        Fentanyl 을 사용한 마취유도

        We evaluated the efficacy as an anesthetic induction agent and cardiovascular effects of fentanyl in sixty patients with myocardial ischemia on electrocardiogram, who were scheduled for elective operations of more than two-hour duration. In addition, the effects of pretreatment with pancuronium and/or lorazepam prior to anesthetic induction were evaluated on the induction speed, the incidence and severity of skeletal muscle rigity, heart rate, and systolic blood pressure. All the patients were given 30μg/kg of fentanyl and divided into four equal groups of fifteen patients according to pretreatment drugs administered before fentanyl. The results were as follows: 1) Eighty-seven percent of the patients in control group (pretreated with saline)were rendered unconscious during or one minute after infusion of 30μg/kg of fentanyl. 2) Pretreatment with lorazepam before fentanyl significant#ly increased the induction speed (p$lt;0.025), but was not significantly correlated with the incidence and severity of skeletaluscle rigidity. Pancuronium pretreatment significantly decreased the incidence and severity of skeletal muscle rigidity (p$lt;0.01) but was not significantly correlated with the induction speed, Pretreatment with lorazepam plus pancuronium significantly increased the induction speed (p$lt;0.025) and significantly decreased the incidence and severity of skeletal muscle rigidity (p$lt;0.01). 3) Saline pretreatment and pancuronium pretreatment did not significantly change heart rate or systolic blood pressure, but both lorazepam poretreatment and lorazepam-pancuronium pretreatment decreased heart rate or systolic blood pressure (p$lt;0.05), which were within thirty percent of preanesthetic values. 4) All the patients did not exhibit significant changes in heart rate or systolic blood pressure immediately after, in comparison to, immediately before tracheal intubation or skin incision (p$gt; 0.05). From the above results, it is concluded that anesthetic induction with 30 μg/kg of fentanyl produces profound analgesia and amnesi, stable cardiovascular dynamics, and a minimum of problems, and pretreatment with small doses of lorazepam and pancuronium before fentanyl facilitates not only a rapid loss of consciousness but also a reduction in the incidence and severity of fentanyl-induced rigidity.

        Pop-up satellite archival tag (PSAT) 체외부착에 따른 해산어의 생리학적 반응

        Recently, the pop-up satellite archival tag (PSAT) is being used in studies as a method of using satellites for monitoring organisms. Because PSATs are attached directly on the organism being monitored, it is used mostly on larger species since the health of the organism is an important concern. For this reason, PSAT-based surveys are lacking in Korea with no studies on the physiological responses of organisms with PSAT attached. Accordingly, as a basic biomonitoring study using PSAT, the present study investigated the physiological changes in fish in response to the attachment of PSAT. The present study used red seabream (Pagrus major) and Korean rockfish (Sebastes schlegelii) as the experimental fish. The PSAT was attached to the muscle below the experimental fish’s dorsal fin using a mono filament (n = 3). To investigate the changes in physiological responses according to PSAT attachment, blood samples were collected from all experimental fish, including the control (n = 3), at 1, 7, 14 and 21 days after the attachment. Upon blood sample collection, whole blood was used to measure hematocrit and hemoglobin levels. After separating the plasma, the separated plasma was used to measure the GOT, GPT, glucose, total protein, and cholesterol levels. Meanwhile, the plasma cortisol, superoxide dismutase, and catalase levels were measured using the ELISA method. The results showed that attaching the tag did not have any impact on the immunity and stress response of the experimental fish. The findings in this study also demonstrated the possibility of using PSAT for studying relatively smaller species living in the coastal waters of Korea.

