The pericardial adhesions following the first open heart surgery pose a major problem that can increase the morbidity and mortality in cases of reoperation because of the danger of damaging the heart, great vessels or grafts. The purpose of this study was to determine the effect of intrapericardial infusion of 5% dextrose solution and heparin on experimental pericardial adhesions. The 15 white rabbits were divided into three treatment groups of 5 animals each: group 1 [control group], group 2[5% dextrose group] and group 3 [heparin group]. At 4 weeks, all animals were sacrificed and the pericardial adhesions were graded as follows: 1[none], ll[mild], Ill[moderate] and IV[severe]. Histological examination was performed on a subset of each group. The difference of adhesion between control and heparin group was evaluated by Chi-square analysis. The results were as follows: 1. Pericardial adhesions developed in 80% of animals in group 1 and 2. 2. Only 40%> of animals in group 3 had mild to moderate pericardial adhesions. This was significantly different from the control group [P=0.01]. 3. Histological examination showed that the pericardium with adhesion was thickened with fibrosis, while that without adhesion was thin, normal appearance. 4. In group 3, there was no evidence of hemorrhage or hemorrhagic tendency in the pericardium.
Ocular Manifestations of Sjögren Syndrome

Sjögren Syndrome (SS) is one of the most frequent systemic autoimmune disorders, mainly involving the eye and mouth due to inflammation of lacrimal and salivary glands. Exocrine glands affected with a typical focal lymphocytic infiltration potentially lead to dry eyes and dry mouth. In addition to the known pathogenic mechanism of SS through autoimmunity, corneal neuropathy, as a peripheral neuropathy which is a relatively frequent extraglandular systemic manifestation of SS, recently draws attention as a possible pathogenic mechanism of ocular symptoms and dry eye induction. The diagnostic criteria of SS changed recently, proposed by the American College of Rheumatology/Sjögren’s International Collaborative Clinical Alliance (ACR/SICCA) in 2012, and the ocular surface staining score is the only required test for ocular manifestation of SS. However, other diagnostic methods evaluating tear film status, though excluded from the new criteria, are still important for the staging and treatment planning, including direct observation of tear film, tear film break up time, Schirmer test, and measurement of the tear film levels of inflammatory mediators. Eye-specific symptoms and signs and ocular treatment options for SS including tear substitutes, secretogogue, topical anti-inflammatory therapy with corticosteroids and cyclosporine, punctal occlusion, autologous serum, and mucolytic therapy were summarized and discussed in this review article.
삼각피판법을 이용한 편측 불완전 구순열 환자의 구순 교정 수술-증례 보고

After Tennison introduced a triangular flap method which, for the first time, preserved the Cupidʼs bow, Randall gave this method a sound mathematical basis. This method is also called as an inferior triangular cheiloplasty which is characterized by making a small triangular flap from the lateral border of cleft destined to be fitted into an incision on the medial side of cleft. He postulated that the height obtained was equal to the sum of the median of the two triangles used in the cheiloplasty. Using this technique, a 22 month-old male patient with incomplete unilateral cleft lip was corrected primarily. The deviation of the columella and flattening of the nostril on the cleft side were minimal. The operation was done under general anesthesia and patient was healed uneventfully. We tried to improve the symmetry and esthetic feature of philtrum, nostril sill, alar-facial groove, preventing the notch formation on the nostril floor, and to reconstruct the muscle sling in the upper part of lip. The shape of Cupidʼs bow was restored, and the symmetry of columella was regained as a result. In summary, the inferior triangular cheiloplasty is effective to correct the primary unilateral cleft lip, results in the restoration of favorable anatomy and function.

Soft-tissue chondroma was very rare in incidence and thought to be benign. Recently, we operated upon a 13 year-old female with a chondroma of the middle mediastinum, which was incidentally detected in chest X-ray as mediastinal mass, measured about 10x8x7 cm in size and completely resected via thoracotomy.

Truncus arteriosus is one of the rare cyanotic congenital cardiac anomalies and thought to be result from complete or partial failure of trunco-conal septum. A single arterial trunk receiving blood from both ventricles supplies the coronary, pulmonary and systemic circulation. The symptoms were usually related to the degree of the pulmonary blood flow and functional status of truncal valve, and mostly appeared within the first two months of life. The prognosis is generally considered to be poor in spite of successful surgical correction. This report is a case of 13 years old female with type I truncus arteriosus, which was successfully corrected using a intracardiac Dacron tunnel graft[semilunar, 18mm] from VSD to the truncal valve, and a extra-cardiac lonescu-Shiley valved[20mm] Dacron conduit [21mm] from RV to the pulmonary artery[Rastelli operation].
