        아세클로페낙과 그 대사체의 약물동태 연구 및 생물학적 동등성

        Aceclofenac, a nonsteroidal antiinflammatory agent of a phenylacetic acid type, has been used for rheumatoid arthritis and osteoarthrits. Although the metabolic pathway of aceclofenc is relatively well-known in vitro, pharmacokinetic profiles of its three major or metabolites are still unclear in human. The present study was designed to investigate pharmacokinetic profiles of the metabolites of aceclofenac, and to evaluate the bioequivalence of the generic preparation of aceclofenac 100 mg tablet. Blood samples were serially collected for a period of 12 hours following a single oral administration of 100 mg aceclofenac in 20 healthy human volunteers. A simple protein precipitation with acetonitrile was employed to purify those substances from plasma. Aceclofenac, diclofenac, 4'-hydroxyaceclofenac and 4'-hydroxy-diclofenac in heparinized plasma were simultaneously measured with flufenamic acid, an internal standard, using HPLC coupled to a tandem mass spectrometer. Time courses of 4'-hydroxydiclofenac, diclofenac and aceclofenac plasma concentrations were clearly revealed, and the pharmacokinetic properties were analyzed. The 90% confidence intervals for the ratios of test/reference for log-transformed AUC and lie within 0.80-1.25.

        Viscosity Measurement of Whole Blood with Parallel Plate Rheometers

        There are many factors that affect the increase or decrease of viscosity of the whole blood. Because of this, measuring the viscosity of whole blood is difficult to correlate with specific etiologies, so it tends to disregard the information on blood viscosity. This study is to find out the characteristics of the method of measuring the viscosity of whole blood using the parallel plate method, which is officially registered as a medical device with the approval of the Korea Food and Drug Administration (KFDA), and to apply it to the measurement of whole blood viscosity. The accuracy of the parallel plate measurement is verified by comparing with the measurement results using the commercially available scanning capillary method. In addition, statistically meaningful results are obtained between the viscosity and some components of the whole blood. It is expected that the research will be carried out to understand the correlation between the viscosity of whole blood and specific components of blood and to identify the prognostic signs of etiology in certain diseases, especially in cardiovascular and cerebral vessels diseases.

        혈액원 임상화학 검사의 정도관리

        The main goal of transfusion medicine is safe and appropriate blood transfusion in all situations. To accomplish this, it is essential to have a high level quality management system for the entire process from blood donation to transfusion. Regulations regarding blood management have been adopted and strictly managed in Korea since 2007. Blood center’s blood management tasks should establish appropriate quality management systems to ensure the safe supply of blood, as well as the basic resources of personnel, facilities and equipment in accordance with laws and regulations governed by the Ministry of Health and Welfare in Korea. The purpose of this review is to examine the contents and processes for quality control of clinical chemistry tests in Korean blood centers.

        전신 자가면역질환에서 Line Immunoassay를 이용한 항 ENA항체 및 항 dsDNA항체의 검출

        Background : Detection of antibodies to extractable nuclear antigens (ENAs) and dsDNA is needed for the diagnosis of and predicting prognosis in systemic autoimmune diseases. Recently introduced line immunoassay (LIA) has the advantage of detecting several autoantibodies simultaneously, and we evaluated its usefulness in the diagnosis of autoimmune diseases in comparison with enzyme-linked immunosorbent assay (ELISA). Methods : Samples were collected from 437 patients referred by rheumatologists. FANA (fluorescent antinuclear antibody) test and LIA for the detection of 13 different autoantibodies, including 6 ENAs and dsDNA were performed. LIA-positive samples for ENA or dsDNA antibodies were further tested with ELISA. Final diagnosis was made by rheumatologists according to the diagnostic criteria. Agreement of results between LIA and ELISA was analyzed in 53 selected patients with systemic autoimmune diseases. Results : The LIA detected antibodies to ENA and dsDNA in 118 and 22 patients, respectively, and ELISA detected 70.3% (83/118) and 45.5% (10/22) of LIA positive samples. Especially, 60.2% (71/118) of patients with positive ENA antibody on LIA was diagnosed as systemic autoimmune diseases. Patients having strong FANA titer and homogenous/speckled pattern showed higher prevalence of autoantibodies, but a small proportion of FANA negative patients also showed positive reactivity (LIA 10.8%, ELISA 5.2%). LIA showed a good agreement with ELISA for the anti-ENA antibodies (≥80%), and a lower agreement for the anti-dsDNA antibody (67.9%). Conclusions : LIA detecting several autoantibodies simultaneously might replace ELISA for anti-ENA antibodies, but not for anti-dsDNA antibodies. When LIA is performed considering clinical manifestations and FANA, it could contribute to the diagnosis of systemic autoimmune disease. (Korean J Lab Med 2008;28:353-61)

