Pulmonary thromboembolism in patient receiving hip and lower extremity orthopedic surgery is not uncommon and can cause life-threatening event. Because of its non-specific symptom, it is not easy to make diagnosis and treat promptly in operating room. Rapid and accurate access of pulmonary thromboembolism is very important in good prognosis of high risk patients. It can develop any time in perioperative time, so that careful monitoring to patient is necessary to early diagnosis. We report a case in which develop pulmonary thromboembolism just after providing combined spinal-epidural anesthesia for femur neck fracture surgery. Because we do nothing but only changing position lateral to supine, change of position must be a factor of developing pulmonary thromboembolism.
확산판의 투과율이 고출력 양자점 조명의 광특성에 미치는 영향에 대한 연구

The optical characteristics of high-power direct-lit white light-emitting diode (LED) lighting were investigated, where a quantum dot (QD) film was adopted to enhance the color-rendering index (CRI). The transmittance of the diffuser plate and the concentration of the QD film were varied in this study. The color coordinates and the correlated color temperature (CCT) did not show any appreciable change, while the CRI values increased slightly as the transmittance of the diffuser plate decreased. The investigated optical properties were nearly independent of the viewing angle, and the luminance distribution was close to Lambertian. The CCT decreased from approximately 6000 K to approximately 4000 K as the concentration of the QD film increased from 0 to 7.5 wt%, which was due to the enhanced red component in the emission spectrum. The CRI increased to approximately 95 for some optical configurations of the lighting. These results demonstrate that glare-free, color-changeable, high-rendering LED lighting can be realized by using a combination of a diffuser plate of appropriate transmittance and a red QD film. 눈부심 방지를 위한 확산판의 투과율과 연색지수 향상을 위한 양자점 필름의 농도가 고출력 직하형 백색 LED 조명의 광특성에 미치는 영향을 조사했다. 확산판의 투과율이 감소할 경우 조명의 휘도는 줄어들었고 색좌표, 상관색온도는 별다른 변화를 보이지 않았으며 연색지수는 약간 상승했다. 시야각에 따른 광특성의 편차는 거의 없었고 휘도 분포는 람버시안 분포에 가까웠다. 양자점 필름의 농도가 증가할 경우 스펙트럼 상 적색 성분이 늘어나면서 이 성분의 비중을 표현하는 색좌표 x가 증가했고 상관색온도는 약 6000 K에서 4000 K 부근으로 현저히 감소했다. 아울러 조명의 구조를 최적화하면 연색지수가 95까지 증가함을 확인했다. 이런 결과는 적절한 투과율의 확산판과 양자점 필름의 조합을 통해 눈부심이 없고 시야각에 따른 색편차가 거의 없으며 특히 색좌표의 가변이 가능한 고연색성의 조명 구현이 가능함을 보여준다.

Objectives: To identify the current status of Korean medical practice pattern, diagnosis and treatment of dementia through recognition survey, and to use it as a preliminary data for various dementia research. Methods: Questionnaires were developed through expert meetings. The disease was defined as dementia and mild cognitive impairment, and areas were designated to practice pattern, diagnosis and treatment. From December 18, 2016-January 18, 2017, 221 respondents, including 36 neuropsychiatrists of Korean Medicine and 185 general physicians (including other medical specialists) were included. Results: 1. In both groups, the most commonly used KCD (Korean standard classification of disease and cause of death) were in the order of Unspecified Alzheimer`s Dementia (F00.9), Mild Cognitive Impairment (F06.7), and Unspecified Dementia (F03). 2. The most commonly used pattern identification were zang-fu and qi-blood-yin-yang in both groups. 3. Diagnostic evaluation tools were mainly conducted by MMSE, radiologic examination, K-DRS, GDS and CDR in both groups. 4. Both groups reported using acupuncture and herbal medicine mainly. 5. In both groups, the acupuncture method was used extensively in the order of Body, Scalp, and Sa-Am. 6. Neuropsychiatrists used a variety of herbal medicines such as Wonjiseokchangpo-san (Yuanzhushichangpu-san), Yukmijihwang-tang (Liuweidihuang-tang), Palmijihwang-won (Baweidihuang-won), Sunghyangjungki-san (Xingxiang Zhengqi-san) and Ondam-tanggami (Wendan-tangjiawei). General physicians used a variety of herbal medicines such as Ondam-tanggami (Wendan-tangjiawei), Bojungikgi-tang (Buzhongyiqi-tang), Yukmijihwang-tang (Liuweidihuang-tang). 7. Neuropsychiatrists used a variety of Korean herbal preparation products (benefit and non-benefit) such as Ekgan-sangajinpibanha (Yigan-sanjiachenpibanxia), Yukmijihwang-tang (Liuweidihuang-tang), Jodeung-san (houteng-san), Palmijihwang-won (Baweidihuang-won). General physicians used a variety of Korean herbal preparation products such as Bojungikgi-tang (Buzhongyiqi-tang), Banhabaegchulcheonma-tang (banxiabaizhutianma-tang), Yukmijihwang-tang (Liuweidihuang-tang), Ekgan-sangajinpibanha (Yigan-sanjiachenpibanxia), Palmijihwang-won (Baweidihuang-won). Conclusions: By confirming awareness of Korean medical doctors treating dementia in clinical fields and understanding differences between neuropsychiatrists of Korean medicine and general physicians, it can be used to understand guideline users` needs and confirm clinical questions during development of future clinical practice guidelines for dementia.

This paper presents a phase-locked loop (PLL) based resonator driving integrated circuit (IC) with automatic parasitic capacitance cancellation and automatic gain control (AGC) loop. The PLL makes the driving frequency to be locked at the resonant frequency. The automatic parasitic capacitance cancellation loop automatically matches the compensation capacitance array to parasitic capacitance of the resonator. The AGC loop keeps the oscillation at the suitable amplitude, and the motional resistance can be detected by the AGC digital output.

High-resolution chirp profiles were analyzed to investigate the echo types of near-surface sediments in the Yellow Sea off the Baegryeong Island. On the basis of seafloor morphology and subbottom echo characters, 7 echo types were identified. Flat seafloor with no internal reflectors or moderately to well-developed subbottom reflectors (echo type1-1 and 1-2) is mainly distributed in the southern part of the study area. Flat seafloor with superposed wavy bedforms(echo type 1-3) is also distributed in the middle part. Mounded seafloor with either smooth surface or superposed bedforms (echo type 2-1, 2-2, and 2-3) occurs in the middle part of the study area. Irregular and eroded seafloor withno subbottom reflectors(echo type 3-1) is present in the northern part of the study area off the Baegryeong Island. According to the distribution pattern and sedimentary facies of echo types, depositional environments can be divided into three distinctive areas: (1) active erosional zone due to strong tidal currents in the northern part; (2) formation of tidal sand ridges in response to tidal currents associated with sea-level rise distributed in the middle part; and (3) transgressive sand sheets in the southern part. Such a depositional pattern, including 7 echo types, in this area reflects depositional process related to the sea-level rise and strong tidal currents during the Holocene transgression.
