Cartilage contact mechanics is essential to understand osteoarthritis, but in vivo cartilage contact mechanics of the knee joint remains unknown. We investigated the influence of loading on in vivo tibiofemoral contact area changes and studied whether in vivo tibiofemoral contact area of subjects with knee osteoarthritis and healthy controls differs. Subjects with radiographic evidence of knee osteoarthritis and control subjects were tested. The coronal 3D-SPGR MR images were acquired under both unloaded and loaded conditions. Tibiofemoral cartilage contact areas were measured using image-based three-dimensional models. Acute loading on the knee joint significantly increased in vivo tibiofemoral contact area in both compartments. The in vivo tibiofemoral contact area in the medial compartment is significantly larger under loaded conditions. Compared with healthy controls, subjects with knee osteoarthritis have significantly larger contact areas under both unloaded and loaded condition.
