Lung Lobe Torsion with Concurrent Intestinal Lymphangiectasia in a Maltese Dog

        윤헌영,정순욱 한국임상수의학회 2011 한국임상수의학회지 Vol.28 No.2

A 4-year-old intact male, Maltese dog weighing 2.2 kg was presented for evaluation of tachypnea and hyperthermia. On initial evaluation, the dog was dyspneic, but alert and responsive. Muffled heart sound was noted with auscultation of the right hemithorax. The radiographic findings were consolidation of the right middle lung lobe, pleural effusion, and abdominal fluid accumulation. Thoracic computed tomography confirmed a contrast-enhancing mass within the right middle lung lobe. Surgical exploration of the thoracic cavity was performed using a right fifth intercostal thoracotomy. Examination of the right lung lobe revealed 360o torsion of the right middle lung lobe at the level of the hilus, in a clockwise direction. Lung lobectomy was performed using a thoracoabdominal stapling device with 2.5-mm vascular staples. Full-thickness intestinal biopsy by exploratory laparotomy was taken from the jejunum for histological assessment. Histopathologic examination of the full-thickness intestinal biopsy revealed distended lacteal located within the submucosa of the jejunum. Intestinal lymphangiectasia was resolved with prednisone and low fat diets. Subsequent communication with the owner revealed that the patient was in good health 6 months post-discharge.

Surgical Repair of Rostral Hemimandibular Fractures with Canine Tooth Luxation in a Dog

        윤헌영,강명곤,정순욱 한국임상수의학회 2009 한국임상수의학회지 Vol.26 No.4

A sexually intact female Alaskan Malamute, 5-year-old weighing 25.2 kg presented to the Hangang Animal Hospital, following hit-by-car. On initial presentation, the dog had severe salivation and the right canine tooth luxation. Radiographs revealed fractures of the right rostral hemimandible with the right canine tooth luxation and symphyseal separation. Surgical repair of fractures was performed by use of an interdental wiring technique and external fixation technique that included pins, an acrylic connecting bar, and polymethyl methacrylate bone cement. Post-operative radiographs revealed that bridging callus was well formed over cortices of the fracture area. Pins and an acrylic connecting bar were removed 7 weeks post-operatively. The dog exhibited evidence of normal mastication.

Comparison of techniques for transdiaphragmatic thoracic drainage after diaphragmatic defect closure in dogs: a cadaveric study

        윤헌영,F. A. Mann,Suhwon Lee,정순욱 대한수의학회 2013 Journal of Veterinary Science Vol.14 No.2

Four thoracic evacuation techniques for pneumothorax elimination after diaphragmatic defect closure were compared in 40 canine cadavers. After creating a defect in the left side of the diaphragm, thoracic drainage was performed by thoracostomy tube insertion through the defect and a small (DD−SP) or large (DD−LP) puncture created in the caudal mediastinum, or through both the diaphragmatic defect and intact contralateral diaphragm with a small (DI−SP) or large (DI−LP) puncture in made in the caudal mediastinum. Differences in intrapleural pressure (IPP) between the right and left hemithoraxes after air evacuation along with differences in IPP before making a defect and after air evacuation in each hemithorax were calculated. A difference (p ≤ 0.0011) in IPP between the left and right hemithoraxes after air evacuation as well as before making a defect and after air evacuation in the right hemithorax was detected for the DD −SP group. No significant differences (p ≥ 0.0835) were observed for the DI−LP, DD−LP, or DI−SP groups. Creation of a large mediastinal puncture or thoracic evacuation through both a diaphragmatic defect and intact contralateral diaphragm can facilitate proper pneumothorax elimination bilaterally after diaphragmatic defect closure in dogs with a small puncture in the caudal mediastinum.

