        Clinical Decision Support Functions and Digitalization of Clinical Documents of Electronic Medical Record Systems

        Objectives: The objective of this study was to investigate the clinical decision support (CDS) functions and digitalization of clinical documents of Electronic Medical Record (EMR) systems in Korea. This exploratory study was conducted focusing on current status of EMR systems. Methods: This study used a nationwide survey on EMR systems conducted from July 25, 2018 to September 30, 2018 in Korea. The unit of analysis was hospitals. Respondents of the survey were mainly medical recorders or staff members in departments of health insurance claims or information technology. This study analyzed data acquired from 132 hospitals that participated in the survey. Results: This study found that approximately 80% of clinical documents were digitalized in both general and small hospitals. The percentages of general and small hospitals with 100% paperless medical charts were 33.7% and 38.2%, respectively. The EMR systems of general hospitals are more likely to have CDS functions of warnings regarding drug dosage, reminders of clinical schedules, and clinical guidelines compared to those of small hospitals; this difference was statistically significant. For the lists of digitalized clinical documents, almost 93% of EMR systems in general hospitals have the inpatient progress note, operation records, and discharge summary notes digitalized. Conclusions: EMRs are becoming increasingly important. This study found that the functions and digital documentation of EMR systems still have a large gap, which should be improved and made more sophisticated. We hope that the results of this study will contribute to the development of more sophisticated EMR systems.

        The main theme of this study is the ascendancy of the military and its influence over the national policy decision-making in North Korea. Many experts are proposing the possibility that the ascendancy of the military in North Korea, such as, ‘the military first politics’, Kim Jong-il’s predominantly military-related visits, the promotion of a large number of senior officers, and military officers’ high ranking in the power hierarchy, might be damaging the party’s control over the military. In order to verify this issue, especially, relations between the Worker’s Party of Korea(KWP) and the Korean People’s Army (KPA) in terms of the decision making process of national politics, this study analyzes the respective role of the main organizations of the KWP/KPA

        Factors Associated with Website Operation among Small Hospitals and Medical and Dental Clinics in Korea

        Objectives: The objective of this study was to investigate the factors associated with website operation among medical facilities. Methods: A cross-sectional study design was employed to investigate 1,519 hospitals, 33,043 medical clinics (MCs), and 18,240 dental clinics (DCs) as of 2020. The main outcome variable was analyzed according to technological, organizational, and environmental factors. Results: The percentages of small hospitals, MCs, and DCs with websites were 26.4%, 9.0%, and 6.6%, respectively. For small hospitals, the nearby presence of a subway station (odds ratio [OR] = 2.772; 95% confidence interval [CI], 1.973–3.892; p < 0.0001) was the only factor significantly associated with website operation status. Among medical and dental clinics, the percentage of specialists—MCs (OR = 1.002; 95% CI, 1.000–1.004; p = 0.0175) and DCs (OR = 1.002; 95% CI, 1.001–1.004; p = 0.0061), the nearby presence of a subway station—MCs (OR = 2.954; 95% CI, 2.613–3.339; p < 0.0001) and DCs (OR = 3.444; 95% CI, 2.945–4.028; p < 0.0001), and the number of clinics in the local area—MCs (OR = 1.029; 95% CI, 1.026–1.031; p < 0.0001) and DCs (OR = 1.080; 95% CI, 1.066–1.093; p < 0.0001)—were significantly associated with website operation. Conclusions: Clinics are critically affected by internal and external factors regarding website operation relative to small hospitals. Healthcare policymakers involved with information technologies may need to pay attention to those factors associated with website dispersion among small clinics.

