        실험적 고실개방술로 진단된 중이 강 내 부신경절종 1례

        Tympanic paraganglioma is the most common neoplasm in the middle ear. There are several distinctive signs and radiologic findings which is supportive to diagonosis. The most common initial symptom is pulsatile tinnitus. In this study, we present a case of the middle ear paraganglioma of a patient with tinnitus and hearing difficulty. We also discussed current surgical approach and precautions when diagnose and treatment of this tumor.

        基於STIRPAT模型的碳排放驅動因素研究 -以中國河北省為例-

        As an important industrial district in China, Hebei Province has been an important factor influencing residents' living standards and impeding sustainable economic development in recent years. In order to further analyze the causes of atmospheric pollution in Hebei Province, this paper uses Kaya identities as the theoretical basis to use the STIRPAT model to decompose the carbon emissions in Hebei Province from 1997 to 2017. And at the same time, in order to verify the existence of the environmental Kuznets curve, in this paper, when adding variables, the square term representing economic development is added. The results show that factors such as population size and carbon emissions per unit of energy consumption contribute to carbon emissions, and there is no environmental Kuznets curve in Hebei Province. According to the results, this paper puts forward corresponding suggestions from the aspects of changing the economic development mode, improving the technical level, and formulating a reasonable population policy. 河北省作為我國重要的工業省份,近年來,環境問題一直成為影響居民生活水準、阻礙經濟可持續發展的重要因素。為進一步剖析造成河北省大氣環境污染的原因,本文以Kaya恒等式為理論依據,運用STIRPAT模型對河北省1997-2017年的碳排量進行分解 ,同時,為了驗證環境庫茲涅茨曲線的存在性,本文在變數選取時加入表示經濟發展的平方項。結果表明,人口規模、單位能耗碳排量等因素對碳排量起到促進作用,且河北省目前尚不存在環境庫茲涅茨曲線。根據結果,本文從改變經濟發展方式、提高技術水準、制定合理的人口政策等方面提出了相應的建議。

        전정 질환 환자에서 억제 두부충동검사의 임상적 유효성

        Background and Objectives The suppression head impulse (SHIMP) test has emerged as one of the new vestibular function tests. The SHIMP test is an analysis of anti-compensatory saccadic movements in gazing moving targets as the head moves. In this study, we investigated the accuracy and sensitivity of the test. Subjects and Method We analyzed the results of SHIMP test (gain, amplitude, and latency) in 24 cases, which included normal, vestibular neuritis, benign paroxysmal positional vertigo (BPPV), Meniere’s disease, bilateral vestibular function loss, temporal bone fracture, tuberculosis meningitis and acoustic schwannoma patients. We also carried out a physical exam, audiologic study, video electronystagmography (vENG) and a conventional video head impulse test to evaluate the feasibility of SHIMP test in cases of vesitibulopathy. Results In patients with vestibulopathy, the analysis of SHIMP test indicated high sensitivity and accuracy, which were supported by accompanying audio-vestibular studies. Patients suspected with BPPV, vestibular neuronitis and Meniere’s disease showed low vestibulo ocular reflex (VOR) gain and delayed anticompensatory saccade with smaller and more scattered amplitudes than the control. Patients with bilateral vestibular function loss, temporal bone fracture, tuberculosis meningitis, and acoustic schwannoma showed delayed latency, and small amplitude or few anticompensatory saccades. Finally, changes in the SHIMP test values might be a hint that patients had recovered from vestibular neuronitis and tuberculosis meningitis. Conclusion We found the feasibility of SHIMP test in detecting the pathologic condition of VOR in vestibulopathy patients. We argue that the usefulness of SHIMP test might be extended to evaluating the effectiveness of rehabilitation.

        Nonshaved Ear Surgery: Effect of Hair on Surgical Site Infection of the Middle Ear/Mastoid Surgery and Patients’ Preference for the Hair Removal

        This study aimed 1) to compare the rates of surgical site infection(SSI) between two groups with and without preoperative hair shaving, 2) to comparethe bacterial colonization just before the skin incision between them, and 3) to evaluate people’spreference for the hair shaving. Subjects and Methods: The retrospective study enrolledcases in which middle ear and mastoid surgery was performed with as well as withouthair removal. Main measurement outcomes were the SSI rate within 3 months following thesurgery, bacterial culture results obtained from the incision area just before the skin incision,and questionnaire to evaluate the preference for hair shaving from patients withchronic suppurative otitis media but without experience with the ear surgery. Results: Thisstudy did not show any difference in the rates of SSI and bacterial colonization betweentwo groups with and without preoperative hair shaving. Most patients without experiencewith the ear surgery chose the nonshaved ear surgery, even though the questionnaire presenteda comment as follow; “Your hair will always grow back as the growth speed ofabout 1.25 cm per month.” Conclusions: There is no evidence showing that preoperativeshaving of the surgical site is helpful for the SSI than no hair removal. Nonshaved middle earand mastoid surgery via postauricular approach appears to be preferable. Contrary to doctors’popular belief, the hair shaving can cause psychological discomfort, especially for women. Now is the time to keep the balance between the professional’s perspective and the patients’preferences.

Na-Bentonite is made use of the field of pharmacology by emulsifying agent, stability agent.

        제주 연약지층 특성을 고려한 해상풍력기초의 수치해석적 연구

        최근 해상풍력발전 단지 조성은 많은 양의 에너지를 창출할 수 있어 그 기대는 점차적으로 증가하고 있다. 특히 제주도는 풍황이 우수하여 해상풍력발전 시스템 운영을 위한 최적의 대상지이기는 하나, 화산활동에 의해 형성된 지형으로 육지부와 달리 기반암인 현무암층 사이에 연약층인 화산쇄설물 및 공동이 불규칙하게 발달된 층상구조로 이루어져 있다. 즉, 해상풍력발전기 기초저면 기반암에 연약한 화산쇄설물이 존재하는 경우 풍력발전기의 주기적인 진동으로 지반의 강도가 저하됨은 물론 장기침하로 인하여 풍력발전기의 장기적인 안정성 확보가 어려울 수도 있다. 본 논문에서는 제주도의 3개 지역에 대한 지표지질조사와 실내실험을 통해 도출된 설계정수를 파악한 후, 수치해석을 통하여 층상구조로 이루어진 연약지층에 모노파일 설치시 연직변위와 응력분포 결과를 도출하였다. Recently, offshore wind farms are increasingly expected, because there are huge resource and large site in offshore. Jeju island has optimum condition for constructing a wind energy farm. Unlike the mainland, Jeju island has stratified structure distribution between rock layers sediments due to volcanic activation. In these case, it can be occur engineering problems in whole structures as well as the safety of foundation as the thickness and distribution of sediment under top rock layer can not support sufficiently the structure. In this paper, the settlement and stress distribution is predicted by numerical analysis when the mono-pile base are constructed on various soft layer between stratified structure. To determine the settlement of the pile foundation supported on stratified rock layer, the geological investigation at the 3 regiions and the results of laboratory experiments of the stratified rock layer is required.
