Ossifications of the ligamentum flavum(OLF) and the posterior longitudinal ligament(OPLL) are uncommon clinical entities as a cause of the progressive compression myelopathy or radiculopathy. Although there are considerable literatures concerning OPLL or OLF in cervical and thoracic spine, there are only a few references about OPLL or OLF in the lumbar spine. OLF and OPLL have been reported that they may lead to severe complication only with a minor trauma or even without trauma, such as paraplegia. The authors have experienced 2 cases of OLF accompanied by OPLL in the lumbar spine, who were treated with decompressive laminectomy and excision of ossified ligaments. For its rarity of OLF and OPLL in the lumbar region, we report here with review of literature.

Summary of Literature Review : Many authors have described the association between lumbar disc degeneration and segmental instability but it has not been delineated in detail. Objectives : To compare the MRI assessment of disc degeneration with the conventional plain Xray evaluation of the intervertebral disc, in order to study lumbar segmental instability. Materials and Methods : In 75 patients with low back pain and/or sciatica, we analyzed disc space height, angular displacement, and horizontal displacement on plain radiographs of the lumbar spine. These parameters were compared with the grade of disc degeneration as evaluated by magnetic resonance imaging. Study Design : We analyzed the association between grade of disc degeneration as evaluated by magnetic resonance imaging assessment and segmental instability as evaluated by plain radiographs of the lumbar spine. Results : Disc space height was decreased in proportion to the grade of disc degeneration. Angular displacement was increased according to the grade of disc degeneration, but significantly less with severe degeneration, accompanied by a tendency to stabilization of the motion segment. Horizontal displacement was not correlated with the grade of disc degeneration. Conclusions : The incidence of lumbar segmental instability increased in proportion to the grade of disc degeneration but significantly decreased with severe disc degeneration.

Congenital deficiency of the anterior cruciate ligament (ACL) is a rare disorder that has been reported in association with other knee dysplasia like as congenital knee dislocation. congenital short femur, congenital absence of menisci, congenital ring meniscus, and thrombocytopenia-absent radius syndrome. There has been no published explanation about the etiology of bilaeral ACL deficiencies without other abnomality. The patient of congenital ACL deficiency must be carefully inspected about combined anomaly. Those efforts may be helful in treatment or ACL deficient patients and evaluation of pathophysiology or ACL deficiency. However there has not been a ruptured congenital deficiency of the ACL without other dysplasia or the knee and other congenital skeletal abnomalities. We reported a case of symptomatic bilateral congenital deficiencies of the ACL which have not been associated with other skeletal abnormalities.
장골의 외골종에서 속발한 연골육종 : 증례보고 A Case Report

This is a case, in a 27 year-old female, of secondary chondrosarcoma originating from a solitary exostosis of the central portion of the left iliac bone. Chief complaints were mild pain and a new hard growing mass which was just proximal to the left hip joint. Roentgenological study revealed a solitary exostosis with a pedicle which grew out from the cen?tral portion of the left ilium. Surgical en bloc excision in rectangular shape was performed, including the left iliac crest, under the clinical suspicion of chondrosarcoma since it had begun to grow with new activity after her growth cessation. Pathological diagnosis revealed an exostosis and there was no evidence of malignancy. With recurrence of the tumor, left hemipelvectomy was advised, under the clinical diagnosis of secondary chondrosarcoma, but the patient refused. Two other surgical excisions were performed and pathological diagnosis revealed secondary chondrosarcoma. A total of 6000 rad of radiation therapy was done. A pulmonary metastasis was noticed 4 months after the 3rd operation. Clinical findings were very important in the diagnosis of malignant change of the tumor.
Pulmonary Hypertrophic Osteoarthropathy : 1例報告 A Case Report

Hypertrophic osteoarthropathy is a syndrome, a triad consisting of clubbing of the fingers and toes, periosteal bone formation of the shafts of the cylindrical bones of the extremities, and a type of degenerative joint disease. When all these are present they are spoken of as Marie's sign group, after the observer who first drew attention to the disease in 1890. It is almost always associated with visceral disease, usually situated within the thorax. This is a case of hypertrophic osteoarthropathy having a symptom triad of a 61 year old woman. X-ray study revealed a typical changes in all cylindrical bones.
