Size and Retention of Tongue Bulb for Tongue Retaining Device

In several treatment modalities for snoring and obstructive sleep apnea (OSA), oral appliances mainly including mandibular advancement appliance (MAA) and tongue retaining device (TRD) are recognized as a non-invasive, reversible alternative with favorable results. Tongue bulb is a major component of TRD which prevents the tongue from approaching the posterior wall of the pharynx and can be combined with MAA. Determination of tongue bulb size for the patient is important for therapeutic effect, but frequently needs time-consuming work. For effective fabrication and standardization of tongue bulbs, this study aimed to categorize tongue bulb size for healthy young men and to examine its relation with maximum retention force and with physical parameters including tongue-related variables. 36 non-snoring, asymptomatic young men with normal occlusion were voluntarily participated in this study (mean age: $24.47{\pm}2.58$ years). Experimental procedures consisted of prefabrication of tongue bulb set (20 types with a width of 27-36mm and thickness of 8 and 10 mm), determination of tongue bulb size and the maximum retention force for each subject, and measurement of physical parameters including body mass index (BMI), neck circumference and width, thickness and length of tongue. This study showed that there was significant difference of retention force among the bulb size-related groups both in upright and supine position (p<0.05) and that retention force increased with bulb size. Correlation of tongue bulb size with physical parameters was not clearly verified and there was no significant difference in retention force between upright and supine positions. Based on our results, it can be suggested that retention force relates with tongue bulb size, ultimately with tongue volume. A further study needs to be performed in the patients with snoring and OSA.

In present day, components of Artemisiae Carpillaris Herba(A.C.H) was investigated by HPLC method. On condition of ODS-10column, 25% acetonitrile for mobile phase, 254nm of UV Detector, the extracts of A.C.H was analyzed. As the result, peaks of coumarin derivatives, at retention time 2.16 and 2.18, were found.
십이지장 과형성 용종의 Dieulafoy양 병변에 대한 내시경적 Hemoclip 치료 1예

Dieulafoy's lesion (DL) is an uncommon but important cause of massive upper gastrointestinal bleeding that has been reported to be involved in 0.3∼6.7% of cases of major gastrointestinal bleeding. The lesion usually occurs in the lesser curvature of the stomach within 6 cm of the gastroesophageal junction. Similar lesions have also been observed in the esophagus, duodenum, small intestine, colon, and rectum. Most DLs encountered in the duodenum occur in the bulb. Recently, with the advances in endoscopic techniques, the successful management of DL has been achieved through the application of a hemoclip or elastic band ligation. In particular, the application of a hemoclip is considered to be a safe and effective treatment for DL located on a relatively narrow and thin walled canal such as the duodenum. We report the successful application of endoscopic hemoclipping for the treatment of a rare Dieulafoy-like lesion on a hyperplastic polyp in the 2nd portion of the duodenum. (Korean J Gastrointest Endosc 2007;34:329-333) Dieulafoy 병변은 위장관 출혈의 드문 원인이며, 위장관 출혈의 0.3~6.7%로 보고되고 있다. 대부분의 경우 위 식도 접합부 6 cm 이내의 위의 근위부에서 발생하고, 식도, 십이지장, 소장, 대장 및 직장 내에서도 드물게 발견되는 것으로 보고되고 있으며, 십이지장에서는 십이지장 구부가 대부분을 차지한다. 최근에는 내시경 치료술의 발달로 hemoclip을 이용하거나 고무 밴드 결찰술을 이용하여 좋은 치료 효과를 거두고 있는데특히, hemoclip의 경우 십이지장과 같이 상대적으로 장관벽이 얇고 내강이 좁은 경우에 대해 안전하고 효과적인 치료로 보고되고 있다. 저자들은 흑색변과 토혈을 주소로 내원한 47세 남자 환자에서 십이지장 제2부 근위부의 과형성 용종 위에 발생한 Dieulafoy양 병변을 발견하고 이를 hemoclip 요법을 통해 성공적으로 지혈하였으며, 이와 유사한 Dieulafoy양 병변의 보고가 드물기에 문헌 고찰과 함께 이를 보고하는 바이다.

This paper is a result of a case study about landslides at Whacheon area in Kangwondo occurred during heavy rainfall in 2006. A-day-accumulative rainfalls from July 12 to July 13 and July 15 to July 16 were 120mm and llOmm respectively. Five sites at which slope failures occurred were visited to figure out main causes of slope failures by investigating characteristics of rainfall, geological forrnation, topography and ground surface exploration around the boundary of the landslides. Based on the site investigation characteristics of landslide with respect to rainfall pattern, geological and topographical condition and pattern of landslide were evaluated.

This paper is to investigate the characteristics of debris flow landslides in Gangwon Province through literature review, data collection and analyses and site investigation. As results of data analyses about landslides occurred currently in this province, the landslide in the form of debris flow is found to be 55 %. Therefore major loss and costs are caused by discharge of soil and rock fragments from landslide. From results of analyzing the geometrical characteristics of landslide, length of most of landslide is less than 200 m, their width is in the range of 10 - 40 m, most of them are know to be occurred in lower elevation than 400 m. Slope angle is in the range of 25 - 35 degrees. Compahng the period of rainfall intensity with the time of landslide being occurred, occurrence of landslide is quite related to duration of a heavy rainfall. For measures of controlling water flow discharge and debris flow, considering geological and topographical ground conditions, appropriate selection and building check dam, erosion control dam and ring net is very beneficial for reducing the loss and costs caused by the landslide of debris flow.
