Tobramycin의 家兎 硝子體內 注入에 依한 網膜의 組織變化에 關한 硏究

Bacterial endophthalmitis caused by intraocular surgery or penetrating wounds has long been a severe ophthalmologic problem. For several decades, antibiotics have been directly injected into the vitreous body as the treatment of endophthalmitis. Since then in order to come up with a treatment that would not cause toxic effect on the retina, experimental reports on many different effect of antibiotics on morphological changes and its safe doses have been documented today. Tobramycin, which is two to four times more effective than gentamicin against Pseudomonas strains but less toxic, has been studied and experimented on rabbits or human retina by many investigators with ophthalmoscope, electroretinogram or light microscope. But these reports were not adequate for fine histological findings since the tissue were embedded in paraffin, and fine structural changes is not known well. Author, by using 84 colored mature rabbits as experimental animals that tobramycin were injected into their vitreous bodies, observed gross findings seen through out the days 1, 2, 4, 7, 14, 28 and 60. Also in order to observe the fine structures of retina, semithin and thin sections were performed after Epon embedding, then studied with both light and electron microscopic observatiou. The following results were obtained: 1. A distinguishable structural changes were not observed in groups injected with 50㎍ of tobramycin. 2. Tissue changes: As time passed, through out the whole layer of retina, vacuolar degeneration in cytoplasm of nerve fiber layer, inner plexiform layer, ganglion cell and inner neuclear layer cells were seen. Also vacuolar changes within outer plexiform layer, karyolysis of outer neuclear layer, appearence of phagocytes occured, clumping of pigment granules of pigment epithelium, and lipoidal degenerations were seen. 3. Electron microscopic observation: Enlargement of the mitochondria and endoplasmic reticulum in the ganglion cells and the cells of inner and outer nuclear layer, especially in the outer segments of the visual cells, and disappearance of the mitochondrial cristae and in the pigment cell there were phagosomes that engulfed outer segments of visual cells were seen.
5-Fluorouracil이 鷄胎眠球 成長에 미치는 影響에 關한 硏究

The author has studied the effcet of 5-fluorouracil on the development of the eye-ball in the chick embryo, which is recently drawing attention in the aspect of its relation to the growth inhibition and malformation as anticarcinogenic agent. On the sixth day of incubation, 0.25mg and 0.50mg of 5-fluorouracil were injected into the chorioallantoic membrane each incubated egg of two groups, and the egg was broken for weighing the eye-ball on the day of 12th, 14th, 16th, and 18th. The effects of doses on the growth were analyzed by the analysis of variance method. The formulas applied to determine the specific growth parameters : y=a+bt+ct^2, y= bx^α and relative growth and the results thus obtained were summarized as follows : 1. There was significance of growth quantity between control and injected groups, and between 0.25mg and 0.50mg injected groups (P<0.005). 2. The growth rate was decreased with development in all groups. 3. The specific growth rate of all groups showed tendency of decrease. The order of the tendency is 0.25mg, 0.50mg injected group and control group. 4. The growth centers of the growth gradient in all groups were the eighteenth day of incubation 5. The coefficient of relative growth in injected groups to control group showed positive allometry. 6. When injected with 5-fluorouracil, eye-balls in the chick embryo were observed with remarkable growth inhibition. The effect in the group injected With 0.50mg was stronger than that in the group injected with 0.25mg.

The author investigated on the ratio of central serous retiopathy(C.S.R) cases among total out patients, the distribution of age and sect, and sites 7f affected eye in 214 crises of C.S.RThe author investigated on the ratio of central serous retinopathy(C.S.R.) cases among total from March 1st 1977 to February 28th 1981. Fluorescein angiography was performed in 132 eyes of 108 cases among them. Each type according to the leaking pattern on fluorescein angiography were characteristically claasified as followings : Tyde A; Leaking point is unremarkable (C.S. R. clinically) . Type B; Leaking point appears at early arterial phase and increases in density gradually but the size of leaking area is not enlarged. TyPe C; Leaking point appears at early arterial phase increases concentrically in its size and density. Type D; Leaking point appears at early arterial phase and increases vertically(mushroomshaped) in its sixte and density. Type E; Leaking point appears at early arterial chase and gradually fades out(scar fo-rmation) . The results were obtained as follows : 1) The eyes of C.S.R were observed in 0.83% among total out patients. 2) As to the distribution of age. 5 th decade twas moat frequantly affected in 37.3%. 3) As to the distribution of sex. male baa observes in 80.8% 4) Among 214 cases, unilateral involvement was observel in 85.1 % without difference of both eyes and binocular involvement was observed in 14.9 % . 5) Type C in leaking pattern was moet frequantly observed in 25.8% 6) A- single leakage-nab-observed in-59.1 % of all eyes. 7) The site of leaking point of fluorescein was found most frequantly in the upper area. 8) The direction in leaking pattern was most frequantly upward in type C, and outward in type D. 9) The detached sixte of type C and D were more larger than other types. 10) Comparing the onsets of each type A,B,C and D could be seen within 3 months, and type E could be found over 3 months. 11) In general, the visual acuity was severely disturbed in type D rather than the other types. And the visual disturbance was remarkable in the case of large serous detachment.