Long-term Follow-up after Implantation of Autologous Adipose Tissue Derived Mesenchymal Stem Cells to Treat a Dog with Stifle Joint Osteoarthrosis

        윤헌영,이정하,정순욱 한국임상수의학회 2012 한국임상수의학회지 Vol.29 No.1

A 5-year-old castrated male Chihuahua weighing 1.54 kg was examined because of a several month history of progressive right hind limb lameness. Physical examination of the stifle joints revealed pain and a grade IV medial patellar luxation on the right stifle joint. The right and left stifle joints were associated with a lameness of grade 2and grade 0, respectively. Radiography revealed osteophytes or subchondral cystic lesions on the right and left stifle joints. Osteoarthrosis (OA) scores for the right and left stifle joints were 20 and 12 respectively. Combination of surgery and implantation of autologous adipose tissue derived mesenchymal stem cells (aAT-MSCs) was determined with informed consent. 1 × 106 aAT-MSCs suspended in PBS and 0.6 mL of hyaluronic acid were injected in the right stifle joint postoperatively. Osteoarthrosis scores and the lameness grade for the right and left stifle joints were 19 and 13,and 0 and 0 19 months after treatment, respectively, and 14 and 15, and 0 and 0 five years after treatment, respectively. This case report shows radiographical evidence of a decrease in osteophytes and subchondral cystic lesions on the stifle joint with OA after aAT-MSCs injection.

Palliative Surgery in Two Dogs with Non-Curative Palatal Tumors

        윤헌영,이정하,신동욱,박희명,정순욱 한국임상수의학회 2014 한국임상수의학회지 Vol.31 No.5

Two dogs were presented with a history of anorexia, respiratory distress, or epistaxis. On physicalexamination, the dogs showed difficulty breathing and open-mouth breathing and large masses were found on the hardpalate and soft palate. Cardiac arrest happened in case 2 and the dog was stabilized with cardiopulmonary resuscitationand oxygen therapy. Computed tomography demonstrated that the mass occupied the oropharynx, intrapharyngealostium, and laryngopharynx including the hard palate and soft palate in case 1. Palliative surgery was decided to improveswallowing and breathing with owner’s consent in two dogs. Buccal mucosal flaps were performed for reconstructionof defects using rotational and single-pedicle advancement flaps and bilateral 90 degree transposition flaps in cases1 and 2, respectively. Histopathology results described the oral masses as amelanotic melanoma in two dogs. Theowners reported that there was normal swallowing and breathing at 7 days postoperatively in two dogs. In case 1,recurrent tumor was identified caudal to the hard palate 4 weeks after surgery. The owner did not allow further treatmentand the dog became lost to follow-up at 2 months postoperatively. In case 2, there was no clinical or radiographicevidence of a local recurrence or distant metastasis at 3 weeks after surgery. The owner informed that the dog diedsuddenly with no signs of anything particularly wrong at 7 weeks after surgery.

Gastrotomy Approach Retrieval of Esophageal Foreign Body using Long Forceps Technique in Five Dogs

        윤헌영,강명곤,정순욱 한국임상수의학회 2009 한국임상수의학회지 Vol.26 No.6

Five dogs presented to the Veterinary Medical Teaching Hospital of the Konkuk University and Hangang Animal Hospital with a history of foreign body ingestion. On physical examination, five dogs showed lethargy, anorexia, or vomiting. Plain radiographs revealed that radiopaque foreign bodies lodged in the heart base or caudal thoracic esophagus. Positive contrast esophagogram revealed that large foreign bodies severely expanded the esophagus and there was no evidence of leakage of the contrast agent from the esophagus into the thoracic cavity. Gastrotomy for retrieval of esophageal foreign bodies using long forceps technique was performed. Esophageal foreign bodies were successfully retrieved in all dogs. The follow-ups were completed 10 days to 2 years after surgery. The follow-up information was based on physical examination by veterinarians and telephone interview with owners. The owners reported that there was no evidence of complications related to surgery such as vomiting, regurgitation, dysphagia, gagging, hyper-salivation, or anorexia in all dogs.

