        술전 폐기능과 전폐적출술후 폐합병증과의 연관성

        Determination of preoperatibe pulmonary function is crucial in avoiding complications from pulmonary resection, especially pneumonectomy. Postoperative morbidity and mortality were correlated with the preoperative results of five widely used tests of pulmonary function in 40 patients who underwent pneumonectomy for bronchiectasis, pulmonary tuberculosis, and carcinoma of the lung. Factors analyzed following operation included 30-day mortality, the incidence of arrhythmia, the frepuency of respiratory complications, and the number of individuals requiring prolonged mechanical ventilation. There were statistically significant differences[p<0.001]in mean values among FVC, FEV1, FEV1/FVC and MVV. But the difference of the FEF25-75% was not statistically significant.

        식도 천공의 보존적 치료 - 임상적 고찰 14례

        Twenty-five patients with perforation of esophagus were managed at Pusan National University Hospital, from 1981 to 1993. The hospital course of 14 of these patients was evaluated with a special emphasis on the cause and location of perforation, clinical presentation, time elapsed from perforation to treatment, method of treatment, and outcome. Patients with anastomotic leak and cancer were excluded.The perforation was iatrogenic in 7 patients, spontaneous in 5, ingestion of foreign body in 1, and traumatic in 1. There were 7 cervical perforations, 2 upper thoracic perforations, and 5 lower thoracic perforations. Chest pain, fever, and dyspnea were frequent symptoms. Esophagography was most diagnostic [11 patients] but thoracentesis was of little diagnostic aid.Antibiotics were administered intravenously to all patients:hyperalimentation was accomplished intravenously in 11, and nasogastric suction was used in all cases. No patient required any surgical procedure, minor or major.

        심장판막질환에 대한 재수술의 임상연구

        During 6 year period from January, 1987 through December, 1992, a total of 17 valvular reoperations [14 mitral and 3 aortic valve reoperations] were performed in 15 patients with previously implanted prosthetic valves. There were 7 men and 8 women, mean age was 35.4 years. Mean time interval was 80 months. Causes for reoperations were prosthetic valve failure, prosthetic valve endocarditis, and periprosthetic leak.3 patients died in hospital. The cause of death were low cardiac output and acute renal failure.

        서울시 거주 대학생의 주거비 부담능력 분석

        The purpose of this study is to examine housing affordability of university student andanalyze the factors to affect university student's housing affordability in terms of characteristics ofuniversity and household, type of residence, income, and expenditure. This study took the ratio basedapproach for determining the university student's housing affordability. The multiple regression analysiswas used for analyzing factors to influence university student's housing affordability. And data wasconstructed by surveying the students of the University of Seoul and the Kyunghee University. Thisstudy shows that RIR index of the university student is measured as 0.398. So it means that the levelof the university student's housing burden is high compared with the ordinary households. The analysisof the factors affecting housing affordability shows that living in the dormitory or the type of jeonseihelps student get high housing affordability. And Seoul and Kyungki area's native student has highhousing affordability than the other area's native student. Also student's housing affordability is lowas there are lots of the living expense. In addition, the higher the age is or the more the roommatesare or the smaller the residential area is, the higher the university student's housing affordability is. 이 연구의 목적은 서울시에 거주하는 대학생의 주거비 부담능력을 파악하고, 대학과 가구, 주거,소득․지출 특성별로 주거비 부담능력에 영향을 미치는 요인을 분석하는 것이다. 대학생의 주거비 부담능력을 측정하기 위한 방법으로는 비율대비 접근법을 활용하였으며, 여기에 영향을 주는 요인을 파악하기 위해서는 다중회귀분석을 실시하였다. 또한 서울시립대학교와 경희대학교 재학생을 대상으로 설문조사를 실시해 데이터를 구축하였다. 연구 결과 대학생의 RIR지수는 0.398로 측정돼, 주거 부담 수준이일반 가구에 비해 높은 것으로 나타났다. 주거비 부담능력에 영향을 미치는 요인을 분석한 결과 기숙사에 거주할수록 주거비 부담능력이 높아지는 것을 확인할 수 있었다. 또한 전세 형태로 거주할수록 주거비 부담능력이 향상되는 것으로 파악되었다. 또한 수도권 외 지역 출신일수록 수도권 출신보다 주거비부담능력이 더 낮게 나타났으며, 생활하는데 꼭 필요한 생계비가 높을수록 주거비 부담능력이 낮아지는것으로 파악되었다. 그밖에 연령이 높을수록, 동거인 수가 많을수록, 주거 면적이 적을수록 주거비 부담능력이 향상되는 것을 확인할 수 있었다. 이 연구는 대학생의 주거비 부담능력을 파악하고 여기에 영향을 주는 요인들을 최초로 실증 분석했다는 점에서 의의가 있다.

        상하지 사이클 운동에 의한 에너지 소모량 및 보행 능력 비교

        Objective: To investigate the effect of cycling ergometry and to compare energy expenditure and walking performance after arm cycling with those after leg cycling in patients with brain diseases. Method: Twenty-two adults with brain diseases (6 stroke, 4 traumatic brain injury, 4 brain tumor, 4 parkinsonism, 4 cerebral palsy) were recruited as subjects. They were randomly assigned to disease-matched groups; arm cycling and leg cycling (n=11 each). VO2 (L/min), VCO2 (L/min), VE (L/min), O2 rate (ml/kgㆍmin), O2 pulse (ml/kgㆍbpm), O2 cost (ml/kgㆍm) and VO2 peak (ml/kgㆍmin) during cycling test or walking test, and walking performance were evaluated after cycling training for 4∼6 weeks. Results: Arm cycling exercise did not improve any parameters such as VO2, VCO2, O2 rate and O2 cost during walking test, whereas it increased VCO2, VE and O2 pulse during cycling test. In contrast, leg cycling significantly improved walking velocity and distance, and decreased O2 cost during walking test. It also increased all parameters including VO2 peak during cycling test (p<0.05). Conclusion: Leg cycling exercise improved walking performance and energy efficiency of walking as well as cardiorespiratory fitness relative to arm cycling. Therefore, leg cycling promoted lower-extremity task such as walking in patients with brain diseases.
