        편마비 환자의 슬건근의 근력의 차이가 보행에 미치는 영향

        The purpose of this study is to estimate how much the motility of affected and unaffected lower limb respectively would have effects on their ambulation through comparing the difference between their affected and unaffected lower limb and studying the correlation between such difference and their ambulation. In addition, the study also intends to find out remedial measures suitable for improving their ambulation with relevant physical treatment. To do this, a quantitative electromyogram(QEMG) test was done to hamstring of affected and unaffected lower limb so as to yield IP values. Based on such IP values, RMS(root mean square) values as the total sum of IP values were found with QEMG analysis system (made by Medelec Co.) and then the ambulation depending on the difference in muscular strength were analyzed by ink-foot print method as well as the corresponding statistics were processed by T-test through SPSS. The differences in muscular strength of hamstring in unaffected limbs of hemiplegic patients only affect stride length. It was statistically significant. The effect of difference in muscular strength of the hamstring in affected limb of hemiplegic patients on their ambulation was very different between strong and weak group in terms of walking velocity, step per minute, stride and step length, showing statistically significant difference(p<.05). Although there was a difference in the step length of the affected limb, it was not statistically significant(p>.05). For the unaffected hamstring, there were significant differences of stride length between in unaffected limb and in affected limb. For the affected hamstring, there were also significant differences in walking velocity, step per minute, stride and step length. Besides, it was found that the difference of the muscular strength between strong and weak group was at the highest.

        연하장애 환자의 턱밑근 근력 운동이 연하기능에 미치는 효과

        이 연구의 목적은 연하장애를 가진 뇌졸중 환자들의 턱밑근육집단 근력향상을 위한 턱밑근 근력훈련(Jaw Muscle Strength Training)을 하였을 때, 연하기능에 미치는 영향을 알아보았다. 연구대상은 2012년 6월에서 10월까지 강원도 소재 H병원에서 입원, 외래로 작업치료를 받는 연하장애를 가진 뇌졸중 환자 25명 중에서 턱밑근 근력 훈련 13명, 전통연하재활치료 12명의 중재 전, 후의 변화를 비교해 보았다. 대상자들은 8주간 주 3회, 30분간 턱밑근 근력훈련 도구(EMST 150 device)를 75% 압력으로 훈련을 실시 후 턱밑근육집단의 근 활성도 향상을 알아보기 위하여 중재 전, 후에 SEMG(Surface Electromyographic)를 실시하고, 연하기능 향상을 알아보기 위해 비디오 투시연하검사(VFSS)를 통한 기능적 연하곤란척도(FDS scale)를 이용하여 검사하였다. 그 결과 턱밑근 근력훈련 후에 턱밑근육 집단의 근 활성도와 연하기능이 유의하게 향상된 결과를 보였다(p<.05). 따라서 연하장애를 가진 뇌졸중 환자들의 재활치료 시 턱밑근 근력훈련을 통한 턱밑근육 집단의 근 활성도 향상이 연하장애를 가진 환자에게 적용할 수 있는 효과적인 치료방법으로 사료된다. The purpose of this study was to identify the effects of jaw muscle strength training in stroke patients with swallowing disorders. The exercises were aimed at improving the muscle strength of the lower jaw muscle group and the swallowing function. There were 25 stroke inpatients and outpatients with dysphagia at H hospital located in Gangwon-do from June to October 2012 and these patients were grouped into the jaw muscle muscle strength training group composed of 13 patients who went through 75% pressure training of EMST 150 device and traditional dysphagia therapy group composed of 12 patients who went through traditional dysphagia therapy. The training was conducted three times a week, for 30 minutes each time, over an 8 week period. Thereafter, improvements in the muscular activation of the lower jaw muscle group were identified by surface electromyography(SEMG). In addition, signs of improvement in the patients' swallowing function were examined by conducting the videofluoroscopic swallow study(VFSS) using the functional dysphagia scale(FDS). The muscular activation and swallowing function of the lower jaw muscle group exhibited a statistically significant improvement after the patients underwent jaw muscle strength training(p <.05).         편마비 환자의 하지의 근력 차이가 보행에 미치는 영향

Although there was a difference in the step length of the affected limb, it was not statistically significant(p>.05). For the unaffected hamstring, there were significant differences of stride length between in unaffected limb and in affected limb. For the affected hamstring, there were also significant differences in walking velocity, step per minute, stride and step length. Besides, it was found that the difference of the muscular strength between strong and weak group was at the highest. Thus, the effect of difference in muscular strength on ambulatory factors such as walking velocity, step per minute, stride and step length did not show any statistical significance but differences in affected and unaffected quadriceps. However, it was found that both the difference of stride in unaffected hamstring and the difference of all ambulatory factors except from step length with affected limb had effects on ambulation depending upon muscular strength.

