A sixteen years old male patient was treated with the fractionated stereotactic radiosurgery(FSR) using Novalis system. The patient had surgery initially at other hospital before FSR and suffered from visual and endocrine dysfunctions which was required hormone replacement. Seven months after FRS, we found marked decrease of tumor volume. Now he is followed for 1 year. There is a still controversy regarding the ideal management of craniopharyngiomas. Radical resection is hazardous and can produce unacceptable mortality and morbidity. Many authors noted that postoperative conventional radiotherapy or Gamma knife radiosurgery after limited excision achieved better results than total excision of tumor for survival and postoperative complications. FRS delivered precise radiation to a target volume in close proximity to surrounding critical structures resulted in tumor regression. FRS using the relocatable mask frame may have a good role for adjuvant treatment of recurred or relapsed craniopharyngiomas.

Meningioma associated with intratumoral hemorrhage is rarely reported. We present two patients with intratumoral hemorrhage. One 70-year-old man was admitted to our department with a decreased level of consciousness and left hemiparesis. CT scan and MRI scan revealed huge tumor with intratumoral hemorrhage in the frontal lobe. After surgical removal of the tumor, histopathological diagnosis was a meningotheliomatous meningioma. The other seemed patients was 56-year-old woman with headache and vomiting. She showed no specific neurological deficit. CT and MRI scan revealed large size tumor with intratumoral hemorrhage that looks like meningioma. The patient died suddenly before surgery. We reviewed the relevant literature and discussed the possible mechanism of hemorrhage in the meningioma.

Hemorrhage remote from the surgical site after transsphenoidal approach is uncommon. We report a rare case of epidural hematoma occurrence following pituitary tumor surgery via transsphenoidal approach. During the operation, the arachnoid membrane seemed not to be disturbed but clear fluid like cerebrospinal fluid(CSF) was suspected to be being leaked. Lumbar drain was inserted immediately after surgery. Four hours after surgery, the patient’s consciousness fell to drowsiness and exhibited right hemiparesis. Brain CT scan revealed large amount of hematoma in the epidural space of left parietal area. Emergency craniotomy was performed and hematoma was removed. Over-drainage of CSF through the lumbar drain seems to be major cause for this postoperative complication.

Vestibular schwannoma (VS) is a benign tumor which tends to grow slowly. In general, the growth rate of VS is about 1mm/year. We treated an unusual case of rapid growing VS associated with multiple cystic changes. Also, we review the natural history and factors influencing tumor growth of VS. A 75-year old female patient presented with gait disturbance and hearing loss. Brain magnetic resonance imaging (MRI) showed a mass of 4.0×4.0×3.5cm size with multiple cystic changes and heterogeneous enhancement in right cerebellopontine angle (CPA). Twelve six months ago, the patient had a brain MRI which revealed CPA mass less than 1.0cm in diameter. Small sized VSs should be followed up with MRI at least every two years and the VSs consisted with cystic component need to be more closed observation than the solid type.

Meningioma is the most common benign primary intracranial tumor and rarely metastasizes. Malignant meningioma is a World Health Organization Grade III tumor and constitutes 1 to 3% of all meningioma. The prognosis of the patient of malignant meningioma is very poor because of its frequent recurrence and distant metastasis. There is no guideline concerning the management of extracranial metastasis of meningioma. A 57-old female patient presented with headache and visual disturbance due to huge skull base meningioma. She underwent subtotal mass excision with craniotomy and TSA followed by radiation therapy. However, follow-up brain magnetic resonance imaging (MRI) revealed tumor growing and the patient received additional surgical resection. After 2nd surgery the patient was presented with weakness of both lower extremities. Whole spine MRI demonstrated thoracic spine metastasis compressed spinal cord. Multiple nodules of lung and liver were noticed as well. We report a patient of malignant meningioma with multiple extracranial metastases including review of literature.

Hemangioblastomas are solid or cystic benign vascular tumors that may arise anywhere in the body. Solid hemangioblastomas can be difficult to treat surgically because of their hypervascularity and reqirement for circumferential dissection. Recently, We experienced a huge solitary solid hemangioblastoma in the cerebellopontine angle. The tumor was safely resected totally. We report the case with review of literatures.

A giant pituitary adenoma remains a major challenge for neurosurgeons, despite advances in en-doscopic endonasal surgery. These tumors can grow in different directions, which can affect the surrounding neurovascular structures. When the tumor grows towards the suprasellar region, it can affect important structures such as the optic nerves, anterior cerebral arteries, and hypothalamus. When it occupies the interpeduncular and prepontine cisterns, the oculomotor nerves or posterior communicating arteries may be at risk. Postoperative hemorrhage due to incomplete resection is also a concern and can lead to the need for revisional surgery or another surgical approach. Patients with giant pituitary adenomas typically have longer hospital stays due to the high rates of postoperative complications, such as cerebrospinal fluid leakage, diabetes insipidus, and infection. Gross total re-section rates are also lower for these types of tumors. This study provides a comprehensive review of the literature to date and reports various clinical outcomes associated with giant pituitary adenomas.

A 72-year-old woman was presented with progressive visual loss and headache for two months. Her visual acuity of right side was no perception of light. Brain MRI showed bulging enhancing mass in right orbit apex extending to cavernous sinus and sinusitis of sphenoid and ethmoid sinus. The patient was suffered from severe eyeball pain and headache not controlled by analgesics. An open biopsy via the extradural cavernous sinus approach was performed. Pus discharge was observed from the cavernous sinus and orbital apex under the anterior clinoid process. Histologic examination demonstrated hyphae. Systemic antifungal treatment such as Amphotericin-B and voriconalzole relived the pain but visual loss of right eye was not improved. We have to consider the fungal infection such aspergillosis as a differential diagnosis of a mass in the cavernous sinus.

We report a rare case of intracranial cystic esthesioneuroblastoma that was originated from the cribriform plate. A 65-year-old female presented with intermittent memory loss and headache a few months ago. On MRI, the tumor looked like meningioma that is located in anterior cranial fossa. So, she underwent an craniotomy and gross total removal was performed. The histopathology of tumor was confirmed as esthesioneuroblastoma. We reviewed intracranial esthesioneuroblastoma with a relevant literatures.
