This paper presents a systematic approach to the input-output data-based fuzzy modeling for the complex and uncertain nonlinear systems, in which the conventional mathematical models may fail to give the satisfying results. To do this, we propose a new method that can yield a successful fuzzy model using a mGA hybrid schemes with a fine-tuning method. We also propose a new coding method fo chromosome for applying the mGA to the structure and parameter identifications of fuzzy model simultaneously. During mGA search, multi-purpose fitness function with a penalty process is proposed and adapted to guarantee the accurate and valid fuzzy modes. This coding scheme can effectively represent the zero-order Takagi-Sugeno fuzzy model. The proposed mGA hybrid schemes can coarsely optimize the structure and the parameters of the fuzzy inference system, and then fine tune the identified fuzzy model by using the gradient descent method. In order to demonstrate the superiority and efficiency of the proposed scheme, we finally show its applications to two nonlinear systems.

Background and Objectives The aim of this study was to evaluate the incidence and prognostic significance of cervical lymph node metastasis in squamous cell carcinoma (SCC) of the hypopharynx. Subjects and Method A retrospective review of the 64 patients who were previously untreated for SCC of the hypopharynx and underwent surgery was performed from October 1993 to June 2008. Fifty-six patients had simultaneous bilateral neck dissection, whereas eight had unilateral neck dissection. Results The median age was 61.0 years (range, 34-75 years) for the study group consisting of 62 males and two females. Evaluating according to the N stages, there were 15 (23.4%), 10 (15.6%), 37 (57.8%), and 2 (3.1%) cases with N0, N1, N2 and N3, respectively. Forty-nine patients (76.6%) had pathologically proven cervical metastasis. Contralateral occult lymph node metastasis occurred in 20.5%. Ipsilateral and contralateral occult metastasis rates for clinically node negative patients were 41.2% and 11.8%, respectively. The most frequent sites for positive neck nodes occuring at each level were as follows: II (48.5%), level III (40.6%), level IV (26.6%), paratracheal node (21.4%), level V (9.4%), levels I (7.8%), and retropharyngeal node (6.3%). The 5-year overall survival and disease-specific survival rates were 50% and 53%, respectively. Cervical nodal metastasis (p=0.044) was statistically significant prognostic factors for disease-specific survival. Conclusion Metastasis to the cervical lymph node group is very frequent and has an impact on survival in patients with hypopharyngeal SCC. Therefore, we advocate bilateral neck dissection in patients with hypopharyngeal SCC with clinically positive metastasis. Ipsilateral elective neck dissection may be needed for clinically node negative patients.

Wegeners granulomatosis is a granulomatous inflamatory proces with necrotizing vasculitis. It can involve any organ in body, granulomatosis presenting as bilateral facial paralysis. A 54-year-old man was sufering from nasal stufiness and rhinorrhea and treated with antibiotics. In spite of the treatment, the nasal symptoms were agravated and left facial paralysis was developed suddenly with left hearing impairment. Diagnosis as Wegeners granulomatosis was suported by biopsy from nasal cavity. He right facial paralysis was emerged abruptly. At 6 months after discharge, his facial nerve continued to function at a House-Brackmann grade II level. (Korean J Otorhinolaryngol-Head Neck Surg 2007 ;50 :822-5)

The simultaneous occurrence of two distinct neoplasms derived from different cells of origin is sometimes recognized. In the thyroid gland, two such distinct tumors could consist of a concurrent squamous cell carcinoma and well-differentiated carcinoma showing follicular epithelial cell differentiation. Because the squamous cell carcinoma is characterized by rapidly progressive clinical course in spite of its differentiated morphologic feature, the tumor should be treated more vigorously at its initial stage. We report a rare, interesting case of concurrent squamous cell and papillary carcinoma of the thyroid gland. A 69-year-old male underwent a total thyroidectomy, central neck dissection and bilaterally modified radical neck dissection, which was diagnosed as papillary carcinoma by ultrasonography-guided aspiration cytology. The pathological report of the surgical specimen of the thyroid gland revealed synchronous squamous cell carcinoma and papillary carcinoma. He received local radiotherapy but died of pneumonia eight months after surgery. (Korean J Otorhinolaryngol-Head Neck Surg 2008;51:1065-7)
Morphologic Assessment of Polycaprolactone Scaffolds for Tracheal Transplantation in a Rabbit Model

