만삭 분만에서 임신 주수와 제대동맥혈 pH의 임상적 고찰

목적: 정상 만삭 분만 후 제대동맥혈 pH를 측정하여 임신주수와 pH의 상관관계를 조사하며 산혈증의 발생빈도 및 연관성을 분석하고자 한다. 방법: 2004년 1월부터 2006년 12월 사이에, 만삭 분만 중 산모측에 내과적 질환이나 임신관련 합병증이 없으며 분만진통 중 태아가사 상태를 보이지 않았던 1,918예에서 측정된 제대동맥혈 가스 분석결과 중 제대동맥혈 pH를 임신주수, 분만 방법에 따라서 분석하였다. 산혈증에 대하여 제대동맥혈 pH 7.10 미만인 경우와 임신주수별 평균값에서 2표준편차 미만인 경우 두 가지로 정의하여 임신주수에 따른 발생빈도 및 신생아 결과와 비교하였다. 결과: 37주 이후 만삭 분만에서 임신주수가 증가함에 제대동맥혈 pH 수치가 유의하게 감소함을 볼 수 있었다(p=0.000). 또한 분만 방법에 따라서 질식 분만 군에서 임신주수에 따른 제대동맥혈의 pH 수치는 유의하게 감소하였으나(p=0.001) 제왕절개군에서는 차이를 보이지 않았다. 산혈증 발생빈도에 있어서, 임신주수별 평균값에서 2표준편차 미만으로 산혈증을 정의한 경우 임신주수에 따른 산혈증 발생빈도는 유의한 차이를 보이지 않았다. 두가지 정의에 의한 산혈증 군에서는 신생아 5분 아프가 점수가 7미만과 grade II 이상의 태변착색이 모두 유의하게 증가하였다. 결론: 만삭 분만 산모에서 분만 시 임신주수가 증가함에 따라서 제대동맥혈 pH는 통계적으로 유의하게 감소하였지만 임신주수 보정에 따른 기준에 의하면 산혈증의 빈도는 증가하지 않았다. Objective: To assess the gestational age adjusted values of umbilical cord artery blood pH in term pregnancy Methods: We retrospectively analyzed umbilical arterial blood pH values and obstetric data of 1,918 term singleton deliveries without maternal medical disease, pregnancy induced hypertension and fetal distress during labor periods at Kangnam St. Mary`s Hospital, Catholic University medical college between January 2004 and December 2006. Low pH was defined as less than 7.10 as a stationary cutoff value, whereas pH less than (mean -2 standard deviations [SDs]) was used when gestational age was considered. We evaluated the risk of 5 minute Apgar score of less than 7 and meconium staining more than grade II at low pH. Results: A significant negative correlation was found between gestational age and umbilical artery pH (p=0.000). We evaluated the umbilical artery pH value in different delivery mode. Vaginal delivery cases showed negative correlation (p=0.000), but elective cesarean section cases didnt. Low pH patients defined by both criteria of pH less than 7.10 and pH less than (mean -2SD) showed significantly increased risk of 5 minutes Apgar score of less than 7 (pH less than 7.10: p=0.000, pH less than (mean -2SD): p=0.001) and meconium staining more than grade II (pH less than 7.10: p=0.001, pH less than (mean -2SD): p=0.001). Conclusion: Our results suggest that umbilical cord pH has a negative relation with gestational age during term period.

Objectives : This study was conducted to evaluate the efficacy of a complex mixture of natural substances of ginseng and baeknyeoncho on the arthritic rats. Methods : In vitro experiments were conducted to ensure the stability of the complex. After setting toxicity and concentration by MTT assay, the antioxidant effect was measured through DPPH and ABTS radical scavenging activity. To confirm the anti-inflammatory effects of the complex, levels of nitric oxide (NO) and pro-inflammatory cytokines (IL-1β, TNF-α) were measured in LPS-treated macrophage cell lines (RAW264.7). We injected monosodium iodoacetate (MIA) 50 μl (60 mg/ml) into knee joints of rats to induce osteoarthritis. The rats were divided into three groups (normal (n=5), control (n=5), and OR (n=5) group). The control group consumed 2 mg/kg of physiological saline once a day for 4 weeks, and the OR group was mixed at a concentration of 416.5 mg/kg of Baengnyeoncho (O) and 208.25 mg/kg of red ginseng (R) and ingested 1 mL each 5 days a week. Results : This complex increased the DPPH and ABTS radical scavenging rate. The complex decreased NO production and pro-inflammatory cytokine production of macrophages. In the OR group, the secretion of cytokine in serum was decreased. In histopathological examination, the joint tissue of the composite showed less damage to the synovial membrane, cartilage, and fibrous tissue than the control group. Conclusions : As a result of this study, natural complexes have antioxidant, anti-inflammatory and cartilage protection effects. Therefore, we expect the complex to be effective in treating osteoarthritis.

Objective : This study was designed to compare acupuncture sensation index among three different acupuncture devices. Methods : A, B and C acupuncture devices were inserted into Quchi(LI 11) of the subjects. After 5 minutes the subjects completed a questionnaire rating the intensity of 21 kinds of acupuncture sensation; hurting, penetrating, sharp, aching, intense, spreading, radiating, tingling, pricking, stinging, pulling, heavy, dull, numb, electric, shocking, hot, burning, cool, pulsing, and throbbing. We compared subjective evaluations of acupuncture sensation among the groups. Results : The acupuncture sensation index when administrated among three different acupuncture devices(A, B and C) was not significantly different. The acupuncture sensation of B acupuncture devices showed significant difference than A or C in Spreading, Dull, Numb item. But, the other items of acupuncture sensation were not significantly different among three acupuncture devices. Conclusions : We found that acupuncture sensation index when administrated among three different acupuncture devices. Further study is needed to know different acupuncture sensation among acupuncture devices.