      • 뇌종양세포의 세포유전학적 고찰

        Cytogenetic results and genetic alterations are closely related with tumorigenesis, progression and promotion of cancers. With the deepening of basic biologic understanding of neoplastic mechanisms, the clinical usefulness of various cytogenetic abnormalities as diagnostic and prognostic aids has been appreciated. Ten cases of brain tumors(2 Glioblastoma multiforme, 3 meningioma, 1 ependymoma, 1 gliosarcoma, 1 astrocytoma, and 1 metastatic tumor) were studied and 8 cases were possible for karyotyping . Four patients showed abnormal karyotypes. Structural and numerical abnormalities were detected. Karyotyping of 2 Glioblastoma multiforme showed trisomy 7, loss of chromosome 10 and loss of chromosome 8. Structural anomalies were detected on chromosome 1q, and 7. Many many marker chromosomes were also detected. One gliosarcoma showed many numerical anomalies, such as +7, +8, +17, and +mar. Meningioma cells studied were normal daryotyping. Although the cases were too small to correlate with the tumorigenesis, but histologically poor grade type of tumors showed very complex karyotypes.

      • 임상화학 검사의 Delta Check System 개발 및 정도관리 효율성 검토

        Background : The application of the delta check in tests in general chemistry detects both random and clerical errors, thus enhancing the reliability of tests. However, since this approach adds the burden of confirming the results to the laboratory, it has the disadvantage of lengthening the turn around time of tests. We speculated that an online delta check system needed to reduce the turn around time. We developed delta quality control system and evaluated its effectiveness of quality control in clinical chemistry. Methods : We developed an online delta check system based on the client-server paradigm. We used IBM PentiumⅢ PC as server and as clients. The database system used was powerbuilder 6.0. Results : In the system we developed, delta screening was performed when test results were input to the computer. The decision over the delta screening was made by comparing it against patient's clinical information and cumulative results within the same screen. Conclusions : The developed delta check method made it available to reduce the turn around time previously spent on delta screening by eliminating the batch processing of tests which was needed in previous approaches separate query cumulative results and patient's clinical informations for screening purposes.

        류마티스 관절염 환자에서 ANti-Cyclic Citrullinated Peptide Antibodies검사의 진단적 유용성

        배경 : 류마티스 관절염에 유용한 혈청학적 검사로 류마티스인자(RF)가 있으나 민감도 및 특이도면에서 보다 우수한 새로운혈청학적 검사방법이 필요하게 되었다. 최근 synthetic cycliccitrullinated peptide를 이용한 ELISA 방법(anti-CCP)이 개발되어 류마티스 관절염 환자에서 anti-CCP 검사의 진단적 유용성을 평가하고자 하였다.대상 및 방법 : 2002년 1월부터 5월까지 류마티스 내과에 내원한 류마티스 관절염 환자 324명과 대조환자군 251명, 건강대조군286명을 대상으로 RF와 citrulline을포함하는 cyclic peptide를기질로 사용하는 anti-CCP 검사를 ELISA 방법으로 시행하였다. ROC curve를 통해 곡선하면적이 가장 큰 값에서 얻은 분별류마티스 관절염 환자에서 Anti-Cyclic Citrullinated Peptide Antibodies검사의 진단적 유용성 137한계치를 사용해 진단적 민감도 및 특이도를 분석하였다.결과 : Anti-CCP 검사의 분별한계치는 3.8 U/mL이었고 민감도 및 특이도는 72.8%, 92% 이었으며, RF의 분별한계치는 9U/mL이었고 민감도 및 특이도는 78.7%, 80.9%로 나타났다. 그러나 anti-CCP와 RF 검사의 분별한계치를 각각 8.4 U/mL과20 U/mL로 할 경우 민감도 및 특이도는 각각 67%, 95.2%와63.3%, 90%로 나타났다. Anti-CCP와 RF를 같이 사용할 경우민감도 및 특이도는 79.3%, 96.4%를 보였다. RF 음성 류마티스관절염 환자에서 anti-CCP 양성률은 63명 중 15명으로 23.8%이었다.
