Variations in Stroke Patients’ Muscle Activity during Head Rotation in Non-Paretic-Side Weight Bearing

        이관섭,최한성,이병주 대한물리치료학회 2015 대한물리치료학회지 Vol.27 No.3

Purpose: This study aimed to determine the interaction among the neck, trunk, and lower extremities on the non-paretic side in head rotation along with non-paretic-side weight shifting of stroke patients. To compare stroke patients’ ability to control posture through muscle activity variation related to pertubation during head rotation along with the non-paretic limb. Methods: We tested 15 hemiplegic patients and 15 normal individuals. Each group’s muscle activity was measured by electromyography in neutral head position and head rotation position. We compared each group’s resu lt based on measured values in patients’ non-paretic neck muscles, trunk muscles, and lower limbs muscles activation. Results: The study showed that muscle activity increased in the sternocleidomastoid muscle (102.26%, 53.00%), splenius capitis muscle (97.93%, 54.93%), erector spinae muscle (241.00%, 127.60%), external oblique abdominal muscle (256.66%, 152.00%), and internal oblique abdominal muscle (252.80%, 152.6%), peroneus longus muscle (117.53%, 137.13%) and gastrocnemius muscle (119.06%, 137.20%), while the results for the sternocleidomastoid muscle, splenius capitis muscle, erector spinae muscle, external oblique abdominal muscle, internal oblique abdominal muscle, peroneus longus muscle, and gastrocnemius muscle showed a statistically significant difference (p<0.05). Conclusion: It is hard for stroke patients to engage in normal movement control under suggested conditions because of the insufficient movement against gravity on the stroke patient’s non-paretic side and impaired cooperative patterns. To solve these problems, patients need their bodies to improve through effective movement, resulting in advanced control of their effective and functional activity.

      • 效率的인 幼兒音樂指導에 關 한 硏究 : 歌唱中心으로

李寬燮 배화여자대학 1989 培花論叢 Vol.8 No.-

With the economic growth and enhancement of people's consciousness in Korea children's attendance rate of the kindergarten has dramatically increased. Accordingly programs of the kindergarten have been developed variously. Of other programs, music programs are the most important in their role as well as influence on children's emotional development. But the survey on the present situations of kindergarten music teachers shows that teachers' musical ability and tastes dictate children's emotional development. The problems found from the survey are as follows 1. Most kindergartens are equipped with musical instruments but they are not fully utilized or sometimes merely displayed. 2. Many teachers generally don't understand musical talents of each child and they lack techniques in teaching music. These problems are mainly derived from teachers inability to utilze musical instruments and childen's musical talents as well. Now I'd like to suggest a few techniques to teach music effectively, hoping for more enthusiastic and dynamic movement to enhance children's musical ability in future. 1. To understand music, teachers should appreciate classical music through FM broadcast or records or at concerts so that they can form a habit of singing enjoying music. 2. They should practice playing the piano, which is indispensible for music education at kindergartens. Unless they are good at it, they'll lose confidence in music. 3. They should teach music in various ways : unison, duet, troll, rhythmic song, appreciation of music, concert, etc. 4. They should select music very appropriately according to season and children's taste. 5. They should develop children's ability to appreciate music creatively and to build up rhythmical sense. Children have various musical tastes which can be channeled into creative musical minds with vivacious rhythms only when teachers awaken them. Teachers should be aware of these situations and exert themselves to teach children music more effectively. Then the children's world will be filled with emotional euphoria, which in turn will make society joyous and bright.

