견봉-쇄골관절 탈구를 동반한 오구 돌기 골절의 봉합나사를 이용한 수술적 치료

목적: 본 논문은 오구 쇄골 인대가 온전하면서 견봉-쇄골 관절 탈구를 동반한 오구 돌기 골절에서 봉합나사를 이용한 고정술의 소개와 임상적 결과를 알아보고자 하였다. 대상 및 방법: 26세 남자 환자로 제3형 견봉-쇄골 탈구와 동반된 오구돌기 골절을 최초로 봉합 나사를 이용한 간접적 정복술을 시행하였다. 결과: 술 후 3개월에 골절의 완전한 유합 및 정상 견관절 운동 범위 및 기능의 회복을 얻었으며, 합병증의 소견은 없었다. 결론: 봉합나사를 이용한 간접적 정복 및 고정술은 견봉-쇄골 관절 탈구를 동반한 오구돌기 골절의 치료에 있어서 안전하고 유용한 수술 방법으로 사료된다. Purpose: This study examined the outcomes of closed reduction and fixation of the coracoid process fracture using a suture anchor in a patient with combined acromioclavicular dislocation for which the coraco-clavicular ligament was intact. Materials and Methods: A 26 year-old male patient with a coracoid process fracture that was associated with a type III acromioclavicular joint dislocation was operated on with anchor suture fixation. This is the first trial of this operative procedure. Results: At the postoperative follow-up at 3 months, complete union of the coracoid process fracture was seen. The shoulder had a full range of motion and the shoulder function was normal. Conclusion: Closed reduction and fixation using one suture anchor for treating coracoid process fracture is a safe, effective procedure.
회전근개 전층파열 환자의 등속성 외전 운동 시 각속도에 따른 견관절 주위근 활성도 변화

This study investigated the differential muscular firing patterns of the shoulder muscles in patients with full-thickness rotator cuff tear. Eleven patients with unilateral rotator cuff tear were recruited. During isokinetic exercises, the muscular activities of upper trapezius, serratus anterior and lateral deltoid were recorded by surface EMG. Subjects performed isokinetic shoulder abduction of 60°/sec, and 180°/sec. The mean EMG amplitude of root mean square (RMS) was calculated to a percentage of maximal activity for each muscle. In the rotator cuff tear sides, the muscular activities of lateral deltoid in abduction were increased in 180°/sec compared to 60°/sec. And the EMG activities of upper trapezius and serratus anterior in the lesion side were significantly decreased compared to sound side in abduction of 180°/sec. But, in the case of lateral deltoid, the EMG activities were not different between sound and lesion sides in abduction of 180°/sec.
