국내 작업환경측정기관들의 개인시료 채취기 운용현황 실태 조사

Objectives: The principal aim of this study is to provide fundamental data for suggesting substantial management plan of personal air sampler by investigating current situation of work environment monitoring service provider. Materials and Methods: The questionnaires regarding a personal air sampler were sent by electronic mail to 150 institutions, except for 22 institutions which are out of business or closed temporarily, among total 172 domestic work environment monitoring service providers designated by Ministry of Employment and Labor based on 2013. The institutions which did not reply to the questionnaire for 30 days were required directly by phone call. Finally 97 institutions gave an answer to the questionnaire and therefore the respondence rate was 65%. Results: The normal operation rate of personal air sampler exceeded 70% in all the types (particle, gas and both combination) in terms of purchased year. Furthermore the personal air sampler purchased recently showed a trend of high operation rate compared to the personal air sampler purchased previously. The distribution patterns of personal air sampler In terms of manufacturers were Gillian>MSA>SKC for particle, Gillian>SKC>AMETEK for gas and Gillian>MSA>Casella for both combination, respectively. As a result, it was found that the personal air sampler manufactured by Gillian company was utilized the most in domestic work environment monitoring service provider regardless of types of personal air sampler. Conclusions: Based on the results obtained from this study, the normal operation rate according to types of personal air sampler was as follows: 94% for personal air sampler for collecting particle only, 93% for personal air sampler for collecting gas only and 89% for collecting both particle and gas. Overall, the numbers of personal air sampler possessed and operated normally in domestic work environment monitoring service provider were 5,458 and 5,062, which indicates the normal operation rate of personal air sampler in total aspect is 93%.

The diagnosis hematology test is not only the most of patient medical examination and treatment based on use, but also in representative routine CBC test a kind of patient therapy along with variable be exchange, in capillary blood have each blood component exactly calculation diagnosis and therapy important mean. Recently, computer industry development be owing to quality control the concept of delta\and panic value checks has been received the wide acceptance and practical popularity as the means of reducing the various errors. Sysmex NE-8000 is a new breed automated blood cell analyzer, which performs simultaneously the complete blood cell count and white blood cell differential count, based on the principles of light scattering and impedance electronic counting method. In the departement of clinical hematology laboratory, the established data-input and management system between Sysmex NE-8000 hematology analyzer and main computer is accomplished by RS-232C port. As the serial test results of each patient accumulated and data management system enabled us to review the cummulated data. We tried in delta and panic value checks of each parameter in routine hematology test. In our laboratory, we succeeded in detecting and correcting preanalytical errors such as label mis-identification and clerical errors, so the performance of internal quality control was greatly enhanced. We analyzed Sysmex NE-8000 scattergram & histogram and delta & panic value check of CBC have been used of quality assurance from September 15th of 1993 to January 15th of 1994, in Dong-A University Hospital. The results are as follows; 1. Among 12,363 specimens evaluated, proportion of results out of delta & panic value was 6,752 cases(54.6 %). Among the 6,752 cases, ratio of delta value check was 4,313 cases (33.4 %) and ratio of panic value check was 2,621 cases(21.2 %). 2. The proportion out of ranged delta value was WBC 1,198 cases(9.7 %), Platelet l,056cases(8.5 %), Hemoglobin 273cases(2.2 %) and panic value was WBC 609cases (4.9 %), Platelet 722cases(5.8 %), Hemoglobin 162cases(1.3 %), respectively. 3. The chief causes of delta & panic analysis were transfusion of patient, inadequate samples, platelet aggregation and clotting of samples etc.

Japan`s Diagnosis Procedure Combination(DPC/PDPS) is a payment system unique to the country, and most characteristically Japanese in view of the DRG payment system throughout the world. While there is a ``bundling part`` having a per diem score diagnosis by diagnosis, the ``non-bundled (free-for-service) items`` are also widely accepted. The ``bundling part`` assigns a high score for a brief stay of hospitalization, thus promoting shortened ALOS. Moreover, ``hospital coefficients`` are prepared to compensate for any monetary loss due to the application of the DPC/PDPS and, on a long term basis, a wide range of incentives are provided to absorb and reflect various additional payments that are already applied under the current fee-for-service system. Though it is a voluntary participating system, many hospitals hope to join the program, meeting participation conditions such as submission of required data. The most outstanding achievement is its building of a database having plenty of clinical information from medical records and payment claims submitted by both DPC-applying and DPC-preparing hospitals to the Ministry. Japan`s DPC system is appraised as stably operative and progressive by succeeding in inducing consensus of interested parties in policy decision-making, and securing a high rate of participation of tertiary hospitals within a short period of time. This conveys a strong message to the Korea`s health insurance system that has failed to inch forward due to the medical providers` opposition in spite of having made efforts to introduce the DRG payment system for a long period of time.
수출 딸기 팔레타이징 적합 상자 및 파프리카 전용 수확 상자에 따른 선도유지 효과

딸기는 꾸준히 수출량이 증가하고 있으며 주로 홍콩, 싱가포르 등 동남아국가로 수출되고 있다. 딸기 수출 시 진주, 논산, 담양 등 국내 주요 산지별 외포장 상자 규격이 다르고, 팔레타이징에 부적합하여 장거리 운송 중 흔들림으로 인한 딸기 품질저하가 발생되는 문제점이 있다. 파프리카는 수출 시 저온장해, 꼭지부패, 착색불량 등 품질저하로 인한 클레임이 발생되고 있으며 특히 수확 및 예냉 시 주로 사용되는 상자는 파프리카 호흡특성에 적합하지 않아 개선이 필요한 상황이다. 이에 본 연구에서는 딸기 수출 시 팔레타이징에 적합한 포장상자를 표준화하고, 파프리카 호흡특성에 적합한 개공율을 개선한 상자를 개발하여 선도유지 효과를 알아보고자 하였다. 개선된 팔레타이징 적합 딸기 외포장 상자 규격은 L 520 × W 270 × H 75 mm로 다단 적재 시 상자의 미끄럼 방지 및 고정을 위한 틀을 추가하였고, 측면 공기 순환구를 통해 통기를 원활하게 하였다. 또한 파프리카 수확 상자는 개공율을 3~4% 수준으로 낮춰 수분 손실로 인한 증산작용을 억제하는데 효과적이었다. 딸기 팔레타이징 적합 상자 및 파프리카 수확 상자 적용으로 손실률을 절감하고, 수출 시 상품성이 향상되리라 기대한다.