뇌졸중 환자의 머리회전 각도가 내.외측 균형에 미치는 영향

        이관섭,김중선,Lee, Kwan-Sub,Kim, Chung-Sun 대한물리치료학회 2012 대한물리치료학회지 Vol.24 No.5

Purpose: This study was conducted in order to compare the ability to control postural sway during perturbation when stroke patients received postural sway induced by head rotation. Methods: This study included 15 stroke patients and 15 healthy adults. Each group was measured by 3D motion analysis for determination of the angle of the neck in static position and by balance performance monitor for estimation of swaying angle in both neutral posture and head rotation position. These results were then analyzed in order to compare the healthy control group and the stroke patients group. Results: In both static posture ($60.7{\pm}4.81$) and dynamic posture ($51.46{\pm}6.87$, $70.8{\pm}6.55$), significant decreases were observed in the angle of head rotation of the patient group, compared to the healthy group (p<0.05), and significant decreases were observed in the sway angle of the patient group when in the neutral position ($3.62{\pm}7$, $24{\pm}0.60$) and head rotation ($3.04{\pm}0.80$, $51.46{\pm}6.87$), compared to the healthy group (p<0.05). Conclusion: According to these findings, patients with stroke tend to restrict the ROM of head rotation and swaying angle in dynamic posture and maintain their posture instability using limitation of head movement relative to the trunk and sway angle of area which is larger than that of affected side in unaffected side.

      • Handel의 Oratorio "Messiah"의 合唱曲 分析 硏究 : 주의 영광, 할렐루야

李寬燮 배화여자대학 2000 培花論叢 Vol.19 No.-

This study puts its principle objective on academically inquiring into comprehending 2 chorals among Handel's 'Messiah', via a detailed analysis of its forms, techniques, and harmonics, and then providing opportunities to effectively conduct music with confidence. The resultant findings from the present study may be summarized, as follows : First, the fugal techniques used chorals in Handel's 'Messiah' differ from the typical fugue of Bach's style, consist for the most part of fughetta techniques. Harmony between these Handel's unique free and harmonic techniques reached the climax of the choral music. Second, Handel's choral music goes through a rest that leads a strain reticent moment after the climax at the end of the composition, puts a coda of final two or three measures, changes its tempo into an adagio, progresses vigorously, captivates the audience with closing chords brilliantly sounding, and then maximizing the effects of excitement. Third, it was found via the present study that Handel is a master composer of choruses who was informed of an effective method to build a dramatic climax through the beauty of well-knit and harmonious music in his choruses. Fourth, a conductor leading a choir is recommended to clearly grasp the subject and circumstances, confirming which vocal part the subject is, and then emphasize the vocal part. Fifth, what has been perceived via the present study is that a conductor needs to grasp the periodical characteristics of the work to be performed, fully comprehend the contents of the work via analysis of the composition and its chords, along with understanding of the intention of the composer and correct interpretation of the text by inquiring into the compositional techniques of the composers, so that the message contained in it could be transmitted in a right way.

      • 效率的인 合唱演奏를 爲한 發聲法硏究

李寬燮 배화여자대학 1986 培花論叢 Vol.5 No.-

The number of people related to choir is on the increase day by day. The reason is that not only professional musicians but also any one who has interests and talents in choir can easily enjoy it in an emotive atmosphere. The reality of Korean chorus has reached to such a point as requires high techniques of performance. For better performances are there many areas to be studied; Vocalization, Pronunciation, Balance, Blanding, Dynamics, Performing manner and Translation, to name a few. The paper centered on, among other things, posture, breathing, consonance, and pronunciation which are closely linked with voice, the most important thing in choral performances. Sound is vital not merely to choral but to all kinds of music. Thus it is necessary to build up a well-training based on scientific methods. All members of a choir should try to make a united tone which their conductor wants. It is necessary to have a good command of posture, abdominal breath, consonance and definite and harmonized pronunciation of vowels and consonants for better musical vocalization. There are many choruses in Korea but most of them endeavour only with enthusiasm without any musical techniques. Poor performances will continue if some one conducts a chorus only with some experiences and enthusiasm. Therefore a correct knowledge on vocalization and incessant efforts are required for an improved performance. When a conductor recognized the fundamental principles of vocalization, he can possibly have an effective choral performance. It is difficult to expect a perfect performance but the future will be bright when muscians make every effort to develop a beautiful and systematized performances with a welltrained and beautiful voice.
