Solid and papillary epithelial tumor that occurs chiefly in young woman. It is amenable to surgical resection and cure. Recurrence after operation and distant metastasis are rare. But, a few cases with recurrence or distant metastasis have been reported. We have encounted a case of solid and papillary tumor of the pancreas with distant metastasis and recurrence 7 years after resection in 34 years old female, so report it.

목적: 인공관절 치환술에서 치환물에 시멘팅을 하는 시간에 따른 치환물과 시멘트 사이의 결합강도의 변화를 알아보고자 하였다. 대상 및 방법: 인공 슬관절에 흔히 사용되는 CMW<SUP>®</SUP> 1과 CMW<SUP>®</SUP> 3 (Depuy Ltd., Blackpool, UK) 두 가지 종류의 시멘트를 사용하였다. CMW<SUP>®</SUP> 1을 시멘팅 후 각각 2, 4, 5, 6분 후에, CMW<SUP>®</SUP> 3은 각각 3, 5, 6, 7분 후에 치환물에서 잘라낸 금속블럭 위에서 압력을 가하여 접착시키고 36.5℃ 생리식염수에 24시간 담근 후, Instron Model 8874를 이용하여 인장강도를 측정하였다. 인장응력은 10 ㎜/min의 비율로 시멘트가 금속블럭에서 분리될 때까지 가하였다. Kruskal-Wallis test와 Mann-Whitney U test를 이용하여 통계학적 차이에 대한 유의성을 판정하였다. 결과: CMW<SUP>®</SUP> 1의 경우 2분 후, CMW<SUP>®</SUP> 3의 경우는 3분 후 최대 인장강도를 나타내었으며, 5분 후와 6분 후 인장강도는 각각 급격히 감소하였다. 결론: 시멘팅 시간에 따른 결합강도의 변화를 고려하여 적절한 시멘팅 시간을 지킴으로써 해리 현상을 줄일 수 있을 것으로 사료된다. Purpose: The objective of this study was to compare the bonding strength between cement and an implant according to the cementing time. Materials and Methods: The two types of cement used were CMW<SUP>®</SUP> and 3 (Depuy Ltd., Blackpool, UK). Plastic molds containing CMW<SUP>®</SUP> 1 were pressed onto metal blocks at 2, 4, 5 or 6 minutes after mixing the cement, while molds containing CMW<SUP>®</SUP> 3 were pressed onto blocks at 3, 5, 6 or 7 minutes after mixing the cement. Tensile strength was tested with using an Instron Model 8874 (Instron Corp., Canton, MA, USA). Tensile strengths were compared using the Kruskal-Wallis test and the Mann-Whitney U test. Results: The strongest bonding strengths for the tensile load were at 2 minutes post-mixing for CMW<SUP>®</SUP> 1, and at 3 minutes post-mixing for CMW<SUP>®</SUP> 3. The strength rapidly decreased after 5 minutes for CMW<SUP>®</SUP> 1 and after 6 minutes for CMW<SUP>®</SUP> 3. Conclusion: This study suggests that the risk of loosening between cement and an implant is likely to be minimized by the surgical technique that considers the bonding strength according to time.

Purpose: The patellar thickness is one of the most important factors that affect patellar tracking in patients who undergo total knee arthroplasty (TKA). The purpose of this study is to help the surgeons who perform TKA by presenting basic data on the patellar thickness of the osteoarthritic Korean women. Materials and Methods: From January 2006 to May 2007, we measured the patellar thickness of 330 patients (538 cases) who were undergoing TKA. The thickest portion of the patella was measured on a scale of 0.01 mm with using an electronic caliper before patellar osteotomy. We analyzed the data to determine the relationships between the patellar thickness and the patients` age, height, weight and BMI, and the osteoarthritic change of the patella. Results: The average thickness was 21.37±1.43 mm. There was a statistically significant positive relation of patellar thickness with weight and height and a negative relation with the degree of osteoarthritis. Age and BMI were not related with the patellar thickness. Conclusion: The average thickness was 21.37±1.43 mm and it was closely relate with weight and height and the osteoarthritic change of the patella.

신경섬유종은 비교적 흔한 연부 조직 종양이지만 고립성 골내 신경 섬유종은 가장 희귀한 골종양 중 하나로 전세계적으로 수십 예만이 보고되고 있으며 이 역시 상악골과 하악골에 국한되어 있다. 저자들은 대퇴 경부에 발생한 고립성 골내 신경섬유종 1예에 대하여 대퇴골 대전자 절골술을 통한 병소내 소파술과 골이식술을 시행하여 추시 중인 환자를 문헌 고찰과 함께 보고하는 바이다. Whereas neurofibroma is a fairly common soft tissue tumor, solitary intraosseous neurofibroma is a very rare type of bone tumor, and there relatively few cases have been reported worldwide, which were localized to the maxilla and mandible. We experienced a solitary intraosseous neurofibroma and treated it by curettage and bone graft through trochanter osteotomy of the femur. We report a rare case of solitary intraosseous neurofibroma and provide a review of reported cases.
