        Brown-Sequard 증후군과 Horner씨 증후군으로 발현된 동정맥 기형 파열로 인한 경추 경막외 혈종 - 증례보고 -

        We report a rare case of Brown-Sequard syndrome associated with Horner's syndrome in cervical epidural hematoma caused by a ruptured arteriovenous malformation. A 54-year-old man developed sudden sharp neck pain, radiating to the interscapular area. Within hours, left side hemiplegia and decreased tactile sense and loss of contralateral pain sense ensued. Emergency cervical magnetic resonance image showed an epidural hematoma over the cervico-thoracic junction. The hematoma was located in the left posterolateral area of the cervical spinal canal. Emergent decompressive laminectomy and an evacuation of the hematoma were performed. A tangled soft tissue mass found in the hematoma was proven to be an arteriovenous malformation. To the authors, knowledge, this might be the first case of a Brown-Sequard syndrome associated with Horner's syndrome caused by ruptured cervical epidural arteriovenous malformation.

        자기공명 촬영상 요추 추간반 병변의 판독자내 및 판독자간 해석의 다양성

        Objective : The terminology of degenerative disc disease lacks official standardization. Lacks of such standardization may provoke some clinical and litigation problems. The authors investigated interobserver and intraobserver variability in interpretation of lumbar disc abnormality. Methods : Magnetic resonance imaging studies of the lumbar spine performed prospectively in 50 patients, were read blindly by three doctors dealing spinal disorders, using two nomenclature. Nomenclature I was normal, bulging, protrusion, extrusion. Nomenclature II was normal, bulging, herniation without neural compression, with neural compression. Intraobserver and interobserver variation were measured statistically. Results : Interobserver agreement was 70.4-80.8% for nomenclature I, 76.2-80.2% for nomenclature II. Intraobserver agreement was 84.0-88.0% for nomenclature I, 79.2-86.8% for nomenclature II. Interobserver Kappa statistic was 0.53-0.56 for nomenclature I, 0.54-0.57 for nomenclature II. Intraobserver Kappa statistic was 0.60-0.85 for nomenclature I, 0.53-0.72 for nomenclature II. Conclusion : Experienced doctors showed only moderate interobserver agreement when interpreting disc status on lumbar magnetic resonance imaging. Intraobserver agreement was superior to interbserver. The standardization of nomenclatures for lumbar disc extension beyond interspace are needed.

        Rupture of the rectus femoris at the musculotendinous junction is very rarely reported in the literature, especially which is caused by chronic repetitive stimulation. A 21 year old soldier complained of progressing right thigh pain and mass for 8 weeks after military training. MRI of the right thigh showed inhomogenous mass lesion suspecting soft tissue tumor. However ultrasonographic examination showed torn retracted rectus femoris tendon at musculotendinous junction. One year after primary repair of ruptured tendon, the patient is free from symptoms and returned to full previous activities. Knowledge of this sonographic and MRI finding will help the surgeon to diagnose and treat this rare injury.

        역도(力道)선수에서 견관절 질환

        Purpose: Weight lifting is a good training to control body weight, to correct body shape and to relieve stress. How-ever if the training is continued by inadequate training method and technique, the risks of the shoulder injuries are relatively high. Main Subject: The rotator cuff injury is the most common disorder to wright lifters and often results from the train- ing program of upright row, military press and pectoral deck. The chances of subacromial impingement in these postures are high because the shoulder rotates under the acromion at 90 abduction state. Shoulder instability in weight lifters can develop due to various causes. aepeated microtrauma and excessive abduction and external rotation may result in laxity of the anterior capsular structure, ligament and muscles. Behind the neck and bench press are high risk training postures. Other than those injuries, idiopathic osteolysis of distal clavicle, acromioclavicular separation, pectoralis major muscle rupture, and triceps muscle rupture nay develop. Conclusion: The best treatment option of the shoulder injury to weight lifters is to eliminate the possible risk elements for the weight lifters in training program and to provide proper and prompt treatment as soon as possible.

        수학활동에서의 초등 수학영재학생들의 손 조작 능력

        Recent mathematical education in Korea is changing very rapidly. The pursuit of such changes in mathematics education can be summarized as “How can a better understanding and interest in mathematics, can be effectively developed the mathematical competence of students.” There are many opinions on this as a methodology. But one thing is important is that mathematics lessons is much more fun and should include a variety of math activities. Therefore, I think math activities in the future of mathematics lessons are increasing and their importances will grow increasingly. The purpose of this paper is to evaluate whether there is a significant difference between the regular class 9 students from Incheon J elementary school and the gifted class 10 students from Incheon J elementary school, and between the gifted class 10 students from Incheon J elementary school and mathematically gifted 9 students from Institute of Science Education for the Gifted and Talented. We used a test paper developed by Jeon In Ho(2015) and test data were processed by t-test or ANOVA according to statistical matters or in Excel 2013. As results, for the three categories in mathematics activities, there is no differences between the regular class group and the gifted class group. On the other hand, in the category to use a compass, there is a difference of hand manipulation abilities. Specifically, there is no differences between the regular class group and the gifted class group but these two groups make differences in hand manipulation ability with the gifted group from Institute of Science Education for the Gifted and Talented.

