전이성 척추 종양에서 시행한 고주파 열 용해술 후 척추 성형술과 방사선 치료의 비교

목적 : 전이성 척추 종양 환자에 대한 고주파 열 용해술 후 척추 성형술의 유용성에 대하여 알아보고자 한다. 대상 및 방법 : 2007년 3월부터 2009년 3월 까지 본원에서 치료한 전이성 척추 종양 환자 34명을 대상으로 하였으며, 고주파 열 용해술 후 척추 성형술을 시행한 17명과 방사선 요법을 시행한 17명에 대하여 비교 연구하였다. 두 군 간의 통증의 경감과 생활 능력 수준은 Visual analog scale(VAS)과 Roland Moris Questionnaire(RMQ)를 이용하여 평가하였다. 결과 : 고주파 열 용해술 후 척추 성형술을 시행한 군은 9/17명이, 방사선 치료를 시행한 군은 8/17명이 사망하였다. 고주파 열 용해술 후 종양의 부피에 대한 괴사의 정도는 28-100%(평균, 61.2±22.4%)로 관찰되었다. 통증 경감의 정도는 고주파 열 용해술 후 척추 성형술을 시행한 군의 술 전, 술 후 1주 및 4주의 VAS 점수는 7.12, 3.82 그리고 3.65, 방사선 치료를 한 군에서는 7.00, 5.39 그리고 4.94로 두 군 모두에서 치료 후 1주와 4주에 의미있는 감소를 보였다(p-value <0.001, <0.001), RMQ 점수는 수술을 시행한 군에서 12.94에서 7.18로 개선되었으며, 방사선 치료를 한 군에서 14,11에서 10,89로 개선되었다. 하지만 통증의 경감(VAS 점수, p-value for interaction 0.004)과 삶의 질 개선 측면(RMQ 점수, p-value for interaction 0.024)에서 고주파열 용해술 후 척추 성형술을 시행한 군에서 방사선 치료를 시행한 군보다 통계학적으로 의미있게 더 나은 결과를 보였다. 결론 : 고주파 열 용해술 후 척추 성형술은 전이성 척추 종양 환자에서 통증 경감과 보다 나은 삶의 질을 가져다주는 유용한 치료법으로 생각된다. Purpose : To evaluation the availability of combined treatment with radiofrequency (RF) ablation with vertebroplasty in patients with metastatic spine tumor. Materials and Methods : Between March 2007 and March 2009, 34 patients with the metastatic spine tumor, RF ablation with vertebroplasty was performed in 17 patients, and radiotherapy in the other 17 patients. Cumulative l-year patient survival rate were compared with the both groups. Pain relief and quality of life were evaluated with use of the visual analogue scale (VAS) and Roland Moris Questionnaire (RMQ). Results : Nine of 17 patients with the RF ablasion therapy with vertebroplasty and 8 of 17 patients with radiotherapy were died. Tumor necrosis was observed in 61.2±22.4% (range 28-100%) of the tumor volume. In terms of pain relief, VAS score of preoperative, 1 week and 4 weeks following were 7.12, 3.82 and 3.65 in RF ablation with vertebroplasty group, and 7.00, 5.39 and 4.94 in radiotherapy group(p-value <0.001, <0.001). The score of RMQ score was improved from 12.94 to 7.18 in operation group (p-value <0.001). and 14.11 to 10.89 in radiotherapy group(p-value 0.001). But in terms of pain relief (score of VAS) and quality of life (score of RMQ). RF with vertebroplasty group showed significantly better than the radiotheray group (VAS, p for intercation 0.004, RMQ, p for interaction 0.024). Conclusion : RF with vertebroplasty is thought to be a useful method to improve the pain relief and quality of life in patients with metastatic spine tumor.

We present a case of angiomyofibroblastoma of the vulva in a 32-year-old woman. The asymptomatic lesion measured 4×3cm and appeared as a gray-white myxoid mass on cut sections Histologically, it appeared as a relatively well-circumscribed spindle cell proliferation with alternating hypercellular and hypocellular areas and proliferation of thin-walled blood vessels. Immunohistochemically, the stromal cells expressed vimentin, CD34 and progesterone receptor, but not desmin, α-smooth muscle actin, S-100 protein and estrogen receptor.
