        우측 광대뼈에 발생한 중심 거대 세포 육아종 1예

        The central giant cell granuloma is a benign tumor seen generally in the mandible, but rarelyin other cranial bones. Herein, we present a 51-year-old man with central giant cell granulomain the right zygomatic bone. Physical and radiologic examinations of the central giant cellgranuloma in the zygomatic bone showed that specific and preoperative diagnosis is usuallydifficult. Therefore, clinicians should consider the possibility that central giant cell granulomamay occur in the zygomatic bone mimicking other more frequently observed lesions.

        OTP를 이용한 모바일 RFID 상호인증 프로토콜

        모바일 단말기와 RFID(Radio Frequency Identification)통신 기능을 결합한 모바일 RFID는 객체에 대한 정보 확인 및 관련 응용 서비스를 쉽게 이용할 수 있는 기술이다. 모바일 RFID는 기존 RFID와 마찬가지로 보안 기능이 취약해 많은 보안적 위협에 노출되어있다. 본 논문에서는 통신에 참여하는 각 요소들이 생성한 임의의 난수와 대칭키 암호화 알고리즘 OTP(One time Password)를 이용하여 보다 강력한 보안성을 갖는 상호인증 프로토콜을 제안한다. 제안한 프로토콜은 매 인증시 메시지가 변경되기 때문에 기존 프로토콜과 비교하여 스푸핑 공격 및 재전송 공격 등에 안전하다. Mobile RFID system, that consists of the existing RFID reader mounted on the mobile devices such as smartphones, is able to provide the users a variety of services and convenience. But security of mobile RFID system is too weak like the existing RFID system. In this paper, the mobile RFID mutual authentication protocol with high level of security is proposed to overcome the troubles such as cryptographic protocols in the existing RFID system responding with the same value in every authentication procedure and the exposure in the exchange of messages. The proposed protocol exchanges messages unexposed by using the random numbers generated in the mutual authentication between the tag and the reader and making numbers coded with the symmetric key. Besides, the protocol uses the mutual authentication utilizing OTP by considering the characteristics of the reader embedded in mobile devices in the mutual authentication process between the reader and the server. Because changed message in every authentication, which produces safe from spoofing attacks and replay attacks, etc.

        Lymphadenoma is a rare benign neoplasm of the salivary gland. Herein, we present a 67-year old woman with lymphadenoma in the parotid gland. Physical and radiologic examinations of parotid lymphadenoma are not specific and preoperative diagnosis is usually difficult. Therefore, clinicians should consider the possibility that lymphadenoma may occur in the parotid gland mimicking the other more frequently observed lesions.

        레벨2 장비로 전용가능한 레벨1 장비의 수면다원검사 자동화 판독과 검사자 판독 간의 비교

        Background and Objectives A lack of investigators for polysomnography has risen due toincreased demand since health insurance started to cover the cost of the test. We examined thereliability of the automated scoring of polysomnography, which has been deployed to improvethis imbalance. Subjects and Method We analyzed the data of 20 patients who underwent level 1 polysomnographyfrom April 1 to July 27, 2019. The software from Noxturnal (Nox Medical) was usedfor the scoring of the Polysomnography data. Each of the polysomnographic data was scoredboth by the automated scoring system and by a skilled technician. Results Twenty patients were analyzed. There was no significant difference between automatedscoring and manual scoring in sleep latency, apnea index, and rapid eye movementsleep stage ratio. However, the concordance rate of the sleep stage by epoch was 83.32%, andthere was a significant difference with regards to apnea-hypoapnea index (AHI) and respiratorydisturbance index (RDI). Two obvious errors were noted in the automated scoring that couldbe easily fixed; the failure to recognize wakefulness during sleep and the fragmentation of respiratoryevents. When two errors were corrected, many differences in polysomnography parameters,including AHI and RDI, were eliminated. Conclusion It showed 80% coincidence of epoch in the sleep stage between the automatedscoring and manual scoring. However, there was no difference in AHI and RDI when the fragmentedrespiratory events of the automated scoring were adjusted. Therefore, automated scoringis considered to be useful if only a little modification could be made.

