        제2형 당뇨병 환자의 혈당조절 영향요인 분석: 제7기 국민건강영양조사자료(2016∼2018) 이용

        Purpose: This study examined factors influencing glycemic control among type 2 diabetes mellitus (T2DM) patients using data from the seventh Korea National Health and Nutrition Examination Survey. Methods: The subjects were 1,459 patients aged 30 years and older who with T2DM. The subjects were divided into three groups according to glycated hemoglobin A (HbA1c) values: good glycemic control group (good GCG, HbA1c<7%), insufficient GCG (7≤HbA1c< 8%), and poor GCG (8%≥HbA1c). Univariate multiple multinomial logistic regression was used. Results: The patients were distributed as follows: 52.7%, good GCG; 28.4%, insufficient GCG; and 18.9%, poor GCG. In the insufficient GCG, DM duration was a significant factor influencing glycemic control in T2DM. In the poor GCG, age, DM duration, hypertriglyceridemia, waist circumference and muscular exercise were significant factors influencing glycemic control in T2DM. Conclusion: Strategies for insufficient glycemic control should include intensive glucose control interventions for patients with a DM duration over 5 years. Strategies for poor glycemic control should include intensive glucose control interventions for younger age groups, those with a DM duration >1 year, and those with hypertriglyceridemia. In addition, nursing interventions for weight loss and muscular exercise are needed.

        서울시 주거지의 근린환경특성이 개인의 신체활동과 건강수준에 미치는 영향 분석

        This study examines the relationships between the neighborhood environment, physical activity, and health status of the 176 neighborhood units of ‘Dong’ in Seoul using the ‘National Health and Nutrition Examination Survey’ (2007-2012). The multilevel analysis results indicate that demographic and socioeconomic variables such as a respondent’s age, health condition, education, marital status, and income are very significant variables to explain physical activity and health status. Some neighborhood environment variables such as floor area of commercial use or office use, neighborhood park, and neighborhood school facilities also show significant relationships with low-intensity physical activity time of walking. However, most neighborhood environment variables do not show a significant association with moderate- or high-intensity physical activity time except a few variables. The relationship between built environment and health status indicates that while neighborhood environment variables have no association with quality of life (EQ-VAS), the floor area of multi-family housing, neighborhood park and children’s park shows some associations with overall health status (EQ-5D). In contrast, the floor area of multi-family housing, land use mix, and the length of bike roads are significant variables to explain BMI. Thus, this study shows mixed results on the relationship between the neighborhood environment and physical activity and health status.

        보행자 연령대별 보행만족도에 영향을 미치는 가로환경의 특성분석

        This study examines the impact of street environment on pedestrian walking satisfaction for different age groups using the 2012 Pedestrian Satisfaction Survey Data(PSSD) in Seoul, Korea. This study confirms that multilevel ordered logistic model was an appropriate method to control for both personal-level and location-level differences on walking satisfaction which has an ordered unit. The results indicate that street level physical environments have consistent or different impacts on pedestrian satisfaction by age groups. In particular, pedestrian specialization street has statistically significant and positive association with pedestrian satisfaction regardless of different age groups. In addition, it shows stronger impact on walking satisfaction for the aged population. We also found that some physical environmental variables including sidewalk width were important variables for walking satisfaction. However, street-level land use mix variable did not show any statistical significance to the level of pedestrian satisfaction. This study concludes that pedestrian street design strategies should consider specific determinants of pedestrian satisfaction for different age groups.

      • 신체활동 친화적인 건강증진 계단 디자인을 위한 사전방향성 연구

        현대인은 자동차 중심적인 생활과 신체활동 부족으로 비만과 이로 인한 각종 만성질병의 위험에 노출되어 있다. 따라서 일상생활 속에서 지속적이며 자발적인 신체활동을 유도할 수 있는 공공 공간의 디자인이 중요하다. 지하철 및 공공 공간에 비치된 계단은 신체활동뿐만 아니라 건강증진과 공중보건 제고에 효과적이다. 본 연구는 건강증진 계단디자인을 위한 사전연구로서 국내외 유사사례를 분석하고 그 결과를 통해, 공공 공간 계단디자인의 요인별 고려요소를 탐색하였다. 이러한 요소는 신체활동 친화적인 건강증진 계단 조성을 위한 디자인 선호도 및 관찰 분석의 근거로 활용되었다. 분석결과 건강계단 디자인에 있어 시각적 자극을 유도하는 색채, 그래픽, 조명과 청각적 요소인 음악 등에 대해 선호도가 높은 것으로 나타났다. 또한, 생활에서 걷는다는 의미의 중요성을 인지할 수 있는 칼로리 표기나 걷기를 통해 연장되는 수명표기 등의 정보가 계단이용을 유도하는 데 도움을 주는 것으로 나타났다. 그리고 계단이용이 기부금 적립 등으로 연결되어 자긍심과 보람을 느낄 수 있는 사회 공헌적 적립 모듈 등과 연계될 때 계단이용을 더 선호하는 것으로 나타났다. 본 연구는 신체활동과 건강증진을 유도할 수 있는 건강계단 디자인 요소들을 분석하였으며, 향후 우리나라에서 공공 공간을 대상으로 건강증진을 위한 액티브 디자인 운동의 기초 자료로 활용될 수 있을 것이다. People in contemporary societies have been increasingly exposed to various diseases that are attributed to physical inactivity and an automobile-dependent lifestyle, such as obesity and chronic diseases. Therefore, it is important to design public spaces that encourage sustainable and spontaneous physical activity and promote health in everyday life. Stairs in public spaces, such as within subway system and in public buildings, has great potentials not only to promote physical activity, but also to enhance public health. This study consists of preliminary research on stair design that can promote healthy lifestyles. It analyzes existing cases and explores specific elements of stair design that can promote public health. These specific design elements identified in this study have been included in a preference survey and examined in observational research in order to identify important design factors that can promote physical activity and health. Analysis results show that both visual stimulation such as graphics color, and lighting and music are effective elements that can encourage people to use stairs rather than escalator. In addition, inspirational and motivational signs addressing calorie expenditures and life longevity are positive design elements. Lastly, social activities, such as exercises that accumulate points for donation through stair use have the potential to motivate people to use stairs. By analyzing the stair design elements that promote physical activity and health, this study provides basic information for implementing active design movement in public space in Korea.
