Laparoscopic surgery is frequently used in various surgical procedures. The artificial pneumoperitoneum using carbon dioxide is an essential element in laparoscopic surgery. However, intra-abdomonal insufflation stretches the peritoneum, which increases vagal tone and can lead to life-threatening bradyarrhythmias including asystole. We report two cases of severe bradycardia and impending cardiac arrest at the beginning of peritoneal insufflation in patients undergoing laparoscopic surgery. In first case, we performed cardiopulmonary resuscitation with external cardiac compression and epinephrine administration to prevent progressing into cardiac arrest. In second case, we used atropine while deflating the abdomen. The patients were recovered without any problem. Anesthesiologists should be aware of the potential for this critical complication.

Cardiac arrest during spinal anesthesia are considered rare, but are actually relatively common. We present a case of severe bradycardia and asystolic cardiac arrest during spinal anesthesia in young healthy patient scheduled for minor surgery. About 25 minutes after the spinal anesthesia, severe bradycardia and cardiac arrest suddenly occurred. The patient recovered completely after cardiopulmonary resuscitation with Trendelenburg position, rapid hydration, epinephrine administration, manual ventilation with 100% oxygen, and external cardiac compression. Anesthesiologists should be aware of the potential for this unexpected critical complication.

This study aims to analyze the frequency of Asian dust occurrences and the factors related to dust rises in the source region using the MODIS Land Cover data, WMO SYNOP data, and climate index data. All dust rise cases at sites which experienced more than 20 time dust rise events during the 21 years from 1997 to 2017 are analyzed. The most frequent dust rise occurred over the barren region followed by grasslands and croplands. It is found that the frequency of Asian dust events is positively correlated with the number of days on which wind speed exceeds the threshold value. It is also found that the number of days over the threshold wind speed in the barren region is negatively correlated with the wintertime North Atlantic Oscillation (NAO) and that the number of days over the threshold wind speed in grassland is negatively correlated with the El Nino/Southern Oscillation (ENSO) in spring.

The i-gel was designed to avoid compression or displacement trauma from the inflatable laryngeal mask airway (LMA) cuff. We present a case of transient cyanosis on both lateral sides of the tongue observed after use of the i-gel which is believed to have been caused by transient obstruction of the lingual artery due to compression of the tongue. Careful attention is required to reduce injuries associated with direct compression when using LMA including i-gel.
전산유체역학을 이용한 응집지 유입 분배수로의 유량 균등배분에 관한 연구

The water should be evenly distributed to each flocculation/sedimentation basin because it can affect the floc formation and the efficiency of sedimentation. In order to find the optimum operation method for the equal distribution of the flow rate by using the CFD (Water-CFD), the flow analysis was carried out by considering the basic operation condition and improvement plans. From the results of the CFD simulation, the following conclusions were obtained. When 8 all flocculation/sedimentation basins were operated, the influx of the outermost 1st and 8th flocculation/sedimentation basins were the largest but the amount of 4th and 5th flocculation/sedimentation basins which were located at the center of the distribution channel were the least. The operation rate of water purification plant was 40% compared to facility capacity, therefore it was considered to stop the flocculation/sedimentation basins. The optimum operation method to minimize the influent flow difference was the stopping the 4th and 5th flocculation/sedimentation basins located at the center. The other methods such as the extension of the distribution channel(waterway) and the installation of the disturbance plate near the inlet of flocculation basin need the civil engineering work and the lots of extra costs. However, the effect of the equal distribution is not greater than the change of operation method.
