Multiple endocrine neoplasia type 1 (MEN1) syndrome is a relatively rare disease, characterized by the occurrence of multiple endocrine tumors in the parathyroid and pituitary glands as well as the pancreas. Here, we report a case of MEN1 with neuroendocrine tumors (NETs) in the stomach, duodenum, and pancreas. A 53-year-old man visited our hospital to manage gastric NET. Five years prior to his visit, he had undergone surgery for incidental meningioma. His brother had pancreatic nodules and a history of surgery for adrenal adenoma. His brother`s daughter also had pancreatic nodules, but had not undergone surgery. The lesion was treated by endoscopic submucosal dissection and diagnosed as a grade 1 NET. Another small NET was detected in the second duodenal portion, resected by endoscopic submucosal dissection, which was also diagnosed as a grade 1 NET. During evaluation, three nodules were detected in the pancreas, and no evidence of pituitary, parathyroid tumors, or metastasis was observed. After surgery, the pancreatic lesions were diagnosed as NETs, with the same immunohistochemical patterns as those of the stomach and duodenum. Genetic testing was performed, and a heterozygous mutation was detected in the MEN1 gene, which is located on 11q13. (Korean J Gastroenterol 2017;69:181-186)
한글 철자 발달검사(Korean Developmental Spelling Assessment: KDSA)의 타당도 검증

This study was designed to investigate the validity of a newly developed Korean developmental spelling assessment (KDSA). KDSA is a standardized test for first and second graders. Five elementary school teachers evaluated the content validity of KDSA and the construct validity was investigated with 1,049 first and second graders using confirmatory factor analysis (CFA). Finally, the concurrent validity was assessed by comparing the KDSA score with the scores of KISE-BAAT and KOLRA spelling tests. The content validity scores were satisfactory, proving that the words and sentences are appropriate to test young students` spelling abilities. Results of model fit indices (e.g., chi-square, GFI, RMR, CFI, RMSEA) obtained from CFA showed that the construction of KDSA us solid. The relatively high correlation of the KDSA score with the KISE-BAAT and KOLRA spelling tests revealed that the concurrent validity of KDSA is appropriate. Overall, the results of the study showed that KDSA is a valid assessment to test first and second grade students`spelling abilities.
치주건강에 대한 주관적 인식과 현존영구치수와의 관련성에 관한 연구

Purpose: periodontal health index is a dentist-centered index in which dentists perform examination and evaluation. Therefore, it is not appropriate for evaluation subjective symptoms and satisfaction levels of patients. To compensate for this shortcoming, we made use of a subjective index of periodontal health status with which patients can evaluate their oral health status. This research evaluated the usefulness of Periodontal age index in examining subjective consciousness of periodontal health status, estimated the number of existing permanent teeth, which is the objective index for oral health, and compared the relationships between them. Materials and Methods: The number of subjects was 2,198, consisting of 1,115 (50.7%) men and 1.083 (49.7%) women. The periodontal age increased in the 18-24 and 40-44 age groups. There were no significant differences between the 40-44 and 55-59 age groups. Further, periodontal age decreased in those aged over 60-64. Results: In this study, periodontal age index increased to age 40 when teeth are lost to periodontal disease and decreased with a decline in the number of permanent teeth. Conclusion: Therefore, the periodontal age index reflects the periodontal state rather than hypofunction by tooth loss.

Venous air embolism (VAE) was reported in up to 23% of patients after contrastenhanced computed tomography (CT) examination; most of the emboli are small and not associated with the symptoms. In cases of small and moderate-sized emboli, most patients recovered with supportive care, including intravascular fluid infusion to increase venous pressure and venous return, oxygen therapy, and patient positioning. However, with larger emboli, VAE can be a serious and even fatal event. There have been few cases of VAE in Korea, in which patients improved with supportive treatment. We report a successfully treated case of VAE in a patient with end-stage renal disease (ESRD) on regular hemodialysis, in whom large air emboli developed after contrast-enhanced CT scan, and who experienced improvement with supportive care including cardiopulmonary resuscitation.
예제 기반 챗봇을 위한 기계 학습 기반의 발화 간 유사도 측정 방법

예제 기반 챗봇은 사용자 발화와 가장 유사한 예제 발화를 대화 예제 데이터베이스로부터 검색하여 응답을 생성한다. 가장 유사한 발화를 찾는 것은 응답의 적절성과 직결되는 것임에도 불구하고, 유사 발화 검색을 위해 어 떠한 자질을 사용할 것인지, 어떠한 방식이 좋은 지에 대한 기존 연구는 부족하였다. 본 연구에서는 검색의 정확도 와 예제의 활용도를 높이기 위해 다양한 어휘적, 의미적 자질을 이용한 기계 학습 방법을 제안한다. 실험 결과 1) 대화 예제 데이터베이스의 활용도 2) 예제 발화의 매칭의 정확률 3) 답변의 질적인 측면에서 제안하는 방법은 기존 의 방법에 비해 더 나은 성능을 보였다. Example-based chatBot generates a response to user’s utterance by searching the most similar utterance in a collection of dialogue examples. Though finding an appropriate example is very important as it is closely related to a response quality, few studies have reported regarding what features should be considered and how to use the features for similar utterance searching. In this paper, we propose a machine learning framework which uses various linguistic features. Experimental results show that simultaneously using both semantic features and lexical features significantly improves the performance, compared to conventional approaches, in terms of 1) the utilization of example database, 2) precision of example matching, and 3) the quality of responses.
파닉스 기반 철자교수가 작문능력에 미치는 영향 -학습장애 학생들을 대상으로-

