연구에서는 더 많은 변수와 더 다양한 건강 지표를 고려하여 챗봇의 효과를 더 깊이 분석할 필요가 있다. By 2025, South Korea is expected to enter an ultra-aged society with the elderly comprising 20.6% of the population. We measured changes in health status before and after by the use of a "Cognition-Emotion Enhancement Chatbot Integrated Product" among Baby Boomer generation women. Fifty participants, proficient in smart device usage and willing to provide data, were selected from health communities in Seoul. After excluding some applicants, 43 Baby Boomer women were analyzed. Results revealed significant differences in post-chatbot use physical activity (43.5.21 ± 1310.39 MET) and depression levels (6.84 ± 3.53). Correlation between the two variables was not statistically significant. The findings suggest specific effects of the chatbot on physical activity and depression, emphasizing the need for future research with diverse health indicators.
복합근육활동전위의 시작잠복기와 진폭에 대한 기준전극의 영향

Background: Background: In belly-tendon (bipolar) montage, reference (R2) electrode placed on muscle’s tendon has traditionally been considered to be electrically inactive. However, recent studies have revealed that R2 electrode is not simply referential, but actively contributes to compound muscle action potential (CMAP) waveform morphology. These findings suggest that CMAP onset latency and amplitude may also be influenced by the position of R2 electrode. This study was performed in order to evaluate the effect of R2 electrode position on CMAP onset latency and amplitude. Methods: We performed motor nerve conduction studies of median, ulnar, tibial and peroneal nerves on bilateral limbs of 20 normal subjects. We used traditional bipolar and monopolar montage and compared their CMAP onset latencies and amplitudes. In bipolar montage, recording (R1) electrode was placed on mid-belly of muscle with R2 electrode on the tendon of the muscle. In monopolar montage, R1 electrode was placed on the same site of bipolar montage, while R2 electrode was placed on the contralateral limb. Results: The mean CMAP onset latencies of median and peroneal nerves in bipolar montage were significantly different (p<0.05) with those in monopolar montage. And those of ulnar and tibial nerves were not significantly different (p>0.05). The mean CMAP amplitudes of all the tested nerves except ulnar nerve were significantly different (p<0.05). Conclusions: This study shows that change in R2 electrode position can affect the CMAP onset latency and amplitude, and these differences seem to be related to the generation of far field potential by CMAP.

Reversible splenial lesion may occur in encephalitis rarely. A 21-year-old man was admitted with viral encephalitis. High signal on the diffusion MRI and low signal on the ADC map image was showed in the splenium. As the patient improved, the lesion suggestive of cytotoxic edema was completely resolved one month later. Although the concrete pathophysiology of focal cytotoxic edema in encephalitis is unclear yet, MRI is valuable diagnostic tool to manage encephalitis in the view of prognosis.