        This main purpose of this study analyzed the difference and the cause and effect between customer satisfaction and revisit intention using the customer types by market segmentation of wine restaurant' store image. To accomplish this purpose, the sample selected 15 wine restaurants in the south and north of the Han River. And the questionnaire was distributed 260 to the customers. The main findings are as follows : the customer types by market segmentation verified the most positively recognizing group, the positively recognizing group, the fairly recognizing group, the negatively recognizing group, the most negatively recognizing group. Store image reinforcement of wine restaurant elevates customer's satisfaction, and is judged that can keep the high competition position.

        우리나라 초등학교 수학 교과용 도서에 나타난 공간 감각 개념에 대한 고찰

        In this study, we examined the definition and concept of spatial sense described in the national elementary mathematics teacher's guide according to the 2015 revised curriculum and 10 elementary mathematics teacher's guides by certified author. Based on the research results, the following conclusions were reached. First, most of the national and certified elementary mathematics teacher's guides define spatial sense as “an intuitive feeling about the situation and objects around oneself,” and describe spatial sense with the same meaning as spatial perception, spatial visualization, and spatial ability. However, it needs to be revised from a cognitive psychological perspective, and as an alternative, it is proposed to use the term spatial sense as spatial ability. Second, since most of the national and approved elementary mathematics teacher's guides agree that the sub-abilities of spatial sense are comprised of spatial visualization and spatial orientation, it is proposed to define the sub-abilities of spatial sense as spatial visualization and spatial orientation.

        주관절 주위 외상후 합병증

        목적: 주관절은 인체에서 가장 안정적인 관절 중 하나지만, 견관절 다음으로 탈구와 골절이 흔히 발생한다. 이러한 손상은 발생 당시 기전과 환자의 나이 그리고 충격에 따라 다양하게 나타나는데 적절한 치료에도 불구하고 여러 가지 합병증들이 발생 가능하다. 이에 저자들은 외상후 합병증을 고찰하고자 한다. 대상 및 방법: 저자들은 일반적인 주관절 탈구의 합병증인 관절 운동제한, 불안정성, 그리고 관절 주위 이소성 골형성의 원인과 치료 등에 대해 기술하였다. 결과 및 결론: 주관절 주위 골절 이후 발생 가능한 합병증 중 부정유합, 불유합, 외상성 관절염 등에 대한 그 기전과 치료에 대해 기술하고자 한다. Purpose: The elbow joint is one of the most stable joints. Dislocation and fracture can occur in elbow joint most commonly next to shoulder joint. Various injuries can occur according to generated mechanism, age of patient and impact. Despite proper treatment, various complications can occur. Materials and Methods: We describe etiology and treatment of these complications after elbow trauma such as stiffness, instability and heterotopic ossification. Results and Conclusion: Malunion, nonunion and traumatic arthritis are addressed as a possible complication after fracture around elbow joint.

        미술치료를 위한 컴퓨터 심리진단 장치

        본 논문에서는 미술치료 이론을 기반으로 하는 아동심리 진단용 컴퓨터 장치를 제안하고 그에 대한 연구 결과를 소개하고자 한다. 제안된 시스템에서는 진단 대상 아동이 그린 그림으로부터 색상과 관련된 요소뿐만 아니라 사용된 선의 특성, 필압(pen pressure), 그림의 위치, 크기 등의 다양한 정보를 컴퓨터로 구현된 장치를 이용하여 측정하며, 측정 후 분석된 결과를 기반으로 치료사에게 치료 대상자에 대한 보다 많은 유용한 정보를 제공하여 진단의 신뢰성을 높이고자 한다. 미술치료는 근본적으로 진단 대상자의 그림을 평가하는 치료사의 주관이 개입될 수 있다는 단점이 존재한다. 대상자의 그림으로부터 어떤 정보를 어떻게 추출할 것인지부터 추출된 정보로부터 대상자의 심리상태에 대한 해석에 이르기까지 전적으로 치료사가 어떤 이론을 적용하는지에 따라 달라지며, 치료사의 학문적 깊이에 따라서도 달라지게 된다. 따라서 본 논문에서는 여러 정보 중에 ‘무엇’을 치료 대상자의 그림에서 얻어낼 수 있는지와 ‘어떻게’ 읽어낼 것인지를 알아보고, 치료 대상자의 심리 상태를 객관적으로 판단하기 위한 기준을 마련하는데 유용한 시스템을 제공하고자 한다. 이를 위해 컴퓨터를 기반으로 한 심리진단용 시스템을 제안하며, 대상자가 제안된 장치에 그리게 되는 그림을 통해 주제 색, 선의 특성, 필압, 그림의 위치 등의 의미 있는 데이터를 추출해 내며 이를 수치화함으로써, 미술심리치료 분석에 새로운 가능성을 제시하고자 한다. In this paper, we propose a computerized psychological test system for art therapy. The proposed system with the computerized devices can measure various information, such as colors, characters of lines, pen pressure, size of objects, position of objects, and so on, from the drawn pictures by the child. The measured information can be analyzed to extract more meaningful information about the child and to provide more effective remedial information for art therapy. Art therapy essentially has a potential drawback that the results of test can be influenced by therapist’s subjectivity. The results can be different from the method of gathering information, the method of analysis, the interpretation of analyzed data, and even the therapist’s academic backgrounds. Therefore, in this paper, we focus on what information needs to be measured and how to measure the data from drawn pictures and propose a computerized psychological test system for art therapy. The proposed system can measure and analyze various information from the drawn picture by the child. Since the proposed system can provide valuable psychological information of child in the form of quantitative data, it can be exploited as a new method for art therapy.