본 연구에서는 파닉스 기반 철자교수가 학습장애 학생의 작문능력에 미치는 영향을 알아보고자 학습장애 학생을 대상으로 간헐 중다기초선설계를 이용한 단일대상연구를 실시하였다. 중재 결과 직접적으로 교수 받은 철자에서는 모든 참여자가 향상된 수행능력을 보였으며, 작문 유창성에도 향상된 결과가 있는 것으로 나타났다. 이는 효율적인 텍스트 생성을 위해서는 자동화된 전사기술(글씨쓰기와 철자)이 필수적이라는 작문의 작업기억 용량이론을 뒷받침하는 결과이다. 반면, 작문의 질이나 문법적 복잡성에서는 중재의 효과를 보이지 않았다. 이와 같은 결과는 작문의 인지과정 중 계획하기, 수정하기와 밀접한 관련이 있는 작문의 질이나 문법적 복잡성은 철자교수 외에 별도의 작문교수를 필요로 한다는 것을 반증한다. 본 연구의 결과는 학습장애 아동과 같이 초기학습자들에게 작문을 교수하기 위해서는 계획하기, 수정하기와 같은 상위 인지처리 기술을 가르치기 전에 효과적인 철자교수를 제공하는 것이 필수적임을 제안한다. This study reports the instructional effects of a phonics based spelling intervention conducted on four students with LD. Based on the previous study(Berninger et al., 2002), It was hypothesized that the phonics based spelling intervention would affect students`` writing skills. The multiple baseline single subject design with intermittence multiple baseline was applied. The results showed that LD students who received the phonics based spelling intervention not only increased their abilities in spelling, but also their writing fluency. No such effect was found on the writing quality and syntactic complexity in writing. The results of the current study support the capacity theory of writing discovering that the text generation requires a certain level of automaticity in spelling. The results also revealed that writing quality and syntactic complexity, which are the products of cognitive processes of planning and revising, were not improved as a result of the current intervention. These results suggest that for the beginning learners such as students with LD teachers need to provide effective spelling instruction before they teach higher-level cognitive processing skills such as planning and revision.
특수교육법 개정을 위한 학습장애 관련 법령 분석과 제안

본 연구는 개정을 앞둔「장애인 등에 대한 특수교육법」의 학습장애 조항들이 어떠한 방향으로 개선되어야 할지 제안하기 위하여 학습장애와 관련된 최근의 여러 법령 및 법안들을 분석하고, 더불어 해외 특수교육 법들을 분석함으로써 그 방향성의 근거를 들었다. 먼저 특수교육 패러다임이 세계적으로 변화하고 있는 추세에서 국내 특수교육도 장애로 판별된 학생들을 위해서만 존재하는 법령이 아니라, 일반교실에 배치되어 있으나 특별한 교육지원을 필요로 하는 학생들에게 필요한 지원까지 아우르는 법령이어야 함을 제안하였다. 이어서 학습장애의 개념과 진단과정, 그리고 선별 조항들과 배치 및 교사양성에 대한 관계법령을 분석하고 개정안을 제안하였다. 현재 학습장애를 가지고 있거나 학습장애에 준하는 학업의 어려움을 경험하고 있는 학생들은 적합한 공교육 지원을 받지 못하고 교육의 사각지대에 쉽게 방치되고 있다. 개정 특수교육법의 학습장애 조항들이 실제적으로 학습장애 또는 그에 준하는 학업적 어려움을 가지고 있지만 일반교실에서 지원을 받지 못하고 있는 다수의 학생들에게 교육적 요구에 적합한 지원을 보장해 줄 수 있는 법적 근거가 될 수 있기를 기대하며 특수교육에 대한 인식변화의 필요성도 함께 논하였다. The revision of the special education law is currently in process. The authors of this paper claimed that the current special education law has failed to support students with learning disabilities who are learned in the general education classrooms and who are not identified as having learning disabilities. It was asserted that the new special education law needs to be based squarely on the students’ special education needs, not on their disability itself. The current special education law along with the related educational laws and policies on supporting students with learning disabilities were analyzed together to explain recent movements in the Korean legal and administrative system to support students with learning difficulties and disabilities. Changes in the special education laws in other countries were also analyzed in support of the arguments. The authors found that emphasis needs to be placed more on “support” rather than “disability itself” through reflections on the definition, screening and identification procedures, as well as in teacher education and placements.
