        지지면에 따른 체중 이동 시 체간 근육의 근활성도에 미치는 영향

        Purpose: This study examined the muscle activity of the trunk muscles during weight shifting exercises on a stable and unstable surface. Methods: Ten healthy young subjects (4 males, 6 females) with no medical history of lower-extremity or lumbar spine disease were enrolled in this study. The muscle activity was recorded using surface electromyography (EMG) electrodes from the both sides of the external obliques and erector spinae muscles. Results: The EMG activities of the both rectus abdominalis, both external obliques and erector spinae muscles were significantly higher when the weight shifting exercise was performed on a firm surface than when a balance pad with a balance ball was used. In weight shifting exercises, the EMG activity of the contralateral external obliques and erector spinae muscles was significantly higher than that of the ipsilateral external obliques and erector spinae muscles under all three support surface conditions. Conclusion: Performing weight shifting exercise using an unstable surface is a useful method for facilitating the trunk-muscle strength and trunk stability.

        뇌졸중 환자에서 지팡이 종류에 따른 지팡이와 양발의 체중지지에 미치는 영향

        Purpose: The purpose of this study was to analyze the weight bearing of the cane and foot for the different walking aids during walking. Methods: A total of 12 subjects (6 males, 6 female) with stroke were enrolled in the study. Foot sensor and an instrumented cane were integrated to analyze the vertical peak force on the foot and cane. Results: The vertical peak force applied on the quad cane gait resulted in a significantly higher rate, which was $10.60{\pm}6.48%$ of the body weight, when compared to that of mono cane gait which was $7.91{\pm}4.11%$. The results indicated significantly lower vertical peak force on the affected foot, without the help of a walking aid, as compared to that of walking with a cane (respectively, p<0.05). However, results showed that the differences in vertical peak force on the affected foot, between mono cane and quad cane, were not significant. Conclusion: In conclusion, the vertical peak forces were significantly greater, during a comparison between walking with a quad cane and walking with a mono cane. On the contrary, no significant difference in the vertical peak force on the affected foot between walking with quad cane and walking with a mono cane. Muscle activation pattern and walking pattern should be measured in future studies, to study the differences between walking with various walking aids in the lower and higher functioning hemiparetic subjects, as its use may mask underlying gait impairment.

        테이핑과 병행한 스위스볼 운동이 임신성 요통 환자의 통증과 기능장애 및 삶의 질에 미치는 효과

        본 연구는 4주간 테이핑을 병행한 스위스 볼 운동이 임신성 요통환자의 통증과 기능장애 및 삶의 질에 미치는 효과를 규명하기 위하여 실시되었다. 단일맹검 및 사전 무작위 임상시험으로 진행되었으며, 16명의 임신성 요통 환자는 무작위로 테이핑과 스위스볼 운동군(n=8)과 테이핑과 신장 운동군(n=8)으로 배정되었다. 모든 대상자는 허리에 테이핑을 적용하였다. 추가적으로 테이핑과 스위스 볼 운동군은 스위스볼 운동을 하루 30분, 주 5회, 4주간 시행하였고 테이핑과 신장 운동군은 같은 시간동안 스트레칭 운동을 적용하였다. 통증은 시각사상척도(VAS)를 사용하여 측정하였고, 기능장애는 Roland Morris Disability Questionnaire (RMDQ), 삶의 질은 Short-Form 36items (SF-36)을 사용하였다. 훈련 후 테이핑과 스위스 볼 운동군이 테이핑과 신장 운동군에 비해 통증과 기능장애, 삶의 질이 유의하게 개선되었다(p<.05). 본 연구는 테이핑을 병행한 스위스 볼운동이 임산부의 요통과 기능장애 및 삶의 질을 효과적으로 향상시킴을 증명하였다. The purpose of this study was to determine whether swiss ball exercise combined with taping would improve low back pain, disability and quality of life in women with pregnancy-related Low Back Pain (PR-LBP). Sixteen patients with PR-LBP were recruited and were randomly divided into two groups: taping and swiss ball exercise group (n=8) and taping and stretching group (n=8). The taping and swiss ball exercise group was treated with swiss ball exercise with kinesio taping, while the taping and stretching group received only taping. The taping and swiss ball exercise group performed swiss ball exercise for 30 minutes a day, 5 times a week for 4weeks, while the taping and stretching group conducted stretching exercise for the same amount of time. VAS was used to assess pain level of low back. Disability was measured using a Roland Morris Disability Questionnaire(RMDQ). Quality of life were measured by SF-36. The pain intensity of low back in the taping and swiss ball exercise group improved significantly greater than the taping and stretching group (p<0.05). Significant improvement in the disability was observed in the taping and swiss ball exercise group compared to the taping and stretching group (p<0.05). The SF 36 in the taping and swiss ball exercise group improved significantly greater than the taping and stretching group (p<0.05). Our findings indicate that swiss ball exercise combined with taping is beneficial and effective to improve low back health and quality of life in women with PR-LBP.

