Takayasu’s arteritis is a chronic inflammatory arteriopathy affecting the aorta and its major branches, leading to stenosis or occlusion of the major arteries and causing ischemic symptoms. Nasal septal perforation is known to have various causes and to occur in patients with diverse rheumatic diseases such as Wegener’s granulomatosis, relapsing polychondritis, and systemic lupus erythematosus. However, septal perforation in Takayasu's arteritis is extremely rare. A 48-year-old woman without any other underlying disease presented nasal septal perforation (18×14 mm) associated with Takayasu’s arteritis. Septal perforation was treated conservatively, and Takayasu's arteritis was treated with immunosuppressive drugs, so septal perforation did not worsen and the patient did not experience any discomfort. Although septal perforation is extremely rare in Takayasu's arteritis, if other causes of perforation are excluded, Takayasu's arteritis also can be considered as a cause for septal perforation.
Skin Burn at the Oral Commissure Caused by Harmonic Scalpel during Sleep Apnea Surgery

In patients with obstructive sleep apnea who have not responded to continuous positive airway pressure, surgical intervention may be considered for areas where obstruction or stenosis has been confirmed through drug-induced sleep endoscopy. The harmonic scalpel, a device that operates at a frequency of 55,500 Hz to cut and coagulate tissue, can be used for procedures such as epiglottectomy and tonsillectomy during sleep apnea surgery. In the present case reported herein, the harmonic scalpel was carefully maneuvered without contacting the surrounding tissue, successfully completing the operation. Nevertheless, a second-degree burn at the oral commissure was postoperatively detected and promptly managed, resulting in resolution without scarring or complications. To the best of our knowledge, there have been no previous reports of burns caused by the harmonic scalpel during sleep apnea surgery. Therefore, we present this case report along with recommendations to mitigate such unanticipated incidents in the future.

Adenocarcinoma occurs very rarely in the paranasal sinuses; moreover, adenocarcinoma di-agnosed in the external nose is rarely reported. A 58-year-old male patient visited our hospitalwith an enlarged right nasal mass. On image studies, a 1.5 cm sized mass was found on theright nasolabial fold, which was surgically removed via sublabial approach. Intraoperative fro-zen section examination showed inflammatory tissue without tumor cells. However, adeno-carcinoma showing infiltration between the skeletal muscle layers was diagnosed on the finalhistopathologic examination after surgery. In the immunohistochemical staining test, CK7positive, CK20 negative, and GCDFP-15 positive were confirmed, so metastasis of adenocar-cinoma was considered first rather than primary cancer of the nasolabial fold. There was norecurrence and no tumor development in other sites after postoperative chemoradiation thera-py of 32 months.

Hemangioma is a benign tumor composed of blood vessels. It is classified into three types: cavernous, capillary, and mixed. Intraosseous hemangioma is a type of cavernous hemangioma that originates within the bone. It is very rare for it to originate from the inferior turbinate. Furthermore, we report a new case of recurrence after incomplete excision. The patient was a 60-year-old woman who had been experiencing left nasal obstruction for two years and recent nasal pain. She had previously undergone surgery for a tumor that occurred in her left inferior turbinate. During physical examination and imaging workup, hemangioma was suspected. The hemangioma was completely excised under the endoscopic approach, and complete excision is the only treatment that can prevent a recurrence. We report a recurrent inferior turbinate intraosseous hemangioma, provide a literature review, and compare the tendency with previous reports, including Korean cases.
Techniques of Ultrasound-Guided Ethanol Ablation for Thyroglossal Duct Cysts

Thyroglossal duct cyst (TGDC) is the most common congenital neck mass caused by rem-nants of the thyroglossal duct during fetal development and migration. Traditionally, theSistrunk procedure has been recommended as the standard surgical treatment for TGDC. How-ever, this procedure is associated with several surgical burdens such as the need for generalanesthesia, surgical scarring, and possible postoperative complications. Recently, ultrasound-guided ethanol ablation (US-EA) has been suggested as a good alternative for treating TGDCand has shown excellent treatment outcomes in terms of treatment efficacy and cosmetic satis-faction. Here, we introduce detailed techniques of US-EA for head and neck surgeons to useas a primary treatment of TGDC.