The Use of Internal Obturator Transposition for Perineal Herniorrhaphy in Three Dogs

        윤헌영,강명곤,정순욱 한국임상수의학회 2009 한국임상수의학회지 Vol.26 No.6

Three dogs presented to the Veterinary Medical Teaching Hospital of the University of Konkuk and Hangang Animal Hospital with a history of perineal swelling. No tenesmus, stranguria, or any clinical signs other than the swelling was observed by the owners in three dogs. On physical examination, the swelling was observed unilaterally in two dogs and bilaterally in a dog. Digital palpation to the swelling confirmed reducible perineal herniation in two dogs and irreducible perineal herniation in a dog. Plain radiographs revealed that no pelvic or abdominal contents other than the fat were displaced into subcutaneous perineal region in three dogs. Internal obturator transposition herniorrhaphy was performed for correction of perineal herniation in three dogs. Contralateral herniation involving fat was noted after surgery in a dog. The follow-up information was based on physical examination by veterinarians or telephone interview with owners. The owners reported that there was no evidence of complications related to surgery such as sciatic nerve injury, rectal prolapse, wound dehiscence, or perineal hernia recurrence in all dogs.

Clinical Application of Autologous Adipose Tissue Derived Mesenchymal Stem Cells in Five Dogs with Stifle Joint Osteoarthrosis

        윤헌영,강동준,이수한,정순욱,정병현 한국임상수의학회 2014 한국임상수의학회지 Vol.31 No.3

Five dogs presented with a history of pelvic limb lameness. On physical examination of the stifle joints, five dogs had pain, lameness, patellar luxation, or ligamentous instability. Craniocaudal and mediolateral radiographicprojections revealed osteophytes or subchondral cystic lesions on the stifle joints. Based on a previously describedOsteoarthrosis (OA) scoring technique, five dogs showed high OA scores. Combination of surgery and implantationof autologous adipose tissue derived mesenchymal stem cells (aAT-MSCs) or percutaneous injection of aAT-MSCswas determined with informed consent. 1 × 106 aAT-MSCs suspended in PBS was injected in the stifle joints. Thefollow-ups were completed 12 months after surgery. The follow-up information was based on physical examinationby veterinarians. The lameness, pain on manipulation, and OA scores improved six or 12 months after implantationof aAT-MSCs. There was a radiographic evidence of decreased osteophytes and subchondral cystic lesions. These resultssuggest that implantation of aAT-MSCs can be considered an option for management of cases of OA in the stifle joints.

Stabilization of Recurrent Medial Shoulder Luxation by Use of Three Bone Anchors in a dog

        윤헌영,노미영,정순욱 한국임상수의학회 2013 한국임상수의학회지 Vol.30 No.1

A 12-year-old male poodle weighing 2.0 kg presented for evaluation of right thoracic limb lameness. The owner reported that the dog showed recurrent bilateral shoulder joint luxation after a car accident for a year. Recently, the left shoulder joint appeared well maintained, but right shoulder joint luxation was exacerbated. On physical examination, the dog showed non-weight bearing lameness on the right thoracic limb. Craniocaudal radiographic views revealed medial displacement of the right humerus. Mediolateral radiographic views revealed overlap of the glenoid cavity and humeral head. Muscle atrophy of the right thoracic limb, reduced biceps brachii muscle tendon tone, a tear of the medial glenohumeral ligament, and a rupture of the subscapularis tendon were identified intraoperatively. Transposition of the biceps muscle tendon was performed. However, at 7 days, there was evidence of right shoulder reluxation on radiographs. The second surgery was performed with two 2 mm × 6 mm cortical bone anchors and a 4mm× 6 mm cancellous bone anchor placed in the cortical bone of the distal scapula and the cancellous bone of the proximal humerus respectively. Two scapular bone anchors were then connected with a humeral bone anchor using heavy nylon suture to minimize shoulder abduction range of motion. On radiographs right after surgery and 6 weeks after surgery, the affected limb revealed no evidence of medial shoulder luxation. At 6 months, no evidence of lameness was noted on the right thoracic limb.