The purpose of this study was to clarify whether a polycaprolactone (PCL)-framed porous tracheal scaffold could be used for the replacement of the trachea in rabbits and produce better results in terms of luminal epithelialization. The tracheal scaffold consists of a PCL-framework and a collagen layer. After a longitudinal cervical skin incision,the trachea was exposed and a rectangular defect (1×0.5 cm) was created on the cervical trachea by a scalpel on eight rabbits. PCL-scaffold was trimmed and fixed to defect boundaries with Tisseel. Postoperatively, the site was evaluated endoscopically and histologically. Bronchoscopic examinations at 1 week revealed that implant exposure was recognized in the entire length of the prosthesis. The luminal surface of the implanted scaffold was partially covered at 2 weeks and completely covered at 4 weeks. Histologic data showed that the epithelial lining was nearly completed 1 week after surgery and some inflammatory cells were seen in the submucosa. At 2 weeks, the epithelium was already covered and the migration of inflammatory cells was not observed. However the concentration of cilia was not observed at this week. At 8 weeks there was also a neovascularization with luminal epithelialization. These findings suggest that a PCL-framed porous tracheal scaffold used in our experiment is an effective way to regenerate the epithelium on the surface of an artificial trachea.

Korean Society of Thyroid-Head and Neck Surgery appointed a Task Force to provide guidance on the implementation of a surgical treatment of oral cancer. MEDLINE databases were searched for articles on subjects related to “surgical management of oral cancer” published in English. Results were restricted to systematic reviews, randomized control trials/controlled clinical trials, and observational studies. The quality of evidence was rated with use RoBANS (Risk of Bias Assessment Tool for Nonrandomized Studies) and AMSTAR (A Measurement Tool to Assess the Methodological Quality of Systematic Reviews). Evidence-based recommendations for practice were ranked according to the American College of Physicians grading system. Additional directives are provided as expert opinions and Delphi questionnaire when insufficient evidence existed. The Committee developed 68 evidence-based recommendations in 34 categories intended to assist clinicians and patients and counselors, and health policy-makers. Proper surgical treatment selection for oral cancer, which is directed by patient- and subsite-specific factors, remains the greatest predictor of successful treatment outcomes. These guidelines are intended for use in conjunction with the individual patient’s treatment goals.

Pneumoparotid is a rare, but well reported disease. It is caused by increased intraoral pressure. Cough, wind instrument playing, ballooning of cheek and nervous tics may increase intraoral pressure. We recently experienced a interesting case of pneumoparotid after lifting of heavy luggage. A 20-year-old man was suffering from enlargement of left parotid area, heating sensation of left cheek, bilateral hearing impairment and click sound when he opened mouth. In spite of intravenous antibiotics and antiinflammatory drug administration and conservative treatment including warm compression, massage and hydration, the enlargement of left parotid was not improved. So we performed the ligation of left Stensen’s duct under local anesthesia. His symptoms subsided after operation. Now, the patient continues to be observed without any trouble. Head and neck squamous cell carcinoma (HNSCC) is the sixth most common cancer worldwide. Despite advances in therapy, which have improved the quality of life, survival rates have remained static for many years. To develop more effective therapies for HNSCC, it is essential that we gain a deeper understanding of its biology and the cells that are responsible for recurrent and persistent cancers. Evidences have been accumulated indicating that only a minority of cancer cells with stem cell properties are responsible for maintenance and growth of the tumor. These cells have been designated as cancer stem cells (CSC) for they have been shown to have stem cell-like characters: self-renewal, tumorigenesis and the ability to recapitulate a heterogeneous tumor. A recent work on the role of HNSCC CSC in tumorigenesis has shown that CSC have a greater capacity for tumor growth, increased motility, and invasive characteristics. Clinically, growth of CSC has been shown to be augmented in recurrent disease, treatment failure, and metastasis. CSC is a novel target of study given their slow growth and innate mechanisms conferring treatment resistance. Further understanding of their unique phenotype may reveal potential molecular targets to improve therapeutic and survival outcomes in patients with HNSCC.