        보행속도의 변화가 만성 뇌졸중 환자 지팡이의 수직력과 마비측 발의 체중지지에 미치는 영향

        Purpose The purpose of this study was to analyze weight bearing of cane and affected foot at different speeds during walking. Methods Thirteen subjects (6 males, 7 females) with stroke enrolled in the study. A foot sensor and an instrumented cane were integrated to analyze the vertical peak force on the affected foot and the cane. Results The applied vertical peak force on the cane were 12.02±4.80% (slow speed), 7.97±3.95% (comfortable speed), and 6.86±3.30% (fast speed) body weight, respectively. The results indicated significantly lower vertical peak force on the affected foot in the low speed walking condition when compared to the fast walking (p<.05) and the comfortable walking (p<.05) conditions. The correlations between TUG and vertical peak force on the cane and affected foot were .71, and -.70 (p<.01). There was a higher correlation between the vertical peak force on the cane and affected foot were -.87(p<.01). Conclusion In conclusion, slower walking speed applied greater vertical peak force on the cane, On the contray, slower walking speed applied less vertical peak force on the affected foot. Further studies, duration of force should be measured at different speeds during walking in lower and higher functioning hemiparetic subjects, as its use may mask underlying gait impairment.

        산전⋅산후 물리치료에 대한 가임기 여성의 인식조사연구

        Despite the growing evidence of the importance of prenatal and postnatal physical therapy, it is still not widely popularized in domestic settings in Korea. This study was conducted to examine the awareness, knowledge, and attitudes of fertile women who are the target population of prenatal and postnatal physical therapy services. We analyzed the respondent data from 70 fertile women aged 20 to 60, who had experienced pregnancy and childbirth, using a structured questionnaire. The questionnaire consisted of a total of 17 items, including general characteristics (4 items), awareness (5 items), knowledge (6 items), and attitudes (6 items) towards prenatal and postnatal physical therapy. In this study, frequency analysis was used to present the results for all items. Additionally, Fisher’s exact test were conducted to examine the significance of the association between age group and the awareness towards prenatal and postnatal physical therapy. The results revealed that the overall awareness level of prenatal and postnatal physical therapy among women in Korea was less than 30%, indicating a low level of awareness. Furthermore, significant differences in the awareness owards prenatal and postnatal physical therapy were found based on age group and parity. Additionally, it was found that over 70% of the respondents positively evaluated the role of physical therapy in prenatal and postnatal care. The findings of this study confirmed the awareness of prenatal and postnatal physical therapy among women in Korea, and it is expected to serve as foundational data for promoting prenatal and postnatal physical therapy in the country.

