Hepatocellular carcinoma (HCC) is the third most common malignancy, with a new incidence of more than 11,000 cases per year and the second most common cause of malignancy-related death in Korean males. In Korea, more than 80% of all HCCs have developed from hepatitis B virus (HBV)-related cirrhotic livers. Liver transplantation (LT) is the only treatment that offers a chance of cure for HCC and the underlying liver cirrhosis simultaneously, but the availability of liver grafts and the aggressiveness of tumor recurrence are critical limiting factors of LT for HCC patients. The serious shortage of deceased-donors on strong demand for LT leads to the development of living-donor LT (LDLT) as a practical alternative replacing deceased-donor LT. Considering that HCC recurrence is the most common cause of posttransplant patient death, recipient candidates should be prudently selected through objectively established criteria. Uniquely, some Asian major LDLT centers challenged the Milan criteria, accepting a much higher number of HCC nodules instead of tumor size expansion. The eligibility criteria of LDLT for HCC are likely to be expanded more than before, but it still requires further qualified risk-benefit analyses. The development of new effective treatment modalities for HCC recurrence will reasonably expand the selection criteria further wide without the expense of recurrence rate. This article is mainly focused on the role of LT for HCC and discussed on the validity of currently available indication criteria. (Korean J Gastroenterol 2010;55:350-360)

본 연구는 액체의 유동과 광학적 특징에 대해 알아보고 높이 맵 기반 액체 시뮬레이션을 GPU를 이용하여 만화적으로 표현하는 것을 목표로 하고 있다. 현재까지 컴퓨터 그래픽스에서 기체와 액체에 대한 NPR적 표현 연구는 많이 진행되었다. 본 연구에서 유체의 유동을 비 물리학적 방법인 간단한 알고리즘을 사용하여 표현하였고, 액체의 광학적 표현에서 반사와 굴절 값의 비는 프레넬 방정식을 이용하여 구했다. 액체의 만화적인 표현은 기존의 방법과 달리 환경맵 상의 반사와 굴절 값을 분할했다. 외곽선 검출은 액체의 점프 에지(jump edge)만을 표현하였다. 그 결과, 액체 시뮬레이션을 만화적으로 표현 할 수 있었다. The goal of this study is to represent heightfield-map based cartoon liquid simulation using GPU. To do this, we examine the optical and flow features of liquid. So far, NPR represent study for gas and liquid advanced in computer graphics. In this study the flow of fluid represent using a trivial algorithm using a non-physical method, and optical of liquid represent ratio of value of reflection and refraction using fresnel equation. Cartoon represent of liquid different from the existing method, that the value of reflection and refraction on the environment map. Edge detection represent jump edge of liquid. As a result, the liquid simulation could represent a cartoon.
알콜성 간질환에 대한 간이식: 적절한 이식 시점과 임상경과

Alcoholic liver disease (ALD) is the second most common indication for liver transplantation (LT) in the Western countries. Although the nationwide research has not been performed, ALD may be regarded as 3rd common indication for LT next to HBV and toxic hepatitis in Korea. Alcohol abstinence remains the cornerstone in the management of ALD with the potential for improvement in liver histology. However, severe alcoholic (ALC) hepatitis resulting fulminant liver failure , decompensated ALC liver cirrhosis or combined HCC in ALD can be indicated for LT. In DDLT setting, donated organ is limited public resource which must be shared fairly and effectively. Therefore, recidivism could be considered a reason for transplant units and public opinion to deny transplantation. And pre-transplant sobriety has been considered essential prerequisite for organ allocation. However, recent studies reported that re transplant pattern of alcohol consumption including the duration of abstinence did not affect the risk of recidivism. Recent meta-analysis and systemic review demonstrated that the short- and long-term survival outcome of LT for ALD was comparable or better as compared with non ALD. Furthermore, the impact of recidivism is minimal in terms of long-term survival or morbidity rate unlike general belief. Therefore, a systematic perioperative psychosocial support rather than keeping 6-month abstinence rule should be emphasized to enhance the compliance, quality of life and long-term survival outcome of ALD patients.

Purpose: Opposing portal irradiation with helmet field shape that has been given to a patient with brain metastasis can cause excess dose in patient's scalp, resulting in hair loss. For this reason, this study is to quantitatively analyze scalp dose for effective prevention of hair loss by comparing opposing portal irradiation with scalp-shielding shape and tomotherapy designed to protect patient's scalp with conventional radiation therapy. Materials and Methods: Scalp dose was measured by using three therapies (HELMET, MLC, TOMO) after five thermo-luminescence dosimeters were positioned along center line of frontal lobe by using RANDO Phantom. Scalp dose and change in dose distribution were compared and analyzed with DVH after radiation therapy plan was made by using Radiation Treatment Planning System (Pinnacle3, Philips Medical System, USA) and 6 MV X-ray (Clinac 6EX, VARIAN, USA). Results: When surface dose of scalp by using thermo-luminescence dosimeters was measured, it was revealed that scalp dose decreased by average 87.44% at each point in MLC technique and that scalp dose decreased by average 88.03% at each point in TOMO compared with HELMET field therapy. In addition, when percentage of volume (V95%, V100%, V105% of prescribed dose) was calculated by using Dose Volume Histogram (DVH) in order to evaluate the existence or nonexistence of hotspot in scalp as to three therapies (HELMET, MLC, TOMO), it was revealed that MLC technique and TOMO plan had good dose coverage and did not have hot spot. Conclusion: Reducing hair loss of a patient who receives whole brain radiotherapy treatment can make a contribution to improve life quality of the patient. It is expected that making good use of opposing portal irradiation with scalp-shielding shape and tomotherapy to protect scalp of a patient based on this study will reduce hair loss of a patient.