        IMT-2000시장에서의 단말기보조금

        본 논문은 2003년 6월부터 서비스가 개시되는 IMT-2000시장에서 단말기보조금의 허용여부를 간단한 2기간 동태적 모형을 이용하여 이론적으로 분석하고 있다. 3세대 이동통신서비스인 IMT-2000시장은 사업자들의 시장진입순서와 번호이동성의 보장시점에 있어서 2세대 서비스인 이동전화와는 상당한 차이를 갖고 있다. 따라서 이동전화시장에서 시행되었던 정부의 단말기보조금정책은 IMT-2000시장에서는 대폭적으로 수정 보완되어야 할 것이다. 본 논문의 분석결과에 의하면, 소비자고착현상이 존재하지 않는 한 단말기보조금이 금지된 경우와 허용된 경우의 균형은 동일한 결과를 낳는다. 따라서 정부의 IMT-2000시장에 대한 정책의 기본방향은, 이동전화시장에서 정부가 취해 왔던 정책방향과는 달리, 단말기보조금은 허용하되 자원배분의 왜곡을 낳는 근본원인인 소비자고착현상의 완화에 두는 것이 사회적으로 바람직하다고 판단된다. In terms of number portability policy and sequence of entry, the 3rd generation IMT-2000 service market is quite different from the 2nd generation mobile phone service market. While a subsidy on mobile phone has been prohibited in the 2G market where lack of number portability has prevailed, it is an important issue whether a subsidy on the 3G terminal has to be prohibited. This paper, using a simple two period dynamic model, analyzes effects of subsidy on IMT-2000 terminal. According to this paper, allowing subsidy on IMT-2000 terminal does not decrease consumer and social surplus. On the other hand, prohibiting subsidy on IMT-2000 terminal may induce regulation avoidance efforts by service providers, which the government wants to regulate by introducing another regulation. This paper insists that the government should not prohibit a subsidy on IMT-2000 terminal but should focus on removing switching costs which locks consumers and reduces competition between service providers.

        현훈을 동반한 돌발성 난청 치료 중 발생한 세균성 뇌수막염 1예

        We report a case of sudden sensorineural hearing loss with vertigo in a 68-year-old woman, who developed bacterial meningoencephalitis during steroid treatment. The patient initially showed severe degree of sensorineural hearing loss on the left side with spontaneous nystagmus beating toward the contralateral side. Brain magnetic resonance imaging demonstrated no abnormal finding other than high signal intensity in parts of mastoid air cells and mild mucosal hypertrophy of the paranasal sinuses. During the course of steroid treatment, the hearing worsened to profound hearing loss, and on the 6th day of steroid treatment, the patient demonstrated dysarthria and disorientation with subsequent development of high fever. The patient was diagnosed with bacterial meningoencephalitis and treated with antibiotics. The patient recovered without any neurologic deficit but unilateral profound hearing loss persisted. The case is presented here along with a possible pathogenic mechanism of bacterial meningoencephalitis following sudden sensorineural hearing loss in this patient.         Simulation of Tokamak SOL and Divertor Region Including Heat Flux Mitigation by Gas Puffing

        Two-dimensional (2D), scrape-off layer (SOL)-divertor transport simulations are performed using the integrated plasma-neutral-impurity code KTRAN developed at Seoul National University. Firstly, the code is applied to reproduce a National Spherical Torus eXperiment (NSTX) discharge by using the prescribed transport coefficients and the boundary conditions obtained from the experiment. The plasma density, the heat flux on the divertor plate, and the D emission rate profiles from the numerical simulation are found to follow experimental trends qualitatively. Secondly, predictive simulations are carried out for the baseline operation mode in Korea Superconducting Tokamak Advanced Research (KSTAR) to predict the heat flux on the divertor target plates. The stationary peak heat flux in the KSTAR baseline operation mode is expected to be 6.5 MW/m<sup>2</sup> in the case of an orthogonal divertor. To study the mitigation of the heat flux, we investigated the puffing effects of deuterium and argon gases. The puffing position is assumed to be in front of the strike point at the outer lower divertor plate. In the simulations, mitigation of the peak heat flux at the divertor target plates is found to occur when the gas puffing rate exceeds certain values, ∼1.0 × 10<sup>20</sup> /s and ∼5.0 × 10<sup>18</sup> /s for deuterium and argon, respectively. Multi-charged impurity transport is also investigated for both NSTX and KSTAR SOL and divertor regions.