        뇌졸중 환자의 발목 보조기 착용이 발목 근육의 활성도와 선 자세 균형에 미치는 영향

        본 연구의 목적은 뇌졸중 환자의 선 자세에서 보조기 착용과 미착용 시 발목 근육의 전 경골근과 내측 비복근에 근 활성도의 변화가 균형에 미치는 영향을 알아보았다. 연구 방법은 강원도에 위치한 H, K, R 병, 의원에서 운동치료(Therapeutic Exercise)를 받는 뇌졸중 환자 26명중 발목 보조기 착용군 13명, 미착용군 13명으로 나누어 EMG를 이용하여 건측과 환측의 전경골근(Tibialis anterior;TA)과 내측 비복근(medial gastrocnemius;MGCM)의 근 활성도를 측정하였고, TETRAX 장비로 안정성 지수(Stability index;SI)와 체중 분포 지수(Weight distribution index;WDI)를 측정하였다. 결과는 독립표본 t 검정으로 분석하였다. 연구결과 발목 보조기 착용군은 환측의 전경골근 근 활성도에서 유의한 차이가 있었고(p<.05), 안정성 지수에서는 눈 감고 선 자세와 정적 안정성 지수에서, 체중분포지수에서는 스폰지 위에 눈 뜨고 선 자세에서 유의한 차이가 있었다(p<.05). 따라서 만성 뇌졸중 환자의 근 활성도와 균형개선을 위해 발목 보조기 사용이 선 자세의 안정성 제공과 체중분포에도 영향을 미치는 것으로 예상되므로 뇌졸중 환자의 발목 보조기 사용 시 고려되어야 할 것이다. The objective of this study is to learn how muscle activity of tibialis anterior and medial gastrocnemius changes and the balanced is affected when a stroke patient wears an ankle support in standing posture. The study was conducted from H, K, and R clinic or hospital with 26 stroke patients who participate in the therapeutic exercise. Among the 26 subjects, 13 of them wore an ankle support and the other 13 did not. The EMG was utilized to measure the muscle activity in the tibialis anterior (TA) and the medial gastrocnemius (MGCM), and the TETRAX equipment was used to calculate the stability index (SI) and the weight distribution index (WDI). The data were analyzed using independant t-test. As a result, the group wearing an ankle support showed a statistically significant difference in the muscle activity of tibialis anterior of the affected side(p<.05). In the stability index, a statistically significant difference was observed when the patients stood still with their eyes close, and in the static stability index(p<.05). In the weight distribution index, when they were instructed to stand on sponge with their eyes open (p<.05). It is considered that the ankle support, which is influenced to improve muscle activities and balance of stroke patients, also provides stability in the standing posture and affects the weight distribution. Therefore, stroke patients should take this into account when deciding to use an ankle support.

        청소년기 척추 측만증과 정상학생의 균형능력 및 체간 유연성과 근력의 비교

        본 연구의 목적은 청소년기 특발성 척추측만증 환자를 대상으로 정상군과 비교하여 균형능력, 체간의 유연성, 체간 굴곡 및 신전근의 등속성 근력비교를 통하여 측만증 환자의 특성을 알아보는데 있다. 척추측만증 환자 25명과 대조군26명을 대상으로 하여 Tetrax balance system을 이용한 균형능력 평가와 함께 체간의 굴곡 및 신전 유연성, 등속성근력 장비(CON-TREX)를 이용하여 체간 굴곡, 신전의 등속성 근력평가를 실시하였다. 통계적 처리는 Mann-Whitney test를 이용하여 측 만군과 정상군간의 차이를 비교분석하였다. 척추측만증 유무에 따른 체간의 유연성 검사상 두 군간의 유의한 차이는 없었으며, 균형능력 검사의 안정도 지수 (stability index) 또한 두 군간 차이가 없었다. 그러나 체간의 근력평가에서 각속도 120 °/sec의 등속성 운동 시 측만증 군과 정상 군 간 통계적으로 유의한 차이가 있었다. 이의 결과를 근거로 청소년기 특발성 척추측 만증 환자의 특성을 알 수 있었으며, 이런 특성의 파악이 향후 척추측만증 환자의 치료에 도움이 될 것으로 사료된다. The purpose of this study was to find the characteristics and to find the difference between adolescent idiopathic spinal scoliosis patients and normal students by comparing the difference in balance (stability), flexibility of t runk and muscle power of t runk f lexion and extension. Twenty-five patients who had spinal scoliosis were selected and twenty-six with structurally normal spines were checked their flexibility of trunk, stability using Tetrax balance system, and muscle power or trunk flexor and extensor with CON-TREX biomechanical system. Mann-Whitney test was used to analyze the difference between these groups. It was shown that there was no r emark able d ifference in f lexibility of t runk and a lso in b alance ability b etween the two groups. But, we have discovered statistically remarkable difference in trunk extensor power at 120 °/sec. From this result, we have recognized the characteristics of adolescent idiopathic scoliosis patients, and we believe that these findings may lead us to treat the scoliosis patients in a way we have not tried before.

        고유수용성 신경근 촉진법과 자가 보조적 관절 가동운동이 견관절 유착성 관절낭염 치료에 미치는 영향

        The purpose of the study was to investigate a influence in Proprioceptive Neuromuscular Facilitation, and self-assisted ROM exercise of shoulder adhesive capsulitis. Methods : The methods of the study was to investigate a change in range of motion (ROM), an increase in ROM, visual analogue scale (VAS) and disability questionnaire by Proprioceptive Neuromuscular Facilitation, and self-assisted ROM exercise on 32 patients who were diagnosed with shoulder adhesive capsulitis. Results : There was no significant difference between the two groups who were treated in the Proprioceptive Neuromuscular Facilitation and self-assistive ROM exercise group for flexion, extension, internal rotation, external rotation, VAS and disability questionnaire. Both group`s flexion, extension, internal ratation, and external rotation levels were significantly different before and after the treatment. And significant statistical decrease in VAS and disability was seen. The extension of the shoulder joint was closely related to external rotation (r=0.84). There was a close relationship between internal rotation and external rotation at the shoulder joint. There was no increased range of extension through the exercise method. However, flexion, extension, internal rotation, external rotation, VAS, and disability questionnaire of Proprioceptive Neuromuscular Facilitation groups were obviously higher than in the self-assisted ROM exercise group. Conclusion : Our study suggest that considering Proprioceptive Neuromuscular Facilitation for the patient who has shoulder adhesive capsulitis in clinic.
