      • Propofol, Fentanyl, Lidocaine에 의한 마취유도가 기관내 삽관에 미치는 영향

        저자들은 근이완제를 사용하지 않은 상태에서 propofol을 마취유도제로 사용시, 단독 사용 또는 전투약제로 fentanyl이나 lidocaine을 사용한 후 기관내 삽관을 수행할 수 있는 적절한 후두 상태 및 삽관 가능성을 연구하였다. 방법: 수술의 종류에 관계없이 미국마취과 학회 환자분류 1 또는 2급에 속하는 36명의 환자를 대상으로 하였다. 제1군은 propofol 2.5 ㎎/㎏만을 사용한 군, 제2군은 propofol 2.5 ㎎/㎏에 fentanyl 4 mcg/f㎏을 사용한 군, 제3군은 propofol 2.5 ㎎/㎏, fentanyl 4 mcg/㎏ 과 lidocaine 1 ㎎/㎏을 사용한 군으로 하였다. 전투약제의 사용은 먼저 lidocaine을 투여후 2분이 지난다음 20초이상에 걸쳐서 fentanyl을 투여하고 1분이 경과후 40초이상에 걸쳐 propofol을 투여하였다. 환자가 의식이 소실된 다음 기관내 삽관을 시도하였으며 삽관의 상태는 후두경 조작의 용이도, 성대의 위치, 삽관 튜브를 삽입후 나타나는 기침반사등의 정도에 따라 주어진 평가기준에 기초하여 얻어진 점수에 따라 만족도를 평가하였다. 결과: 전반적인 삽관 상태의 평가시 만족도는 제1군이 58%, 제2군이 75%, 제3군이 83% 를 보였다. 특히 전투약제로 fentanyl과 lidocaine을 사용한 제3군에서는 제1군이나 제2군에 비해 삽관 상태가 양호했으며 삽관 후 발생되는 기침반사등이 훨씬 감소됨을 발견할 수 있었다(p<0.01). 결론: 근이완제를 사용하지 않은 상태에서 propofol을 사용한 후 시도한 기관내 삽관은 적절하지 못하다고 사료되었으며, 전투약제로 fentanyl이나 lidocaine을 사용하였어도 삽관튜브에 의한 후두 및 기관의 반사를 완전히 억제시키지는 못했다.

        Dynamic control schemes for robot manipulators have been studied for high speed motion and precise control. Computed torque technique which is ond of the dynamic control schemes, needs calculation of required control torque through robot manipulator dynamic equations. There are 2 ways in calculation of required control torque : real-time computational method of manipulator dynamics such as Newton-Euler method, and look-up table method with off-line computation of manipulator dynamics such as configuration space method. In configuration space method, very much amount of memory is demanded to assure good accuracy in the computed control torque. In this paper, an improved configuration space method based on Taylor series expansion is proposed and it is shown that it is shown that it has better performance in accuracy than the conventional configuration space method. It is also shown that the improved configuration space method is more efficient when more degrees of freedom are required. A case study for robot manipulator PUMA 560 is also conducted and it is shown that the improved configuration space method is better than the conventional configuration space method from the point of accuracy and amount of required memory.

        In this paper we developed the Print Circuit Plotter that can fabricate the PCB pattern. In this study we developed control program in two ways by using PCL­839 controller for precise control the print circuit board plotter. The first is Image Tool that can produce image data through inputed gerber data. The second is PCB Milling Tool that can milling fabricate in which produce the NC data through inputed image data.

        전기경련요법 시 Clonidine 이 심혈관계와 경련기간에 미치는 영향

        Background: Elecroconvulsive therapy (ECT) is frequently associated with cardiovascular complications such as hypertension and tachycardia. The aim of this study was to evaluate whether clonidine given as an oral preanesthetic medication would inf1uence the hemodynamic stress response, peripheral oxygen saturation and seizure duration which follows ECT. Methods : Twenty-two ASA physical status 1, ll patients with major depressive disorders were included in a crossover study design and assigned randomly to either a control group who received placebo, or a clonidine group who received oral clonidine of 3 mg/kg 90 min before preparation of ECT. All patients received glycopyrrolate 0.2 mg intramuscularly 60 min before anesthetic induction. Electrocardiography, pulse oximetry, and blood pressure monitors were applied to all patients. Patients were pre-oxygenated with 100% 02. Patients received thiopental 2.5 mg/kg and succinylcholine 0.5 mg/kg for anesthetic indtlction. Noninvasive mean arterial blood pressur (MAP), heart rate, and oxygen saturation were recorded .iust before test drug admlinistration, immediately before ECT, and each minute for five minutes after ECT. The times from ECT stimulus to the cessation of clonictonic motor activity in the $quot;isolated$quot; arm were noted. Results : There was a significan decrease in MAP (P = 0.007) through the peri-ECT period in groups with oral clonidine pretreatment (3 mg/kg) relative to the control group. There were no significant differences in heart rate and peripheral oxygen saturation values between two groups. The duration of motor sizure activity was similar between the clonidine pretreatment and placebo groups. Conclusions: We conclude that oral clonidine 3 mg/kg as a pretreatment medication is effective in attenuating the MAP increase in routine ECT. (KOREAN J Anesthesiol 2000: 38: 243 ~250)

      • 단층영상으로부터의 표면 재구성 방법 연구

        This paper addresses a new surface reconstruction scheme for approximating the isosurface from a set of tomographic cross sectional images. Differently from the novel Marching cube algorithm, our method does not extract iso-density surface(isosurface) directly from the voxels but calculates the iso-density point(isopoint) first. After building the relatively coarse initial mesh by the Cell-boundary algorithm approximating the isosurface, it produces the final isosurface by iteratively shrinking and smoothing the initial mesh. Comparing with the Marching Cube algorithm, our method is robust and does not make any crack in resulting surface model. Furthermore, proposed method surmounts the O(1)-adjacency limitation of MC in defining the isopoints by permitting the O(2) and O(3)-adjacent isopoints in surface reconstruction, and can produce more accurate isosurface. According to experiments, it is proved to be very robust and efficient for isosurface reconstruction from cross sectional images.

        Kidney scan에 대한 고찰

        The kidney scan and the excretory urography of 21 cases in kidney disease proved clinically or pathologically at Uonsei University, Severance Hospital were reviewed and analysed briefly. RESULTS : 1. In all except 2 cases of pyelonephritis, changes on the kidney scan were noted with generalized mottled density and cold area in the kidney, and with diffuse poor or non uptake density and various size or shape of the kidney. 2. On the excretory urogram, the demonstrable findings n the kidney parenchyma were discribed on the only 7 of 21 cases, hypernephroma, tuberculosis kidney, necrotizing papillits, medullary sponge kidny, and stenosis of relnal artery. 3. Three of all cases, renal carbuncle, retroperitoneal hematoma with aberrent vessel and angiolipomyoma were o appreciable finding on the pyelography, but on the view of kidney scannig, there were apparently demonstrated abnormal cold area and some enlargement of kidney size. We concluded that the close correlation of the two techniques, kidney scanning and excretory urography on the kidney disease leads to more improved diagnostic accuracy.