        북한의 黨 주도 전쟁 수행체제의 兩價性과 대응전략: 치명적 대남 위협과 북한군의 취약성

        North Korea’s war against the South will be mainly performed by the party. In case of the war on the Korean Peninsula, the Supreme Commander, Kim Jungeun will control the party, which executes war in front. Therefore, North Korean military is only a part of organization in terms of war. This paper clarifies North Korea’s war as ‘the leader’s war, the party’s war, the party-organization’s war, and the revolution-base war. This war has critical strengths against South Korea in terms of the communist strategy on the peninsula, however, the weakness exists in the North Korean military, such as the possibility of weakness soldiers’ royalty for the party as well as the Supreme Commander. In this sense, recognizing and alarming the characteristics of North Korea’s party-leading war, and preparing reasonable and practical strategy against the North in case of the future Korean war will be very essential for our national security. 북한의 전쟁은 당이 주도하는데, 그 정점에 최고사령관이 있으며 당은 최고사령관의명령을 집행하는 위치에 있다. 당 주도의 전쟁은 북한의 총력전을 가능하게 하는 체제로서 당이 최고사령관의 명령에 따라 전략과 전술, 군의 전투력 유지, 모병, 그리고 후방지원 임무를 총괄한다. 본 연구에서는 당 주도 전쟁의 특성을 표현하기 위하여 수령의 전쟁, 당의 전쟁, 당조직망 전쟁, 그리고 혁명진지 전쟁이라고 규정하였다. 북한에서는 이러한 전쟁 수행을 위하여 해방 이후부터 북한군의 혁명사상과 당성을 강화하고 영도체계를 확립하는데 전력을 기울여 왔다. 북한의 당주도의 전쟁은 일반적인 군에 비해 지휘의 와해상황이 발생하는 취약점이 있으나, 전시에는 반면에 김정은이 살아 있고 북한군을 격멸한다고 해도당원이 존재하는 한 전쟁이 지속된다는 치명성이 존재한다. 따라서 북한군의 당주도 수행체제의 개념을 파악하고 우리의 체제 전반의 와해 공작을 병행하는 북한의 전쟁수행체제에 대한 철저한 대비가 필요하다.

        Association between Health Information Technology and Case Mix Index

        Objectives: Health information technology (IT) can assist healthcare providers in ordering medication and adhering to guidelines while improving communication among providers and the quality of care. However, the relationship between health IT and Case Mix Index (CMI) has not been thoroughly investigated; therefore, this study aimed to clarify this relationship. Methods: To examine the effect of health IT on CMI, a generalized estimation equation (GEE) was applied to two years of California hospital data. Results: We found that IT was positively associated with CMI, indicating that increased IT adoption could lead to a higher CMI or billing though DRG up-coding. This implies that hospitals' revenue could increase around $40,000 by increasing IT investment by 10%. Conclusions: The positive association between IT and CMI implies that IT adoption itself could lead to higher patient billings. Generally, a higher CMI in a hospital indicates that the hospital provides expensive services with higher coding and therefore receives more money from patients. Therefore, measures to prevent upcoding through IT systems should be implemented.

        Constructing a Real-Time Prescription Drug Monitoring System

        Objectives: The objective of this investigation was to demonstrate the possibility of the construction of a real-time prescription drug monitoring system (PDMOS) using data from the nationwide Drug Utilization Review (DUR) system in Korea. Methods: The DUR system collects information on drug prescriptions issued by healthcare practitioners and on drugs dispensed by pharmacies. PDMOS was constructed using this data. The screen of PDMOS is designed to exhibit the number of drug prescriptions, the number of prescriptions dispensed by pharmacies, and the dispensed prescription drug costs on a daily and weekly basis. Data was sourced from the DUR system between June 1, 2016 and July 18, 2016. The TOGA solution developed by the EYEQMC Co. Ltd. of Seoul, Korea was used to produce the screen shots. Results: Prescription numbers by medical facilities were more numerous than the number of prescriptions dispensed by pharmacies, as expected. The number of prescriptions per day was between 2 to 3 million. The prescriptions issued by primary care clinics were most numerous, at 75% of the total number of prescriptions. Daily prescription drug costs were found to be approximately US $50 million. The prescription drug costs were highest on Mondays and were reduced towards the end of the week. Prescriptions and dispensed prescriptions numbered approximately 1,200 and 1,000 million, respectively. Conclusions: The construction of a real-time PDMOS has been successful to provide daily and weekly information. There was a lag time of only one day at the national level in terms of information extraction, and scarcely any time was required to load the data. Therefore, this study highlights the potential of constructing a PDMOS to monitor the estimate the number of prescriptions and the resulting expenditures from prescriptions.
