A numerical method has been proposed to predict 3-dimensional flow in a duct system with multiple outlets. For the duct system, it is supposed that the pressure values are given at multiple outlets while the velocity profile is given at a inlet. To maintain the continuity of pressure distribution between main and branch duct, present method allows that the pressure value taken from analysis of branch duct can be converted to the main duct analysis. The result from present method which can handle the pressure boundary condition closely coincided with that from regular method which can handle the velocity boundary condition only. Furthermore the flow distribution from present method showed good agreement with that from the single block method. From the comparison of the present method with the total pressure method used for engineering duct design, 13% of discrepancy in pressure loss was shown between the main duct inlet and the branch duct outlet.

Marine sources as potential treatment options for various diseases have been a subject of growing interest. However, information on the anti-inflammatory mechanism employed by Undaria pinnatifida (UP) remains limited. The present study was conducted to investigate the mechanisms of UP on the mast cell-mediated inflammatory response. To determine the pharmacological mechanism of UP in inflammatory reaction, we evaluated the effects of UP on interleukin (IL)-6, IL-8 and tumor necrosis factor (TNF)-α production and nuclear factor-κB (NF-κB) and caspase-1 activation in calcium ionophore A23187 plus phorbol 12-myristate 13-acetate-stimulated human mast cells-1 (HMC-1). The results showed that UP suppressed IL-6, IL-8 and TNF-α production in a dose-dependent manner. Moreover, UP significantly attenuated NF-kB/caspase-1 activation in stimulated HMC-1. Collectively, these findings provide experimental evidence that UP may be a useful candidate for the inflammation-related diseases treatment.

The group formation problem of the machine and part is a critical issue in the planning stage of cellular manufacturing systems. The machine-part grouping with alternative process plans means to form machine-part groupings in which a part may be processed not only by a specific process but by many alternative processes. For this problem, this study presents an algorithm based on self organizing neural networks, so called SOM (Self Organizing feature Map). The SOM, a special type of neural networks is an intelligent tool for grouping machines and parts in group formation problem of the machine and part. SOM can learn from complex, multi-dimensional data and transform them into visually decipherable clusters. In the proposed algorithm, output layer in SOM network had been set as one-dimensional structure and the number of output node has been set sufficiently large in order to spread out the input vectors in the order of similarity. In the first stage of the proposed algorithm, SOM has been applied twice to form an initial machine-process group. In the second stage, grouping efficacy is considered to transform the initial machine-process group into a final machine-process group and a final machine-part group. The proposed algorithm was tested on well-known machine-part grouping problems with alternative process plans. The results of this computational study demonstrate the superiority of the proposed algorithm. The proposed algorithm can be easily applied to the group formation problem compared to other meta-heuristic based algorithms. In addition, it can be used to solve large-scale group formation problems.

Self Organizing Map (SOM) is a neural network that is effective in classifying patterns that form the feature map by extracting characteristics of the input data. In this study, we propose an algorithm to determine the cell formation and the machine layout within the cell for the cell formation problem with operation sequence using the SOM. In the proposed algorithm, the output layer of the SOM is a one-dimensional structure, and the SOM is applied to the parts and the machine in two steps. The initial cell is formed when the formed clusters is grouped largely by the utilization of the machine within the cell. At this stage, machine cell are formed. The next step is to create a flow matrix of the all machine that calculates the frequency of consecutive forward movement for the machine. The machine layout order in each machine cell is determined based on this flow matrix so that the machine operation sequence is most reflected. The final step is to optimize the overall machine and parts to increase machine layout efficiency. As a result, the final cell is formed and the machine layout within the cell is determined. The proposed algorithm was tested on well-known cell formation problems with operation sequence shown in previous papers. The proposed algorithm has better performance than the other algorithms.

Hepatitis A virus (HAV) infection is common in developing countries, including Korea. It can be accompanied by extrahepatic complications such as renal failure, arthritis, and vasculitis. Pleural effusion is a very rare complication of HAV infection, which has been reported usually in children, and has benign clinical courses. Here we report a case of pleural effusion with ascites which occurred in an adult hepatitis A patient. A 26-year-old-woman presented generalized myalgia and fever and was diagnosed as acute hepatitis A. Despite of the improvement of laboratory findings, fever and cough persisted. Pleural effusion newly appeared on the serial chest radiologic images. After the fever settled down, the pleural effusion resolved spontaneously at 13th day of admission. Stenotrophomonas maltophilia is increasingly being recognized as an important cause of infection on immunodeficient patients. But S. maltophilia peritonitis in continuous ambulatory peritoneal dialysis (CAPD) are not common. Here, we report the case of a 40-year-old male with HIV infection and end-stage renal disease treated with CAPD. The patient had frequent previous history of peritonitis and known HIV infection. He presented with chilling, abdominal pain, and cloudy peritoneal effluent. White blood cell and neutrophil count were elevated at the initial peritoneal fluid analysis, so we diagnosed him as having CAPD-associated peritonitis. On the seventh hospital day, the organism isolated from the effluent was S. maltophilia. We changed the antibiotics to trimethoprim-sulfamethoxazole (TMP-SMX). But it was happened severe nausea, vomiting due to TMP-SMX complication. Thus, TMP-SMX was changed to colistin. after 8 days, He recovered completely, with no need for catheter removal. No recurrence was observed.

저자드은 전신 무력감과 양측 다리의 근무력감을 주소로 내원한 80세 여자 환자에서 감초에 의한 의인성 외견상 염류 코르티코이드 과다증후군(apparent mineralocorticoid excess syndrome)을 진단 하였고 환자의 임상경과를 기존의 문헌 고찰과 함께 보고하는 바이다. Prolonged ingestion of licorice can hypermineralocorticoidism with sodium retension and potassium loss, edema, increased blood pressure and depression of rennin-angiotensin-aldosterone system. We describe a 80-year old woman with muscle weakness with hypokalemia(serum K+: 1.6 mEq/L) and rhabdomyolysis(urine myoglobin> 2000 ng/mL) due to licorice tea. After quitting the licorice and potassium replacement therapy, hypokalemia and muscle weakness gradually improved within 5 days.

It is well known that the reactivation of hepatitis B virus (HBV) may occur as an acute hepatitis after chemotherapy or immunosuppressive therapy. Although most of these cases have been reported in HBsAg-positive patients, there have been a few reports of HBV reactivation in HBsAg-negative patients. There have been concerns for the need to screen the reactivation as well as anti-viral prophylaxis in HBsAg-negative patients with possible HBV occult infection who are planning to undergo chemotherapy or immunosuppressive therapy. Rituximab, an anti-CD20 monoclonal antibody, is effective in the treatment of non-Hodgkin`s lymphoma. However, rituximab can affect the immunity against HBV, consequently increasing viral replication. In fact, there have been reports of HBV reactivation after treatment with rituximab. Here, we report a case of HBV reactivation following rituximab plus systemic chemotherapy in diffuse large B cell lymphoma patient who was HBsAg negative, anti-HBs positive, and anti-HBc positive, ultimately leading to treatment-unresponsive fulminant hepatic failure.
