        고인성 열가소성 복합재료 AS4/PEEK의 피로강도에 관한 기초적 검토

        First, various specimen geometries, namely, coupon type, waisted type and dog-bone type, were examined to determine appropriate fatigue specimen of thermoplastic composite material AS4/PEEK and the n, fatigue strength of smooth and notched specimens of AS4/PEEK [-45/0/+45/90]2s was investigated. Fatigue tests were performed under load controlled condition at a stress ratio of 0. 1 at a frequency of 5Hz. Stiffness degradation of specimens with fatigue cycling was monitored using an automated unloading compliance technique. The waisted type specimen is found appropriate for smooth fatigue specimen geometry of AS4/PEEK. As for the effect of stress concentration, it is found that fatigue strength is higher for a 2mm-diameter hole notched specimen than a 5mm-diameter one. Fatigue notch factor decreases with the increase of fatigue life. These results are far different from the trend of fatigue strength of metallic materials. The stiffness variation of smooth specimen was only 4% at maximum until final fracture. On the other hand, the stiffness of hole notched specimen was reduced by 45% at maximum. Notched fatigue strength was compared between thermoplastic composite AS4/PEEK and thermosetting composite Graphite/Epoxy. In long-life fatigue (>104), the AS4/PEEK composite shows superior fatigue strength, but in short-life fatigue, the fatigue strength of the Graphite/Epoxy composite is nearly equal or somewhat higher than that of the AS4/PEEK composite.

        International trade agreements are impacting on nursing. Nursing is becoming global profession, so nurses need to be responsive to practice globally. Goals for globalization of nursing are to improve the health of people in the world and to share nursing knowledge and experiences among nurses across countries and to advance nursing science and art to benefit the people in the global village. A shortage of RNs exists in countries throughout the world. The primary receiving countries are U.S, Australia, Canada and U.K. The effects of migration on donor countries include the loss of skilled personnel and economic investment, receiving countries receive skilled nurses to fill critical shortages with less economic investment But nurses migrates to seek better wages and working conditions than in their native countries. Most of Korean nurses want to work in the U.S. When Korean nurses work in the hospital setting in the U.S, the language barrier was a problem faced by them. They faced problems of a difference of nursing practice and cultural difference, interpersonal relationship in the stage of adjustment. This shows some kind of programmes should be developed to facilitate and solve Korean Nurses` adjustment to USA hospital in the future.

        간호학 임상실습교육 실태 조사

        Purpose: The purpose of this study was to ascertain the current state of clinical nursing education at hospitals. Methods: Questionnaires were sent out to 236 hospitals which have over 300 beds. Out of these, 116 hospitals responded, and the collected data was analyzed by mean, frequency, t-test, and ANOVA test. Results: These hospitals have teaching agreements with 4.2 nursing colleges and clinical education lasts 8 months. Clinical education status regarding hospital characteristics, between a university hospital, non-university hospitals, among advanced general hospitals, general hospitals, and special hospitals showed statistical differences in colleges per hospital and nurses’ degree. 37.9% of cases have no internal regulation for nursing education, and in 68.1% of cases, students practice was limited to simple nursing care. The current primary guide for students practice was head nurse (61.25%), and the course professor took charge of mainly the conference. The difficulties as an educational hospital are increases in work load, difficulties in teaching, excessive number of students, simultaneous practical training, complaints from patients, lack in training manpower, and stress. Conclusion: This study determined that the big hospitals are heavily burdened by nursing education and that it will be necessary to establish standards for educational hospitals to ensure higher quality education.

