기저세포암의 bcl - 2 발현에 대한 면역조직화학적 연구

Background: Basal cell carcinoma(BCC) is the most common tumor of light-skinned people. Most of them are relatively innocuous, but some may cause extensive tissue destruction, and result in death by local infiltration of vital structure or metastatic disease. The bcl-2 is a protooncogene involved in tumorigenesis by blocking apoptosis or programmed cell death. Objective : To investigate whether there was a difference in bcl-2 expression in different histopathological types of BCC. Method: We performed immunohistochemical staining by using a monoclonal mouse anti-human bcl-2 oncoprotein in 28 BCC(nodular: 7 cases, micronodular; 3 cases, adenoid; 1 case, infiltrative; 9 cases, and mixed; 8 cases). Result: All 28 BCC expressed bcl-2, and the degree of staining was different according to histopathological types; poor prognostic micronodular(100%) and infiltrative(73.3%) types were relatively strong, and nodular(66.7% ) and adenoid(75.0%) types were moderate. Conclusion . These results suggest that the degree of bcl-2 expression in BCC seems to be dependent to the histopathological types together with clinical behavior. Papular elastorrhexis is a connective tissue nevus that occurs in the second decade of life as asymptomatic small white-creamy papules scattered over the chest, shoulder, abdomen, or back Histopathologically, there is a decrease in elastic fibers with focal homogenization of collagen. Recognition of this entity is important to prevent use of unnecessary diagnostic procedures and therapy. We present three cases of papular elastorrhexis. Topical immunotherapy with diphenylcyclopropenone(DPCP) has been used for the treatment of alopecia areata. Due to the therapeutic principle of producing a contact eczema, itching, erythema and scaling are inevitable or even desired side effects. However, erythema multiforme-like reactions following topical DPCP treatment have been rarely reported with an estimated incidence of 1.2%. We present herein a case of Stevens-Johnson syndrome, the severe form of erythema multiforme as a side effect of topical DPCP. A 57-year old male patient visited us for the treatment of alopecia totalis. After sensitization with 0.2% DPCP in acetone, we applied DPCP on the scalp once a week for three weeks. Following the 3rd challenge of DPCP, iris-shaped lesions, erosions, vesicles, and bullae developed with fever. Also, he had vesicles and erosions in the oral cavity. The patient was treated with systemic antibiotics, steroids, and antihistamines. The cutaneous lesions were cleared with hyperpigmentation, and pronounced hair regrowth was observed. N/A Cutaneous hypersensitivity reaction due to Rhus can develop against antigens delivered through the epidermis(allergic contact dermatitis: ACD) or through the blood vessels(systemic contact dermatitis: SCD). Although the role of Langerhans cells(LC) in sensitization and elicitation of ACD(external antigen) has been extensively investigated, there are few studies in cases of SCD(internal antigen). Immunohistochemical differences of Langerhans cells were investigated in cases of two different routes(external antigen vs. internal antigen) of the same antigens. The 7 cases of ACD due to Rhus and the 14 cases of SCD due to ingestion of Rhus were involved in this study. Immunohisto - chemical staining with anti-CDla, S-100 protein, HLA-DR, factor XIIIa antibodies were performed. Numbers of CDla, S-100 protein, and HLA-DR positive cells per linear millimeter of epidermis were counted. The ratio between HLA-DR positive cells and CDla positive cells(HLA-DR/CDla) was considered as a possible index of activation of LC. The number of factor XIIIa positive dermal dendritic cells(DDC) per mm was evaluated. CDla positive LC were more significantly numerous in ACD than in SCD(19.6±1.95 vs. 9.0±0.91 p<0.05). Also, HLA-DR/CDla in ACD were higher than SCD, Factor XIIIa positive DDC were more numerous in SCD than in ACD(21.6±1.57 vs. 16.9±2.48), but the differences were not significant statistically. Therefore, the epidermal LC appear to be involved in antigen presentation in ACD (external antigens) as to be well-known. On the other hand, it seems possible that DDC plays some role in immune reactions in SCD (intemal antigens).
A Case of Disseminated Perforating Granuloma Annulare in a Child

Granuloma annulare is a common, benign, inflamatory, usually self-limited dermatosis. Disseminated perforating granuloma annulare is a rare variant of granuloma annulare. A 23-month-old female patient had asymptomatic, multiple umbilicated papules on the face, limbs including palms and soles, buttock and trunk for about 5 months. The new lesions have developed with mild fever or symptoms of upper respiratory infection. Histopathological examination revealed transepidermal elimination of mucinous degenerated collagen fibers and surrounding palisading lymphohistiocytic granuloma. After she was treated with prednisolone and antibiotics, the lesions slightly resolved, but thereafter, new lesions have developed frequently with mild fever. We herein reported a rare case of disseminated perforating granuloma annulare in the youngest patient yet reported.