        과제지향훈련 및 부가적인 TENS 중재가 만성 뇌졸중 환자의 상지 근활성, 운동기능 및 경직에 미치는 효과

        본 연구는 만성뇌졸중 환자에게 과제지향훈련 및 부가적인 TENS(transcutaneous electrical nerve stimulation) 중재가 상지의 근활성, 운동기능 그리고 경직에 미치는 효과를 규명하기 위하여 실시되었다. 단일맹검 및 사전 무작위 임상시험으로 진행되었으며, 24명의 만성 뇌줄중 환자는 무작위로 실험군(n=12)과 대조군(n=12)로 배정되었다. 실험군은 과제지향훈련을 4주간 주5회, 총 20회 시행하였고, 과제수행 전 30분간 TENS자극을 마비측 팔에 적용받았다. 대조군은 동일한 과제지향훈련을 수행하였고, 속임 TENS자극을 받았다. 근활성은 표면 근전도를 이용하였고, 상지의 운동기능을 측정하기 위하여 Fugl-Meyer 상지 검사를 이용하였다. 또한 경직을 측정하기 위하여 Modified Ashworth Scale(MAS)를 사용하였다. 중재 후 실험군과 대조군 모두에서 근활성이 유의하게 증가하였고, 상지의 운동기능이 유의하게 향상되었다. 또한 경직도 유의한 개선을 보였다. 실험군은 대조군에 비하여 긴노쪽손목폄근과 운동기능 그리고 팔꿉 굽힘근의 경직을 유의하게 개선하였다. 본 연구는 과제지향훈련은 만성 뇌졸중 환자의 근활성 증가, 운동기능개선 그리고 경직 중재에 효과적이며, 부가적인 TENS 중재는 이를 더욱 효과적으로 향상시킴을 증명하였다. This study investigated the efficacy of task-related training (TRT) combined with Transcutaneous Electrical Nerve Stimulation (TENS) on the improvement of upper limb muscle activation, motor function and spasticity of upper extremity in chronic stroke patients. This study designed as a pilot randomized clinical trial study. 24 inpatients with chronic stroke were recruited and randomly assigned into two group: the experimental group (n=12) and the control group (n=12). Subjects in the experimental group received high-frequency TENS for 30 minutes on wrist and elbow extensors, while the control group received placebo-TENS stimulation that was not real electrical stimulation. All participants in both groups conducted task-relate training for 30 minutes after TENS application five days a week for four weeks (20 sessions). The primary outcomes of upper limb muscle activation were measured by surface electromyography (EMG), and Fugl-Meyer upper extremity scale (FMUE) was used to assess motor function of upper extremity. Modified ashworth scale (MAS) was used to identify spasticity. The measurements were performed before and after the 4 weeks intervention period. Both groups showed significant improvements in muscle activation, motor function, and spasticity after intervention. Especially, the experimental group shows a significant differences in muscle activation of extensor carpi radialis longus, FMUE, and spasticity on elbow flexor compared to the control group. Our findings indicate that TRT is effective to improve muscle activity, motor function and spasticity in paretic upper limb of chronic stroke patients, and additional TENS application makes this more efficient.

        경통 환자들을 위한 한국어판 Northwick Park Neck Pain Questionnaire의 신뢰도와 타당도

        The purpose of this study was to establish reliability and validity of the Northwick Park Neck Pain Questionnaire (NPQ) translated into Korean for neck pain patients. Sixty-two subjects (35 males, 27 females) with neck pain enrolled in the study. They completed a standardized self-administered questionnaire that included pain intensity, sleeping, sensory at night, duration of symptoms, carrying, reading and watching television, working, social activities, and driving. Reliability was determined by intraclass correlation coefficient (ICC) and Cronbach's alpha by internal consistency. Validity was examined by correlating the NPQ scores to the Visual Analog Scale (VAS) score. Test-retest reliability of the translated versions of the NPQ was good ICC(2,1)=.83 (95%CI=.85-.95). Cronbach's alpha value for NPQ was found to be .87 and this was statistically significant (p<.05). The criterion-related validity coefficients was .75 (p<'Ol). We concluded that the Korean version NPQ was shown to be a reliable and valid instrument for the assessment of neck pain.
