한국의 몸, 『황정내경경』의 새로 태어나는 나 『황정내경경』의 수행과 신체관

제천 감악산 백련사 극락전에는 나한도가 있다. 그 중에는 도교의 태식수행을 연상시키는 그림이 있다. 태식수행과 관련된 나한도는 중국의 절에서는 종종 발견되지만 한국에서는 감악산의 나한도가 유일하다. 본고에서는 태식수행의 원형을 갖추고 있는 『황정내경경』에서 이 그림의 가장 오래된 이야기를 찾아봤다. 『황정내경경』의 수행은 존사수행으로 알려져 있는데, 사실 축정을 통해 신성한 생명을 기르는 수행법도 병존한다. 본고에서는 존사수행을 고찰한 후, 이 수행법의 대강을 신체관과 결부지어서 검토했다. 여섯마리의 용으로 상징되는 하복부에 있는 생명력은 간의 힘을 타고 하늘로 올라갔다가 다시 내려와서 배꼽 뒤에 있는 황정에 저장된다는 것이 이 수행법의 얼개다. 그 과정은 지기와 천기의 결합이라고 해석할 수도 있다. 나한도의 개심견불도는 도교수행의 불교적수용을 보여주는 동시에, 순환을 통해 부처화한 즉, 부처와 결합한 불교구도자의 종교적체험을 상징한다.

우리 나라도 머지 않아 인구 고령화, 의료기술의 발달, 의료수요의 다양화, 의료보장범위의 확대 등으로 막대한 의료비의 증가 및 보건의료 부문의 확대가 예상된다. 따라서 거대하게 성장할 보건의료 부문의 낭비적 요소를 줄이기 위한 효율화 노력은 머지 않아 보건의료부문의 관심사가 아닌 중차대한 거시경제적 관심사기 될 것이다. 이러한 보건의료부문의 효율화 달성에 필수적인 요소가 바로 지식정보화라 할 수 있는데, 이의 걸림돌이 되는 각종 규제를 개선·정비하는 것은 지식정보화 사회구현을 위해 필수불가결한 요소이다. 본 논문은 아래에서 보건의료부문의 지식정보화를 위해 수행되어야 할 우선 과제를 제시하고 이에 대한 개선방안을 논하고자 한다.

Antifungal Activity and Properties of Crude Enzymes Produced from Cellulomonas fimi JH-1

In this study, bacteria were isolated from rosemary rhizosphere and chitinolytic activity was detected in 0.2% chitin agar medium. Using 16s rRNA sequencing, the isolated bacteria were identified as Cellulomonas fimi. Chitinase, cellulase, and protease activities were found to increase with incubation time. Chitosanase activity was highest on day 1, and gradually declined thereafter. Crude enzyme of C. fimi was prepared and separated by SDS-PAGE and the expression patterns of enzyme activity for chitinase and chitosanase isozymes was studied. Three chitinase isozymes (36, 55, and 66 kDa) and one chitosanase (39 kDa) were detected SDS-PAGE gel combinded with glycol chitin and glycol chitosan, and these chitinase and chitosanase had different molecular weights. Indole acetic acid (IAA) production by C. fimi was analyzed in nutrient broth (NB) media containing different concentrations of tryptophan. IAA production was found to increase with increasing tryptophan concentration. Siderophore production by C. fimi was analyzed in colloidal chitin medium for 0, 1, 3, 5, and 7 d. The production of siderophore increased for 5 d and gradually decreased thereafter. The antifungal activities of C. fimi against G. zeae, F. solani, B. cinerea, F. oxysporum, C. cucumerinum, R. solani, and C. acutatum were investigated in potato dextrose broth medium containing chitin. C. fimi showed the highest antifungal activity against G. zeae

다공성 폴리에틸렌 안와삽입물 노출 환자에서 4직근 전방이동술의 효과

Purpose: To investigate the effects of anterior displacement of four rectus muscles in exposure of porous polyethylene orbital implant (Medpor?). Case summary: This retrospective study reviewed 4 eyes with exposed orbital implant who underwent evisceration with porous polyethylene orbital implant (Medpor?). The technique involves dissection of conjunctiva and tenon`s capsule and isolation of four rectus muscles from sclera. Unsure if each muscle was sutured to each other or if a group of muscles were sutured to another group (i.e. superior and inferior to medial and lateral). Posterior tenon`s capsule was closed interruptedly and conjunctiva was closed continuously. Orbital implant exposure was occurred at 2~36 months later after evisceration and implant insertion. One eye was noted wound dehiscence 1 week after operation, then re-suture was done. Re-exposure was notified in 1 eye at postoperative 3 months, but it was small, then we just observed. In other 3 cases, exposure was not identified till last follow up. Conclusions: Four rectus muscle anterior displacement procedure may be simple and useful method in exposure of orbital implant.

의료정책의 정치경제학 : 건강보험 약가 정책을 중심으로 Korea's Drug Pricing Policy Reform

본 연구는 의료정책이 정치·행정·경제의 상호작용에 의해서 변동된다는 것을 보이기 위해 최근 전임 보건복지부장관의 이임 연설로 부각된 건강보험 약가 정책의 정책변동과정을 정치경제학적 시각에서 규명하였다. 이 사례연구를 통하여 과거 '정부 고시가 상환제도'에서는 경제적 요인과 행정적 요인이 상호 작용하면서 정치적 균형을 이루고 있었으나, 신 정부 출범 이후 정치적 요인과 경제적(외교적)요인으로 인해 균형이 파괴되었고 관료의 포획 등 행정적 요인이 주도적 역할을 하면서 '개별 실거래가 상환제도' 및 'A-7 가격 결정제도'의 보험약가 정책변동이 초래되었다는 사실을 확인할 수 있었다. 따라서 인구 고령화, 국민소득 증가, 소비자 주권의식 강화, 삶의 질 향사 욕구 증대로 계속 확대일로에 있는 의료정책 부문은 정치, 경제, 외교, 행정이 복잡하게 상호 작용하는 영역이고 주무부처인 보건복지부 조직과 관련 직능이익단체의 성격 및 상호관련성이 독특하므로 이 부문의 정책학 연구는 보다 정교한 접근이 필요하다는 정책학적 시사점을 도출할 수 있었다. The article examines Korea drug pricing policy reform. The reform was motivated by a desire to enhance the efficiency and transparency of the dung against the backdrop of lack of public support, resistance of various interest print reform agenda-Korea ended up with a drug pricing policy in favor of interest groups, at the cost of heavier general public and fiscal burden. Our study suggests that health policy involves complex decision making, heavily affected by political, economic, and even diplomatic relationships. Without, controlling these, it is difficult to achieve the policy goal of bettering the publics welfare.

Considering scarce records of dissection and the functional view of the body in East Asian Medicine, it is difficult but interesting to explore the motive for dissection in ancient times. The functional view of the body does not match well with dissection, but Huangdineijing that was compiled in the formative period of East Asian Medicine contains records probably related to dissection in two of its treatises. There are two hypotheses about the motive for dissection that are widely accepted; one is to prove the correspondence between the macrocosm and the microcosm and the other is to establish the standard model of body through measurements of organs. This article examined a novel hypothesis that the main motive for human body dissection was to establish the functional properties of the body upon objective anatomical data; anatomical measures were directly used to explain the circulation of qi and the digestive processes as a physiological metabolism. The purpose of anatomical dissection was to integrate physiology and anatomy as the obtained anatomical knowledge played a pivotal role in establishing the functional view of the body in East Asian Medicine.
