1982년 5월 1일부터 1983년 7월 31일까지 고려병원 신경정신과 외래에서 사회공포증으로 진단받고 치료를 받았거나 현재 치료중인 환자 52명(남 : 34명, 여 : 18명) 을 대상으로 하여, 연령, 성, 학력, 가정환경, 증상 및 임상경과등을 분석하고 이를 외국의 경우와 비교, 고찰하였다. 1. 총 외래환자중 사회공포증 환자의 연인원 비율은 2.2%였다. 2. 남녀 비율은 남자에서 높았다(1.9:1). 3. 15세에서 29세 사이가 대부분이었고(88.5%), 내원당시 연령은 20대 후반이 제일 많았으나(44.2%), 여자는 20대 초반이 많았다(50%). 4. 학력 수준이 높은 편 이었다. 5. 장남 및 막내에서 많았다. 6. 증상이 변천하는 것이 특징이었다. 對人累張 → 表情恐怖 → 關係念廬 赤面恐怖 視線恐怖 7. 경계선형(Borderline type)은 다른 유형들과 많은 임상양상의 차이를 보여서 진단에 논의가 있어야 할 것으로 생각된다. 8. 내원당시 공포수는 평균 1.5개였고, 내원시까지 경한한 대인불안 증상수는 13.6개였으며 공포단계의 증상수는 2.3개였다. 9. 발병양상에 있어서 서서히 발병한 군이 33예로 제일 많았으며 이들은 다른 군에 비해 시회적응 정도도 좋았다. 10. 연령층이 낮을수록 사회적응 장애정도가 심했다. 11. 사회적응의 장애가 심한 경우가 12명으로 23%나 된 것은 주목할 사실이었다. The clinical picture of 52 social phobia cases were studied, who had visited Korea Genral Hospital, Seoul, Korea from May 1st, 1982 to July 31st, 1983. Results were as follows: 1. Social phobia patients comprised 2.2% of the total numbers of total psychiatric outpatient visits. 2. Male patients (34) outnumbered female (18) with ratio of 1.9:1 3. Majority of the subjects (88.5%) were between age range of 15 to 29;44.2% of the subjects were in their late 20s for their first visit to the clinic, and 50% of the women were in their early 20s. 4. Subjects tended to have higher education. 5. In male patients, first and last born were over-repersented in the population. 6. One of the characterisitic feature in its clinical course appeared to be that symptoms were changed in sequential fashion as follows: Interpersonal uneasiness Phobia for blushing ↓ Phobia for his own unnatural facial expression Phobia for his own and/or other's sight ↓ Idlea of reference 7. There was a group of subjects who showed somewhat differents clinical pictures from social phobia in general, thus termed as "Borderline Type". Howeer, further diagnostic evaluation is warranted for justification of this diagnostic standing. 8. Average number of phobia sysptoms at the time of clinic visit were 1.3,while the total number of phobia in the past were 2.3, and the total number of transient, interpersonal anxiety symptoms were 13.6. 9. As to the oneset of symptoms, those who developed with gradual oneset were most frequent (33cases), and this group showed the best adjustment in their social life. 10. The younger the subjects, the poorer were the social adjustment. 11. It was remarkable that 23% of the cases were totally incapacitated in their social function.
