Purpose: To report the efficacy of toric orthokeratology lenses in a patient with corneal astigmatism within 1.5 D having difficulty wearing spherical orthokeratology lenses because of limbus-to-limbus corneal astigmatism. Case summary: An 11-year-old boy who wanted to wear orthokeratology lenses had a refractive error of -4.75 D sph = -1.00 D cyl 180 in the right eye and -4.50 D sph = -1.25 D cyl 180 in the left eye. The corneal astigmatism was 1.19 D and 1.19 D, respectively, and limbus-to-limbus corneal astigmatism was observed in topography. After wearing spherical orthokeratology lenses, the patient complained of monocular diplopia and both lenses were found to be displaced nasally. Toric orthokeratology lenses were prescribed for centration of lenses and resulted in significant improvement of uncorrected visual acuity of 20/20, refractive error of +0.75 D sph = -0.50 D cyl 170 in the right eye and +0.50 D sph = -0.50 D cyl 160 in the left eye, and centration of lenses for the 12-month follow-up period. Conclusions: Toric orthokeratology lenses should be considered if spherical orthokeratology lenses are not effective even in patients having less than 1.5 D of corneal astigmatism because of limbus-to-limbus corneal astigmatism.

Rigid gas permeable (RGP) 렌즈는 각막난시 교정 효과가 우수하고, 각막의 산소 부족 현상을 일으키지 않고, 광학적으로 시력교정 효과가 좋으나 렌즈가 각막보다 작아서 움직임으로 인한 이물감 등 불편함과 주변부의 번져 보임 등으로 인하여 착용에 실패하는 경우가 종종 있다. RGP 렌즈 처방에서 성공률을 높이려면 환자 선택, 렌즈 선택, 정확한 처방과 사후 관리를 잘해 주어야 한다. A rigid gas permeable (RGP) lens is good for correcting corneal astigmatism without inducing corneal hypoxia. However, despite the optical advantage of the RGP lens for correcting visual acuity, there may be circumstances in which a patient may choose not to wear the RGP lens. For example, because the RGP lens is smaller than the cornea, it may cause a foreign body sensation due to lens movement and blurring, especially at night. Thus, the success of the RPG lens prescription depends on the selection of the proper candidates and lenses, with appropriate fitting and follow-up management by a doctor.

Among management options of keratoectasia after refractive surgery, Prescriptions of contact lenses such as soft contact lenses, rigid gas permeable lenses and scleral lenses are conservative, safe, economical and effective. Although wearing contact lenses itself is inconvenient and contact lenses have some disadvantages, contact lens fitting is very important role in correction of keratoectasia and visual results of it can be told without any invasive methods. Author describes methods of prescriptions of various contact lenses.
각막굴절교정학 콘택트렌즈가 근시 및 난시의 진행에 미치는 영향

Purpose: To investigate the clinical effects of orthokeratology lens wear on inhibition of the progression of Korean myopia and astigmatism. Methods: The authors reviewed out-patient records of 144 eyes of 74 patients wearing orthokeratology lenses. The cycloplegic refraction and keratometry before and after wearing the lens were compared. The 190 eyes wearing spectacles were included into the control group. We evaluated the relationship between orthokeratology lens wear and control group according to age, initial myopia, initial astigmatism, myopic progression, astigmatic progression and duration of orthokeratology lens wearing. Results: The mean cycloplegic refractive error of spherical equivalent was -3.36 ± 1.96 diopters (D) in the patients wearing orthokeratology lenses, the mean astigmatism was -0.86 ± 0.72 D, and the mean wearing period was 3.41 ± 1.5 years. The mean myopic progression was 0.25 ± 0.31 D in lens wearing, and 0.62 ± 0.39 D in glasses wearing, respectively. There were statistically significant differences between lens and glasses wearing group (t-test, p < 0.01). The mean astigmatic progression was 0.06 ± 0.22 D in lens wearing, and 0.15 ± 0.21 D in glasses wearing control group respectively, and the results also show statistically significant differences (t-test, p < 0.01). There were no relationships between two groups as for age, initial myopia, initial astigmatism and duration of orthokeratology lens wearing (t-test, p > 0.01). Conclusions: The orthokeratology lens was found to be effective in suppression of the progression of Korean myopia and astigmatism, compared with the glasses. The purpose of this study is to suggest useful basic data for planning and designing a large-scale take-out coffee shop in the future, based on the post-occupancy evaluation of the use in the 'P' take-out coffee shop that has been taken the coming generation near the university town. Especially, this study put emphasis on researching the preference and satisfaction of each floor level from users in the 'P' coffee shop. The results of this study are as follows. The second floor is the most preference floor in the users of the coffee shop that has 4 floor levels and has a great number of users. When the users select any floor, the most important factor is the noise. Also, there are the additional factors ; a prospect, a length between the tables, a distance from counter to floor and so on. The results show that it is necessary not only to improve the matter of the noise with various spatial elements but also to consider the method that users are able to be on the floors all over. These results provide useful data for planning and designing of a large-scale coffee shop in the future.
한국인에서 적절하게 처방된 가스투과경성콘택트렌즈 직경에 여러 안구계측치들이 미치는 영향

Purpose: To compare the differences of parameters of 9.3 mm and 8.8 mm rigid gas permeable lenses (RGP lenses) that were prescribed by trial lens fitting. Methods: Ninety-three eyes of 49 patients were prescribed RGP contact lenses (YK spherical lens, Lucid Korea, Bonghwa, Korea) by a single ophthalmologist at the same hospital. We analyzed the differences of parameters of 9.3 mm and 8.8 mm rigid gas permeable lenses (RGP lenses) that were prescribed by trial lens fitting. Steep keratometric value (Ks), flat keratometric value (Kf), and corneal astigmatism (Kast) were measured by keratometer. Simulated steep keratometric value (Sim Ks), simulated flat keratometric value (Sim Kf), simulated corneal astigmatism (Sim Kast), white to white corneal diameter (WTW), and eccentricity were measured by corneal topography. We also analyzed the differences of back optic zone radius (BOZR) and lens power between the two groups. Results: In this study, 40 eyes wearing 8.8 mm lenses and 53 eyes wearing 9.3 mm lenses were included. Keratometric value and eccentricity were significantly higher in the 8.8 mm lens group, while WTW and BOZR were significantly lower. Corneal astigmatism (Kast, Sim Kast) and lens power were not significantly different between the groups. Conclusions: RGP lenses with a smaller diameter were likely to be prescribed to patients with higher keratometric value, higher eccentricity, and shorter WTW, and the average BOZR of the prescribed RGP lenses with a smaller diameter was likely to be steeper. 본 논문은 에폭시 지지애자에 센서를 내장하여 활선상태를 감지할 수 있는 제품개발에 관한 것이다. 개발된 제품은 KS C 3835에 따라 7.2[kV], 12[kV] 및 24[kV] 전압용이다. 지지애자에 내장된 센서 재질은 부분방전이 발생하지 않도록 애자 재질과 동일하게 하였다. 또한 활선상태를 확인할 수 있는 현장용 및 원격용 전압표시장치를 개발하였다. 개발된 애자본체에 대해서는 부분방전, 충격내전압, 상용주파내전압, 굽힘 강도 등 성능평가를 실시하였으며 만족할 만한 결과를 얻었다. 전압표시장치에 대하여도 상용주파내전압, 충격 내전압, 진동성 서지, 온도 특성 등을 확인한 결과 만족할 만한 결과를 얻었다.
