A Case of Late Recurrent Vasospasm After Sirolimus-Eluting Stent Implantation

A 67-year-old male with stable angina was admitted to our cardiovascular center. He had neither any history of smoking, diabetes mellitus, hypertension, cerebrovascular accident nor family history of coronary artery disease. Coronary angiography showed a 90% tubular eccentric luminal narrowing at the mid left anterior descending artery (m-LAD). A sirolimus-eluting stent (SES) was implanted in the m-LAD. Coronary angiography performed after 9 months did not reveal restenosis or recurrent coronary artery disease. However, the patient returned to the emergency room with severe chest pain after 17 months. Coronary angiography showed severe diffuse vasospasm distal to the m-LAD stent site. After 20 days, vasospastic myocardial infarction developed. A zotarolimus-eluting stent with a phosphorylcholine polymer was implanted distal to the m-LAD stent. The zotarolimus-eluting stent was used because the polymer in the SES or sirolimus was considered a possible cause for the recurrent vasospasm. The patient had no further chest pain during the 9 months after zotarolimus-eluting stent implantation. We suspect that the polymer in the SES or sirolimus might have caused endothelial dysfunction and provoked the late vasospasm. The hydraulic equipments use the hydraulic pressure by the control of valve. Most of hydraulic equipments are controlled to be activated in accordance with the proper pressure. In the case of the safety valve, when it is applied over the specific pressure number, the valve is drained to secure the safety of the system. These safety valves are activated according to the current pressure. In this study, prior to the analysis of the main control valve, the computational fluid dynamics are performed for the system in which the valve is opened or closed depending on the using pressure. The using pressure is randomly set and programmed in the scheme language. From the results of simulation, we can see that the boundary condition is changed at the using pressure. Using this simulation technique, we can automatically change the boundary condition when the pressure reaches the specific value. So, we suggested a solution of the problem of the override characteristics.

The many parts in the hydraulic equipments subject to load by hydraulic pressure. And some parts which are installed to the inside of the hydraulic equipments subject to load by the hydraulic pressure as well as the fluid speed. The inside of the valve generally generates the flow. Considering the valve expansion or the flow of the large valve, the flow is similar to the fluctuation flow in the opened flow system. In this study, the analysis of the flow including the fluctuation are performed and this results will apply to the optimal shape design of the flow path. The load acting on the structure was analysed according to the change in the shape and flow speed of the structure which install in fluid, and the safety evaluation wad investigated in consideration of the flow speed and the depth of fluid.
Angioplasty for Difficult Complex Lesions With Using the VentureTM Catheter for Wire Placement

We report here on the use of the VentureTM catheter to facilitate sucesful advancement of a guidewire acros dificult, complex lesions after the prior atempts at guide wire passage were unsucessful with using standard wires. This VentureTM catheter may increase the suces rate and reduce the procedural time for such a chalenging in-terventional procedure.

타이어 트레드 및 사이드 특성 변경에 따른 노면소음에 관한 실험적 연구

The bending exercises of radial tire is one of the most important structural properties of the sidewall relating to ride and road noise of automobiles. The frequency band analysis is more useful for analyzing tire road noise due to property change of tread and sidewall. In this paper, the vertical stiffness and lateral stiffness of tire which have a various tread and sidewall is measured and the road noise is measured about same road condition. Furthermore, we investigated the effect on the structure of the tire tread and sidewall for the sound pressure level.