        소아 신경질환에서의 항강글리오사이드 항체검사의 임상적 유용성

        목적: 항강글리오사이드 항체는 신경세포막 사이에 존재하는 강글리오사이드 세포를 공격하는 기전으로 여러 신경학적 증상을 일으키는 것으로 알려져 있다. 특히 길랑바레증후군 등의 면역매개 신경질환에서의 연관성이 커 이에 대한 여러 연구가 진행되고 있다. 하지만 현재까지 소아영역에서의 연구는 아직 부족한 실정이다. 본 논문에서는 항강글리오사이드 항체 패널 검사를 통해 소아영역에서의 본 검사의 진단학적 유용성을 알아보고자 하였다. 방법: 2010년 10월부터 2016년 4월까지 5년7개월 동안 의식변화, 운동실조, 근력저하, 복시, 마비의 중추신경계 이상 증상을 주소로 전남대학교병원에 내원하여 항강글리오사이드 항체 검사를 시행 받은 18세 미만 소아환자들의 의무 기록을 후향적으로 수집, 검토하였다. 결과: 총 42명의 환자들 중 항강글리오사이드 항체 양성군으로 분류된 것은 11명이었고, 대상들은 GBS, 뇌간뇌염, 급성 파종성 뇌염, 운동실조가 없는 급성안근마비, Tolosa-Hunt 증후군 등으로 각각 진단 할 수 있었다. 진단명에 따라 분류하였을 때 가장 높은 항강글리오사이드항체 양성율을 보인 질환은 뇌간뇌염으로, 총 5명의 뇌간뇌염 환자 검체 전부에서 항체가 검출되어 매우 높은 양성율을 보였고, 항강글리오사이드 항체와의 연관성이 알려진 대표적인 질환인 GBS에서는 오히려 낮은 양성율을 보였다. 결론: 항강글리오사이드 항체검사는 뇌간뇌염 증상을 보이는 환자에서와 실조증 등의 증상을 보이는 소아환자에서 진단시에 매우 유용한 검사임을 확인할 수 있었다. 또한 성인과는 달리 GBS 환자에서는 그리 높지 않은 양성률(20%)을 보였고, 이에 반해 여러가지 질환군들에서 뇌간침범을 보이는 증상이 있는 경우에 더 높은 양성율을 보였다. 그러나 검토한 숫자가 많지 않아, 보다 더 많은 수의 증례가 포함되어야 할 것으로 사료된다. Purpose: Ganglioside antibodies are known to be related to central nervous system diseases such as Gullain-Barre syndrome, and Miller-Fisher syndrome or other illnesses with neurologic symptoms like motor weakness, ataxia, changed mental status, diplopia, and dizziness. In this study the utility of anti-ganglioside antibody titer as a diagnostic marker and its correlation with clinical course in pediatric patients were investigated. Methods: From October 2010 to Apirl 2016, anti-ganglioside antibody panel was performed in 42 patients with one of these symptoms: ataxia, motor weakness, changed mental status, diplopia. The panel was composed of 6 subtypes of anti-ganglioside antibodies: Anti-GM1 IgG/IgM, Anti-GD1b IgG/IgM, Anti-GQ1b IgG/IgM. The results were compared with other aspects of clinical diagnostic studies including Brain magnetic resonance imaging(MRI), electroencephalogram(EEG), cerebrospinal fluid examination and routine laboratory work-up Results: Among 42 patients, total 11 patients showed positivity on antiganglioside antibody. Among anti-ganglioside antibody positive patients, brainstem encephalitis were the most common followed by Gullain-Barre syndrome, acute opthalmoplegia without ataxia, acute disseminated encephalitis, encephalitis and Tolosa-Hunt syndrome. Subtypes of ganglioside antibodies are investigated in each diseases, but not like previous datas, subtypes were varied case by case in same disease, showing lack of correlation. Conclusion: Anti-ganglioside antibody panel exam was useful in diagnosing brainstem encephalitis and several other neurologic illnesses with cerebellar symptoms. Also, unlike previously known adult data, positivity of anti-ganglioside antibodies in GBS was relatively low in children (20%). However, further study might be done to clarify connections between antiganglioside antibodies titer and the clinical course of the diseases.

        A stocked type anchor is used in the stow net. Since one side fluke of this anchor is projected into the upward in the sea bottom, the fishing gears including net and rope are snagged by this fluke, and sometimes a big accident happens. If we could get rid of this unnecessary fluke, it will be the best way in the stow net anchor. The attaching position and shape of stock can be changed in the ideal anchor with only one side fluke. Under the purpose of improving the most effective stock with only one fluke, the model anchor tests were carried out with the changes of size, shape and attaching position of stock, and examined the intrusive quality, stability and holding power which are considered to be the reliability of anchor. In case that the conventional stock is used, acceptable attaching position of stock judged from the initial and secondary holding is position B. In the position A. the right stand corresponding to the initial holding is difficult, on the occasion that the stock length is quite short. The secondary holding gets worse with decreasing of the stock lenth in the position C, but in case that the stock is sufficiently long the secondary holding is rather better than any other position. Therefore, we may admit the possibility to change the present attaching position of stock. The qualities of the transformed stocks are generally interior to the conventional type. But the application possibility can be seen in some model types. We think especially the bent shape round rod type, plate and V-angle types seem to be worthy of study in the future.