        Intractable Methicillin-Resistant Staphylococcus Aureus Otorrhea with Silicone Impression in the Middle Ear

        We report a case of methicillin-resistant Staphylococcus aureus (MRSA) otorrhea with impression material of hearing aid in the middle ear. The patient had chronic otitis media in the right ear with sensorineural hearing loss in both ears. The silicone flowed into the middle ear through a tympanic membrane perforation during the process of making an ear mold. Several days after hearing aid fitting, the patient had severe otalgia, intractable otorrhea, aggravated hearing loss, and dizziness. The pus culture and sensitivity test revealed MRSA. After topical treatment using diluted vinegar irrigation and ototopical vancomycin solution, intractable otorrhea was controlled. The infected silicone impression was removed by canal wall-up mastoidectomy, and hearing was saved. We present here a review of the literature regarding silicone impression in the middle ear after hearing aid mold fitting.

        전음성 난청 환자에서 내시경을 이용한 시험적 고실개방술의 유용성

        Background and Objectives The aim of this study is to investigate the effectiveness of the endoscopic exclusive transcanalar approach for the management of conductive hearing loss. Subjects and Method This was a retrospective comparative study of 106 patients who underwent exploratory tympanotomy at Chonnam National University Hospital from January 2008 to March 2016. The subjects were classified into two groups; endoscopic tympanotomy (ET, n=26) and microscopic tympanotomy (MT, n=80). Demographic data, operation time, postoperative admission days, pure tone audiometric results of pre-operation and post-operation at 3 months, and hearing success rate were evaluated. Results The mean operation time of MT (101.7±24.8 minutes) was longer than that of ET (59.8±23.2 minutes) with a statistical significance (p<0.01). The mean postoperative admission days of MT (4.4±1.3) was longer than that of ET (2.0±0.6) with a statistical significance (p<0.01). Hearing gain (air-conduction) and air-bone gap improvements were not significantly different between the groups. However, the hearing success rate of ET [partial ossicular replacement prosthesis (PORP) 85.7%, total ossicular replacement prosthesis (TORP) 81.8%] was significantly improved enormously compared to that of MT (PORP 62.9%, TORP 64.3%). Conclusion With regards to conductive hearing loss, the endoscopic approach provided a better surgical view and less invasiveness compared with the microscopic approach. In conclusion, endoscopic exploratory tympanotomy had several advantages in operation time, admission days and hearing success rate. Korean J Otorhinolaryngol-Head Neck Surg 2017;60(1):7-12

        일반적으로 10㎓ 이상의 주파수 대역에서의 신호 전송 시 감쇠 요인은 대기중의 산소와 수증기에 의한 흡수와 강우, 눈, 우박, 안개, 구름 등의 자연적인 대기현상이 지배적이다. 이 중 가장 큰 영향은 강우로 인한 전파감쇠이다. 강우로 인한 전파감쇠는 강우강도에 따라서 감쇠정도가 다양하고, 강우강도는 공간적ㆍ시간적으로 그의 변동이 매우 다양하다. 따라서 강우강도에 의한 시간적ㆍ공간적 데이터의 확보와 정확한 강우강도의 추정은 정확하고 신뢰성 있는 링크설계를 위한 가장 중요한 요소이다. 본 연구에서는 서울을 포함한 9개 지역의 최근 5년간(2004~2008)의 디지털 강우강도 데이터를 확보하여 지역별 강우강도와 시간율의 관계를 경험적으로 분석하였다. In general, attenuation at frequencies over 10㎓ is mainly due to absorption by oxygen and water vapor, snow, and atmospheric gases. Among this factor rain become an important limiting factor. However, rain attenuation is highly influenced by rainfall intensity and it varies over time and space. In this paper, rainfall intensity eight measurement sites including seoul over recent 5 years is obtained and the regional between rainfall intensity and percent of time is analyzed.
